Chemical peels use a variety of ingredients to exfoliate the skin to reveal a more rejuvenated complexion. TCA peels are popular for both at-home and professional use. DIY versions are sold at lower concentration ranging from 5-15% while higher concentrations of 20-35% should be used only by licensed providers. TCA peels have been performed at even higher concentrations but were associated with a greater risk of scarring. Some plastic surgeons and dermatologists recommend that regardless of concentration, chemical peels should be done in an office setting to ensure proper application and depth of penetration. Otherwise, patients may accidentally cause severe chemical burns to the treated area. 

Once a patient is determined to be a good candidate for a TCA chemical peel, they will need to complete any pretreatment skincare regimens, such as a daily tretinoin cream to reduce healing time or a series of lighter chemical peels to prepare the skin for a deeper exfoliation. On the day of treatment, the patient may require general, local, or no anesthesia depending on the depth and concentration of the chemical peel. The customized solution will be applied to the face, with multiple layers applied to areas of concern such as deep wrinkling and uneven pigmentation. The solution will be left on the skin until a frosting affect occurs. The provider will determine the depth of exfoliation visually, by observing the progression of skin frosting. Once the desired depth of exfoliation has occurred, the chemical solution will be neutralized and removed from the skin. A soothing lotion or serum may be applied after treatment.

TCA Peel

A skin resurfacing technique that causes the skin to exfoliate and peel to enhance its appearance.

The ideal chemical is chosen for the desired depth of penetration. The skin is thoroughly cleansed prior to the administration of the solution. The chemical solution is then applied to the desired areas of treatment. The candidate is closely observed for changes to the skin. Whitening of the skin is monitored to assess epidermal damage and is a sign of epidermolysis. As the peel progresses to a frosted appearance this indicates a deeper depth of penetration into the dermis.

Once the appropriate amount of time has elapsed, the solution is then neutralized with another agent such as sodium bicarbonate. Some peels do not require neutralization, as the skin naturally neutralizes them. The skin is then cleansed. Various dressings such as silicone may be applied, ointments or emollients can be utilized after the peel has completed to form a protective barrier. This is more common for deep peels and is typically not necessary for more superficial peels. Keep in mind that the depth of peel is determined not just by the chemical composition or strength of ingredients, but also by the length of time it is left on the skin.

There are many chemical peels available that range from superficial to deep. The choice depends on the desired depth of penetration, treatment effect, and recovery time. Overcrowded teeth can cause one's smile to look cramped, while gaps can make the teeth appear small. Both conditions can be embarrassing and make an individual self conscious about their smile. Teens and adults alike may postpone fixing their teeth especially if they have already experienced traditional metal braces that are easily noticed and hard to conceal. However, modern orthodontic systems such as lingual braces and clear aligners have made adult braces a more practical solution to crooked smiles. 

Invisalign is one of the most well-known clear aligner programs that uses a progressive system of trays to gently reposition the teeth into a straighter, more aesthetically pleasing smile. A visit to an orthodontist will be necessary to determine if your dental concerns can be corrected by Invisalign. Patients with more severe issues who also require jaw realignment may better benefit from a more aggressive orthodontic treatment. 

A 3D scan will be taken of the teeth in order to create a treatment plan. Additional x-rays and images may be taken during your appointment as well. It will take 3-4 weeks to create the first tray. Once the tray arrives, patients will return to the orthodontist's office to ensure proper fit. Trays will be worn for a period of 2 weeks and should only be removed for eating and brushing the teeth. For optimal results, trays should be worn for 20-22 hours daily. Certain Invisalign systems will have a blue indicator dot on the upper back molars that will gradually disappear with wear, easily notifying your provider if you are wearing your Invisalign tray for the required time. After 2 weeks, a new tray will be worn. Trays will be replaced every 2 weeks for the duration of treatment which typically lasts 10-15 months but can be as short as 3 months or as long as 2 years.

Much like other orthodontic systems, a visit to the orthodontist is first needed to determine if your individual case can be corrected with the ClearCorrect system or if a more aggressive treatment is needed to adjust the position of the jaw. A scan will be taken of the teeth and mouth which is sent to ClearCorrect in order to create the custom trays used to adjust the smile. Once the tray is created, it will be sent to your orthodontist's office as will subsequent replacement trays. Patients will go to their orthodontist to ensure proper fit of the tray which should be worn for a period of 2 weeks before it is exchanged with the next aligner in the treatment series. Trays will be worn and replaced every 2 weeks for the duration of treatment which will last several months to a few years depending on the needs of the patient.

ClearCorrect

ClearCorrect is an alternative orthodontic treatment to traditional braces like traditional metal braces, Damon™ Braces, lingual braces, and ceramic braces for correcting imperfections of the teeth and bite.

Many adults who wore braces as children still suffer from crooked teeth, especially when proper aftercare was not stringently adhered to. Thigh lifts are a common procedure for individuals who have lost a significant amount of weight and as a result have excess skin in the upper leg area. Thigh lifts can tighten and contour the inner and outer thighs, not only improving the shape of the leg but also reducing the appearance of cellulite and stretch marks. It is common for a thigh lift to be combined with other procedures such as an abdominoplasty or butt lift, sometimes referred to as a lower or 360 body lift. 

Patients will be put under general anesthesia for the procedure. Incision patterns for a thigh lift will vary depending on the needs of the patient. One technique used for a thigh lift places incisions in the groin, extending downward and wrapping around the back of the thigh. Another technique removes excess skin from the groin down to the knee using an incision along the inseam. Some patients may qualify for a minimal incision inner thigh lift that involves an incision only in the groin area. Patients who desire outer thigh definition may have an incision that extends from the groin, around the hip, and extending onto the back. Liposuction may be performed before skin removal if there is excess fat in the hip and thigh area for improved contours once the skin is repositioned. Redundant, sagging skin will be removed and deep sutures within underlying tissues help to form and support the newly shaped contours. 

Additional sutures and skin adhesive tapes will be used to close the skin incisions. Patients will be wrapped in compressive garments after surgery. Smoother, tighter contouring is apparent almost immediately, although results will be initially obscured by swelling and bruising.

A needle is inserted in a curved direction in the area to be lifted. The depth of the needle is maintained at the subdermal level. A similar needle is passed approximately 1-1.5 cm above this needle. Bi-directional threads made of polypropylene are inserted through the needles. The barbs along the thread act as cogs to grasp, lift and suspend a facial area. The needle is slowly withdrawn and the tissue is 'gathered' over the thread. Once the desired effect has been reached, the long ends of the thread are then cut off.

The sutures dissolve over the next 6 months leaving no residual material present since they are broken down by a process called hydrolysis. The new collagen and fibrous tissue that had formed around the sutures then continue to hold the skin up and tighten the skin.
