During this type of procedure, patients will undergo a facelift while still technically awake. The operation will still reduce signs of aging by flattening folds of skin between the cheek and upper lip and treating the sagging of the cheek and the jowl, however the patient will not receive general anesthesia. As opposed to a traditional SMAS facelift, a surgeon will only utilize tumescent anesthesia and an oral or IV sedative. To begin the procedure, hair is removed 3cm away from the hairline so that a lateral incision can be made higher into the head. This exposes and creates better access to deeper regions into the upper and middle third of the face. Then much like the SMAS incision, skin is dissected from the muscle to allow for manipulation of the tissue. The facial muscle will then be stretched into a new position that will create a tighter more rejuvenated look for the patient. The surgeon will focus on the cheeks, eye, and forehead region. Once the muscle has been sufficiently stretched, the muscle tissue will be sutured into place, extra skin will be excised, and the incisions will be closed.

Subperiosteal Facelift

The subperiosteal facelift is a type of facelift that specifically targets the upper and middle third of the face. This procedure provides a vertical lift to the soft tissues of the face. It allows soft tissue remodeling and repositioning of the soft tissues at the level of their bony origins.

Gives patients a refreshed look by suspending the SMAS facial muscle and is less dramatic than a regular facelift.

Though less dramatic in results, the mini facelift is still an effective option for reducing signs of aging in the face and neck regions. By making a smaller incision point, the mini-facelift can create a rejuvenating effect on the cheek regions and the jowls while reducing post-op recovery time and procedure complications. Variations such as “the s-lift,” the “ponytail facelift” or the “lunchtime facelift” only slightly differ from the mini-facelift and minimally deviate in incision length or placement. For this procedure, an incision is made at the hairline, extending behind the ear. For this procedure, the incision does not extend into the temporal or upper forehead region unlike its counterparts the SMAS facelift or the deep plane facelift. This allows for a considerably less invasive procedure. With the simple incision made, the surgeon will then raise the SMAS muscle. For this particular procedure, the SMAS can be raised and either plicated or imbricated. To close out the procedure, the incision is then closed and a head dressing is placed on the patient for healing.

Mini-Facelift

The mini-facelift goes by a variety of different names such as “S-lift”, “weekend facelift”, “ponytail facelift” and the “lunchtime facelift”. However, each one is simply a version of the same base procedure. A mini-facelift provides patients with a refreshed look by suspending the SMAS facial muscle, however, the effects are less dramatic than a traditional facelift.

A cutaneous facelift is one of the earliest techniques developed for tightening aging, wrinkled skin. The cutaneous facelift does not manipulate the underlying musculature in the face like more modern SMAS facelifts. Like a thread lift, the results of a cutaneous facelift will not last as long as a technique that cuts and re-drapes the muscles however the recovery time will be faster. 

Once the skin has been marked for incision and the patient is thoroughly anesthetized, the surgeon will cut the skin. Next, the surgeon will undermine the skin, separating the skin from the muscle to the extent of where the excess laxity is present. Special care will be taken to only dissect as far as needed since aggressive undermining may lead to insufficient blood flow resulting in skin bunching or poor healing. The skin will then be pulled into its new position and extra skin will be excised. Removing more skin will produce a tighter result but will create increased tension at the closure and may create a windswept look. Also, earlobe deformities are more common when greater tension is applied near the lower earlobe. Drains may be used to reduce swelling. Sutures will be placed to close the incisions.

Cutaneous Facelift

The cutaneous facelift is a traditional technique used when the patient only needs removal of excess skin from the face. A long incision is placed along the hairline and the surgeon will excise loose skin to tighten and smooth facial skin. The technique does not disturb the underlying muscles in the face.

The liquid facelift achieves a similar effect to that of a more invasive facelift but without the use of a scalpel. By utilizing injections the procedure is considerably less invasive. Cosmetic acupuncture, also known as “natural Botox”, helps to reduce fine lines and wrinkles. Although this treatment will not garner the same results as Botox, the procedure is meant to induce collagen production and provide a natural plumping to the skin. Effects from cosmetic acupuncture are more akin to microneedling than Botox, but will still not produce the same effect as microneedling. This is because microneedling devices cover a larger treatment area than individual acupuncture needles. 

The same needles that are used for acupuncture to treat pain are used during this treatment. The treatment area will be cleansed and the needles will be individually inserted into the skin. The beneficial affects of subzero temperatures on the body have long been observed for improving sore muscles on athletes and more recently in medispas for invigorating full body treatments. Freezing temperatures have been shown to reduce inflammation and have a positive effect on reducing healing time. Most individuals who have done cryotherapy report feeling energized after treatment as well. Although not FDA approved or proven to show any medical benefits beyond inflammation reduction, cryotherapy is becoming increasingly popular as it becomes more readily available. 

Cryotherapy technology, while commonly used in full body treatment, is also used during face only treatments during a Cryofacial. Freezing temperatures of up to -300°F are used to stimulate blood flow to the treated areas, reducing inflammation and purportedly increasing collagen production, shrinking pores, and releasing toxins for instantly smoother, younger looking skin. Liquid nitrogen is vaporized in the machine’s hand piece and is directed to the treatment area several inches away from the face. While there are many different types of facials, most will begin by cleansing the skin using facial sponges, cloths or cotton pads. The eyes will be covered with moistened pads for the next step. Using a lighted magnifying glass, your skin will be evaluated. During this phase, the esthetician may discuss their findings and ask about skin care regimen and your expectations for the facial.

Next, an exfoliation cream, gel, mask, gommage or peel will be used. Certain exfoliation products, such as enzyme peels, require the product to be left on the face to dissolve the plugs that cause blackheads. Other products require either manual or machine stimulation. Depending upon the exfoliation product used, steam will either be applied to the face during that phase or after the product has been removed. Some spas may use heated, moist towels instead of steam. The moist heat loosens plugs and encourages skin to sweat out any impurities that may be caught in pores.

During the extraction phase blackheads and possibly whiteheads will be removed. Facial massage stimulates and lubricates skin. The esthetician may use a massage oil or lotion during this process.  Depending upon the facial, the esthetician may choose a mask that will then be applied. After the mask is removed, the esthetician will likely apply a specialty serum to the skin. Serums include those for aging skin, for blemished skin and for sensitive skin. Lastly, the esthetician will apply a skin-specific cream or gel to the skin. They may also apply an eye gel and/or lip treatment cream.

The ideal chemical is chosen for the desired depth of penetration. The skin is thoroughly cleansed prior to the administration of the solution. The chemical solution is then applied to the desired areas of treatment. The candidate is closely observed for changes to the skin. Whitening of the skin is monitored to assess epidermal damage and is a sign of epidermolysis. As the peel progresses to a frosted appearance this indicates a deeper depth of penetration into the dermis.

Once the appropriate amount of time has elapsed, the solution is then neutralized with another agent such as sodium bicarbonate. Some peels do not require neutralization, as the skin naturally neutralizes them. The skin is then cleansed. Various dressings such as silicone may be applied, ointments or emollients can be utilized after the peel has completed to form a protective barrier. This is more common for deep peels and is typically not necessary for more superficial peels. Keep in mind that the depth of peel is determined not just by the chemical composition or strength of ingredients, but also by the length of time it is left on the skin.

A chemical peel is a skin resurfacing technique to improve the appearance of the skin. A solution is applied to the skin which causes it to exfoliate and "peel off". There are many chemical peels available that range from superficial to deep. The choice depends on the desired depth of penetration, treatment effect, and recovery time. Typically, the deeper the peel, the greater the results, but they are also associated with greater recovery period and adverse effects.

At this point, we’ve all heard about the injectable weight loss craze, but these medications were initially intended for the medical management of type 2 diabetes by healthcare providers. Glucagon-like peptide-1 inhibitors are injectable medications administered into subcutaneous (fat) tissue to alter blood glucose, change insulin secretion, slow gastric emptying, and increase satiety for weight management and also blood sugar control. In clinical trials these prescription drugs have also shown reduced body mass index when compared to placebo.
Ozempic (by Novo Nordisk) is food and drug administration approved to treat patients with type 2 diabetes. Ozempic, which is the brand name for the GLP-1 receptor agonist semaglutide, is also marketed as Wegovy for weight management in those with BMI measurements in the overweight or obesity category.

Glucagon-like peptide-1 inhibitors are injectable medications administered into subcutaneous (fat) tissue to alter blood glucose, change insulin secretion, slow gastric emptying, and increase satiety for weight management and also blood sugar control.

Non-invasive lipolysis allows for the reduction of stubborn fatty deposits without scalpels or scars. SculpSure uses low-energy lasers to target pockets of fat tissue resistant to diet and exercise. Commonly compared to CoolSculpting, SculpSure differs from cryolipolysis in two very distinct ways: 

1. SculpSure heats fat tissue whereas CoolSculpting freezes

Another advantage of the SculpSure system is that treatments generally take half the amount of time as CoolSculpting sessions. No anesthesia is needed during a SculpSure treatment. SculpSure is FDA approved and indicated for treatment of the submental area, stomach, thighs, back, and abdomen. Up to four, flat, non-suction applicator frames will be applied to the skin before treatment. These applicator frames can be arranged in a variety of configurations to suit the needs of the patient. The applicator frames are secured to the patient using a strap system. The SculpSure hand pieces will then be attached to the frames for 25 minutes. During treatment, laser energy will heat the underlying adipose tissue to 42-47 degrees celsius while keeping the external tissue cool. Studies have shown that up to 24% of subcutaneous fat tissue can be destroyed in one session. After treatment, the patient is able to resume all normal activities.

Over time, the destroyed fat cells will be eliminated naturally via the body's lymphatic system. Two to three additional treatments may be required 6 weeks apart. Final results are noticed 6-12 weeks after the final treatment session.

During a treatment session, the patient will sit comfortably in a chair to relax for the duration of the procedure. The treatment area will be cleansed and a topical anesthetic may be applied for patient comfort. A cooling gel will be applied to the surface of the treatment area before the device is applied. The applicator is then placed on the skin and the CoolSculpting device is turned on which vacuums the skin for an even and targeted treatment while freezing fat cells. Patients may feel an intense cool initially, but this feeling dissipates within the first 5-10 minutes of the hour long procedure. Once the session is complete, candidates are able to return to normal activity but may experience mild discomfort and bruising which will fade within 1 week.
