Icon 2940 is an ablative fractional laser that is part of the Palomar Icon aesthetic system. The Palomar Icon system has a variety of hand pieces that can be used for skin resurfacing treatments. The Icon 2940 hand piece is effective in treating wrinkles, fine lines, age spots, brown spots, and volume loss due to natural aging and environmental factors such as sun exposure. 

The Icon 2940 is a wounding laser which ablates damaged skin and causes the treated areas to regenerate healthier skin throughout the healing process. The incisionless otoplasty, also called a scarless otoplasty, utilizes sutures placed discreetly behind the ear to manipulate the cartilage to a more aesthetically pleasing position. Most patients who desire changes in their ear position will not be a candidate for this minimally invasive procedure as it can only accomplish mild adjustments and is best suited for children and teenagers. 

During the procedure, only local anesthesia is needed. Younger patients may not be able to tolerate sitting still for long periods of time and may require general anesthesia in order to reduce anxiety and allow the surgeon to complete the procedure. Once thoroughly anesthetized, the procedure will begin. Using a small needle, a puncture will be made in the front of the ear on the antihelical fold. The needle will be used to scrape and score the ear cartilage for easier advancement and encourage scar production postoperatively. Next, sutures will be placed in a rectangle pattern on the back of the ear, skillfully manipulating skin and cartilage to pull the ear closer to the head. Depending on the degree of protrusion, several sutures may be needed in order to create the desired effect. 

A wrap will be applied to the head postoperatively. Most patients are recovered within 7-10 days and return to full activity within 3 weeks.

Level 1: sometimes referred to as ‘shy nipple,’ Level 1 is the most minor level of inversion. It is assigned when the nipple responds to stimulation — it may protrude temporarily before returning to an inverted position.

Level 2: the nipple remains inverted at all times, but can be pulled out without too much resistance.

Level 3: this is the most severe level of nipple inversion. The nipple will remain inverted despite attempts to stimulate it. This is usually a result of scarring or tissue constriction.

Patients who are having inverted nipple surgery in order to breastfeed can have a procedure designed to partially preserves the milk ducts. Inverted nipple repair with partial preservation of milk ducts usually involves a local anesthetic and a sedative before surgery. An incision is typically placed around the areola (the dark-skinned area surrounding the nipple), followed by the nipple and areola tissue being lifted from the breast and stitched into a new shape. The lifted nipple and tissue is not disconnected from the breast with this technique.

Inverted nipple surgery with milk duct detachment is typically reserved for patients who are not planning to breastfeed. The procedure can be performed using a local anesthetic in combination with a sedative. An incision is made at the base of the nipple, detach the milk ducts, and reshape the nipple for the desired results.

Both inverted nipple surgery techniques generally take only about an hour to complete on an outpatient basis.
