We actually all have NAD+ in our bodies already. But, as we age, our [NAD+ stores decline](https://pmc.ncbi.nlm.nih.gov/articles/PMC7442590/). [Amanda Kahn, MD](https://www.dramandakahn.com/), board-certified internist and longevity specialist in New York City, says that we lose about 50% by age 40. Since the coenzyme plays a critical role in anti-aging by supporting mitochondrial function, DNA repair, and optimizing cellular activity, a decline in NAD+ can contribute to oxidative stress, inflammation, and reduced production of cellular energy called ATP, says Kahn. 

“Decreased ATP can accelerate aging and degeneration of all tissues in the body, including and perhaps most visibly, in the skin. Research suggests that NAD+ helps maintain skin elasticity by promoting collagen and elastin synthesis, improving tissue hydration, and supporting adequate nutrient delivery by supporting the skin’s microvasculature,” explains Kahn. “NAD+ also activates the Sirtuin family of longevity-associated proteins that promote cellular repair and reduce inflammation, both of which contribute to slowing down and possibly reversing the aging process.”

In addition to its anti-aging benefits, many report that the popular treatment boosts energy levels and enhances cognitive function. By supporting mitochondrial health and regulating oxidative stress and inflammation, NAD+ can also improve focus, memory, and mental clarity.

Since it’s absorbed more quickly, some people have difficulty tolerating infusions and experience nausea, stomach pain, dizziness, and headaches during the session. 

- **Oral Supplements:** NAD+ supplements are available in capsule and powder form and are marketed for their potential to boost energy and support longevity. While they offer a more convenient option compared to IV therapy, their effectiveness is patient-dependent since they must first be absorbed through the digestive system. Though, Kahn prefers precursors of NAD+, such as nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN), over direct oral NAD+ supplementation because they are easily accessible over the counter, are bioavailable by mouth, and are efficiently transported into cells. For optimal results, supplements must be taken consistently. As with all supplements, there are potential side effects, such as nausea, headaches, itching, and sweating.

- **Injections and Patches:** While less common than IV therapy and oral supplements, NAD+ can also be administered through intramuscular injections and transdermal patches, either in-office or at home. Intramuscular injections provide a quicker method for NAD+ absorption and are typically administered in a clinical setting. Transdermal patches, on the other hand, deliver a slow and steady release of NAD+ through the skin.

Another benefit? “They can also allow products to penetrate more deeply,” she notes. 

Whether you opt for an [at-home body scrub](https://aedit.com/aedition/body-scrubs-body-moisturizers-for-glowing-skin) or a professional treatment, the smoothing and brightening benefits are similar. But, when it comes to professional body exfoliation, you can expect the full pampering experience. For most spa scrubs, your aesthetician will first apply the exfoliating formula to the entire body, scrubbing away dead skin and boosting circulation. From there, a mineral-rich body mask and/or moisturizer is applied to nourish the skin. 

Depending on the spa, there could be a number of scrubs to choose from. Common options include:

Dr. Engelman says the smaller the particle, the better. “Scrubs can be abrasive,” she cautions. “Large and/or rough particles, like walnut or apricot-pit scrubs, can be too rough on skin, causing redness, irritation, and even tiny injuries that risk getting infected.” As she explains, there is no need to roughly scrape the skin. “Smaller, softer particles — ideally those that dissolve or melt into the skin — are effective at buffing away dead skin cells and are less likely to cause irritation or injury,” she adds. 

## **Professional Body Wraps**
Similar to body scrubs, there are often an array of professional body wrap treatments to choose from. Depending on the type, they can help with detoxification, hydration, and even blood circulation. Regardless of the type, you can bet they will leave you with soft, smooth, and hydrated skin. Here’s what to can expect from a professional body wrap: 

1. First, your technician will use an exfoliant to prepare the skin
2. After the skin has been prepped, the esthetician will apply the mask and tightly wrap the body (usually in plastic or towels)
3. Once cocooned, the therapist will leave you to rest for about 30 minutes to let the mask work its magic

A traditional Hawaiian massage, Lomi Lomi employs long continuous strokes for a heavy dose of calm. Bufanda says you can expect the therapist to use their forearms frequently and move from the top of the shoulders to the tips of the toes. “It is a lovely full-body experience that leaves the client feeling extremely relaxed,” she notes.

### **Shiatsu Massage**
A therapeutic form of massage from Japan, shiatsu massage relies heavily on finger pressure to concentrate on certain energy pathways or meridians. With roots in Traditional Chinese Medicine (TCM), therapists seek to facilitate healing by promoting a healthy flow of chi (read: energy) throughout the body. It is usually performed on the floor or on a low massage table, and, while various pressure points are stimulated during treatment, the massage is not meant to be uncomfortable.

### **Sports Massage**
Therapeutic sports massages come in different forms depending on the reason for treatment. “An ‘event’ massage is typically clothed, uses a lot of compressions and stretching, takes place at marathons and other sporting events, and is perfect for an athlete's warm up and cool down,” Bufanda explains. For athletes looking for longer-term benefits, a ‘maintenance’ sports massage may be in order. “It uses deeper techniques that really engage and stretch the connective tissue,” she says, adding that it is meant “to support an athlete's ongoing training or injury treatment.”

### **Prenatal Massage**
As the name implies, a prenatal massage is designed for women who are [pregnant](https://aedit.com/aedition/skincare-ingredients-to-avoid-during-pregnancy-breastfeeding). According to Bufanda, the difference between a prenatal massage and most other massage treatments is the positioning of the client and the amount of pressure applied. “We change our technique in areas, like the insides of the legs, to light strokes in order to keep baby and mother safe,” she explains. 

At LOTTE Spa, they recommend waiting until the second trimester for a massage. “Depending on how far along the client is, we typically position the client on their side, with lots of cozy pillows for optimal comfort,” she shares. “We generally see pregnant clients that are experiencing lower back pain, hip pain, or are looking for overall relief and relaxation from all of the hard work their body is doing!”

### **Reflexology**
Unlike the other types of massages we’ve covered, reflexology is not a full-body treatment. Instead, massage therapists or reflexologists focus on specific areas — like the feet, hands, and ears — and apply different amounts of pressure to improve both physical and mental health. Depending on the spa, this can be a standalone service or an add-on to other treatments. 

## **A Brief Guide to Massage Etiquette** 
Now that you know which type of massage to book, we want you to feel comfortable and confident both on and off the table. How can you ensure you are a model client and receive the best treatment for your needs? To and Bufanda says there’s not actually a whole lot to do (above all, they want their clients to be relaxed and enjoy themselves), but here are their tips for everything from showing up on time to showing appreciation when you leave.

### **1. Arrive On Time**
When asked for their single most important etiquette tip, both To and Bufanda have the same answer: punctuality. “We’ll always try to accommodate and move an appointment if possible, but, on a busy day, there’s not a lot of flexibility in the schedule,” To explains. In such cases, he says the service will have to end at the scheduled time regardless of when the client arrived. Rather than planning to show up at the time of your appointment, Bufanda recommends coming in at least 15 minutes early. That way, you can fill out paperwork, use the restroom, and take advantage of any other relaxing amenities prior to your treatment!

### **2. Shower First, Shave Later**
Bufanda says it is appreciated when a client is able to shower beforehand, though she is quick to note that massage therapists know that isn't always possible — and “that's totally okay!” As for whether or not you should shave first, don’t reach for your razor on her account. “We never care whether or not a client has shaved,” she says. “We are more concerned with their muscles.”

### **3. Feel Free to Take It All Off** 
When it comes to disobing, what is the proper protocol? “I always tell guests to disrobe at their comfort level, and I explain that they’ll be covered and draped the entire massage,” To shares. “We only uncover the areas we are working on at the moment.” As a male therapist, To says he makes a point to be “very conservative” with his draping technique and to “never massage under clothing that is left on.” In his experience, most people either take everything off or leave their bottoms on. 

In the case of undergarments and jewelry, think about what’s going to make for the most relaxing treatment. “Clients should consider what areas they'd like specific work on and if they are okay with getting oil or lotion on their garment or jewelry, as sometimes they can get in the way,” Bufanda says. It’s probably easiest to remove necklaces and earrings, but, at the end of the day, it’s up to you. “Ultimately, client comfort is the priority,” she shares.

### **4. Don’t Be Afraid to Communicate** 
Your massage therapist is there to deliver the most enjoyable treatment for each client, which means they are willing to talk as much or as little as you would like to. “It’s their time, so they’re welcome to chit chat or speak as much as they want,” To says. “I let the guest lead any conversations.” 

With that said, communication — especially if it’s about certain target areas or pressure — is the key to maximizing your experience. “In order to get the most out of a massage, clients should definitely feel empowered to ask for what they need, whether that means a pressure adjustment, spending a little extra time somewhere, or skipping an area all together,” Bufanda notes. In her opinion, there is no such thing as being “too vocal,” for “this is ultimately the client's massage.”

One thing to keep in mind about pressure: While clients have every right to ask for as much or as little pressure their therapist is capable of giving, those “looking for extremely deep pressure should know that their tissue will need to be warmed up first using lighter techniques and progressing into deep pressure in order to prevent damaging the tissue,” Bufanda shares.

### **5. Gratuity = Gratitude**
If you’re like us, your number one question is: How much should you *really* be tipping? Our experts agree that 18 to 20 percent gratuity is standard for spa massages — “but it is always the client's choice for how much they'd like to leave,” Bufanda says. She does note, however, that you need to consider your setting. Medical clinics and/or chiropractors' offices often don’t accept tips because the massage is being billed to the client’s insurance. When in doubt, inquire about the gratuity policy when you book your appointment. 

## **How to Maximize Your Massage**
Needless to say, massages can do wonders for your mental and physical wellbeing, but did you know you can actually extend the benefits post-treatment? To keep the rest and relaxation going, consider the following:

*May is Mental Health Awareness Month. For so many of us, this last year has been particularly difficult on our mental and physical wellbeing, and it is important to remember that we are never alone. Here, one writer shares her deeply personal experience with stress and depression and the impact it has had on her relationship with her skin during the COVID-19 pandemic.*

Our mental health has been put to the test over the last year, and even people who haven’t suffered emotionally in the past have been finding things difficult recently. Symptoms of depression and anxiety are broad and vary widely from person to person, but many people will experience a sense of lethargy and lack of interest in things they normally take an interest in. 

Like many people reading this, I have dealt with depression on-and-off since I was a teenager, and the uncertainty that the pandemic has thrown our way has certainly made my mental health tougher to manage. I wanted to share how dealing with the effects depression impacts my skin and explore how we can strip back skincare to the bare minimum when the world around us gets to be a bit much.

## **How Stress Affects the Skin** 
Depression, anxiety, and even stress can all affect your complexion, and everyone’s body reacts differently to these factors. People who have [acne-prone](https://aedit.com/concern/acne) skin, for example, may experience more breakouts, and stress can also trigger [eczema](https://aedit.com/aedition/best-skincare-products-to-treat-eczema-flare-ups-in-the-winter) flare ups. Studies have shown that [stress can affect](https://aedit.com/aedition/impact-of-stress-on-skin-acne-wrinkles-dryness) the way the skin’s barrier function regulates water retention, which means your skin may be drier and more sensitive. Since [poor sleep](https://aedit.com/aedition/impact-of-beauty-sleep-on-skin-health-nighttime-skincare-products) is another common symptom of depression, you may find that both your eyes (think: dark circles) and skin looks more tired than usual.

In my case, I find that my skin tends to lose elasticity, which can be attributed to stress-induced changes in the skin’s proteins. When I’m feeling low, I forget about the nice glowy skin I carefully worked on, and my complexion tends to become dull and dehydrated. 

Your skin may also change if, like me, your skincare routine falls to the wayside when you’re not feeling your best. As much as I love skincare — and I will happily indulge in a full-on Korean-inspired skincare routine when I’m feeling well — that routine gets swapped out for a quick cleanse in the shower when I’m not doing so great. Dramatically shifting your skincare routine in this way also impacts your skin, and it may lead previous issues to resurface.

## **Why Routines Can Feel Difficult**
Depression [affects neurotransmitters](https://www.gulfbend.org/poc/view_doc.php?type=doc&id=12999&cn=5) in your brain, and some of these control how motivated we feel. These neurotransmitters are responsible for a wide range of our daily habits — regulating everything from how we sleep to how easily we can recall information. Changes in levels of serotonin and dopamine can impact whether you feel like completing your usual tasks, including your skincare routine. 

When you’re feeling depressed, the thought of having to do a whole number of steps before you hit the hay can feel overwhelming. I can’t tell you how many times over the past few months I have stood in front of my skincare products and just not had the ability to come up with a routine, so I’ve given up and gone to bed. Likewise, if you’re struggling to sleep, getting up to apply products tends to feel pretty unappealing.

In other words, it’s normal to not feel like doing certain things when you’re struggling with depression or chronic stress. This being said, it’s a great idea to try to adapt your old habits, so they feel more manageable while you’re going through a difficult time. In this case, switching out your usual skincare routine for something pared down could be a good compromise.

## **Back to Basics**
If you are someone who is used to taking care of your complexion with a multi-step skincare routine, you may be wondering how to strip it down. The two most important elements are cleansing and sun care. Wear [sunscreen](https://aedit.com/aedition/how-to-apply-sunscreen-and-not-miss-a-spot) during daylight hours, wash it off at night, and you have, theoretically, given your skin the protection it needs. From there, you may consider applying a non-SPF moisturizer before bed to rehydrate the skin and counteract some of the stressors we discussed above.

I personally find that leaving my cleanser in the shower is a surefire way of making sure that I take the time to cleanse regularly. I also leave my sunscreen in the bathroom, so I put it on as part of my morning getting ready routine without going out of my way. 

Finding products that work well for your skin and state of mind is key. This, of course, is always true, but it’s especially important when you’re stripping down your routine.
