As a beauty editor, being non-committal to skincare products and tools is a part of the job. While there are lotions, potions, products, ingredients, and devices we know are tried and true, it’s also our duty to test out the latest launches to stay abreast of the innovations on the market. Consider it research. Therefore, by keeping my vanity open to new products, maintaining a curated routine has been a challenge. Yes, I am mindful of the ingredients I use and how they play together, but, I am not as selective on which cleanser or moisturizer I will use from week to week. So, it comes as no surprise that most of my [twenties](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-20s) were spent experimenting and prioritizing product testing over tailoring a personal [routine](https://aedit.com/aedition/how-to-maintain-youthful-skin-in-your-20s) – until I hit [30](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-30s).  

Knowing that my skin wont be smooth forever as fine lines, dullness, and pigmentation are a part of life, I knew it was time to do what I’ve been dreading: sticking to a routine with tretinoin. Because adding a prescription-strength vitamin A into the regimen requires a methodic approach (and patience), my hesitancy prevented me from diving into the deep end. Three months ago, I decided to finally incorporate the holy grail of youth-preserving ingredients into my routine, and here’s how that journey went. 

## **What is Tretinoin?** 

For the newbies, tretinoin is “[a vitamin A](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare) derivative, a type of retinoid that is available as a prescription,” explains [Marisa Garshick, MD](https://aedit.com/provider/marisa-garshick-md-313), a board certified dermatologist based in New York. The workhorse ingredient works by regulating skin cell turnover and can help prevent and treat acne, explains Dr. Garshick. But, the fun doesn’t stop there. “Additionally, because it can also help boost collagen production, it can help to improve the appearance of fine lines and wrinkles, in addition to acne scarring,” she adds. To simply put it, the prescription-strength ingredient “can help to improve overall skin tone and texture.” 

The ingredient requires a prescription, and while Dr. Garshick was not my personal dermatologist, her insight is invaluable when it comes to all things skincare.  

## **How To Add Tretinoin To Your Skincare Routine** 

What kept me from starting sooner was the barrier to entry, it is a process that requires more than a prescription to truly make it work. Below are some of Dr. Garshick's tips and recommendations for starting tretinoin.  

### Do
- **Start Slow**: When adding any new product into your routine, especially one as potent as tretinoin, modesty is key. A pea-sized drop of the ivory cream is recommended once a week to build up a tolerance, and with time, the amount can be adjusted. As we always say, let your skin guide you and tell you what it needs. “It is best to incorporate tretinoin into a nighttime routine and, to start off using it just a few times per week and then increase the frequency as tolerated.” 
-  **Limit Exfoliants**: Over-exfoliating the skin can cause a weakened skin barrier, loss of moisture, and therefore, more breakouts to arise. “It is best to avoid using any ingredients that can be harsh on the skin at the same time as tretinoin, such as exfoliating acids, including salicylic acid, and benzoyl peroxide – unless directed to do so.”
- **Be Patient**: From purging to causing irritation, tretinoin is fickle as the barrier of entry is almost like an initiation process. Because the skin’s experiencing increased cell turnover, 

### Don’t 
- **Skip the SPF**: Sun protection should not only be the first line of defense in any skincare routine, it is vital when using a chemical exfoliant. “It is important to always be good about sun protection when incorporating a retinoid into your routine and wearing daily sunscreen,” adds Dr. Garshick.  
- **Worry About A Purge**: Purging, as it is lovingly known, is the process of the skin breaking out which may seem paradoxical considering the ingredient's efficacy when treating acne. However, this is perfectly normal and to be expected. If the acne persists, speak with a trusted dermatologist to adjust the routine or switch to a retinol alternative (hello, [bakuchiol](https://aedit.com/aedition/guide-to-bakuchiol-in-skincare)).  
- **Complicate Things**: It’s natural to want to rush the process, the benefits are numerous, after all. During the adjustment period, focus on the basics and how your skin adjusts before incorporating additional products. “I recommend applying tretinoin after cleansing and before moisturizing, but for those with dry or sensitive skin, it may be used after applying moisturizer which may help reduce the potential for irritation in some individuals,” Dr. Gashick advises. 

## **My Experience With Tretinoin** 

We often discuss the timing of starting a product, the importance of doing a patch test, and the mindful approach required to ensure all ingredients work in harmony, so, to do this right, I overhauled my routine. My first step was to start tret during the winter months due to the decreased sun exposure I would have as my skin acclimates. My second step was cutting out other physical exfoliants. Due to tret being a heavy-duty chemical exfoliant, I opted to shelve my favorite [AHAs](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) and [BHAs](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare) for the time being. Because of my hesitancy to stick to a set routine, I wasn’t going to take any risks with over-exfoliating. Here is a breakdown of my AM and PM routines.

Now, to provide clarity on my skin, it’s normal, not prone to acne, and has very little pigmentation. My reasoning for starting tretinoin was to address any fine lines and wrinkles that may arise with time, as my belief has always been that it’s better to prevent than correct. 

During the first few weeks of my journey, the [irritation](https://aedit.com/aedition/skincare-ingredient-alternatives-for-retinol-retinoids) was minimal, and though I was prepared for a purge (cue dramatic music), it never came. Rather than a noticeable breakout, my skin went through a few weeks of whiteheads popping up at inopportune times, slight redness here and there, and more dryness than I usually experience during the colder season. Overall, my fear of the unexpected proved to be just that, a fear.   

I have to admit that due to my being a regular person, my routine did waiver as I would try a new product on days when I wasn’t using tret. My goal was to be litigious throughout the process so as to not throw any curveballs, but I did experiment sporadically with a new gentle mask or two. I maintained my rule to avoid all other chemical exfoliants and was consistent with my SPF application (and re-application). 

After three months, I can confidently say that my skin looks better than I expected. My primary concerns (fine lines forming on my forehead and marionettes) were not as prominent as my skin adjusted to tret. 

## **The Takeaway** 

After carrying what felt like a dirty little secret of not incorporating vitamin A in my routine for quite some time, I am happy to report that the ingredient is all it’s cracked up to be. Offering a noticeable change to my skin in a matter of months, tretinoin has provided me with clearer skin, a smoother complexion, and fewer breakouts. Though the adjustment period felt like a waiting game, trusting the process proved to be worth it.

I Finally Tried Tretinoin And I’m Never Going Back

If you would have asked me a couple of years ago what one aesthetic treatment I would absolutely not be interested in, I wouldn’t have hesitated to say [lip filler](https://aedit.com/procedure/temporary-lip-fillers). I had nothing personally against the minimally invasive lip enhancement procedure – and I had certainly interviewed enough top injectors who sang its praises – but, for me, I just couldn’t get past the fact that it felt like every influencer and reality TV personality increasingly had a mouth that looked all-too-similar and all-too-supernatural. 

I am usually the first person to tell you that aesthetic medicine has long gotten a bad rap because we tend to only spot the overly augmented or unnatural work while more subtle tweaks go unnoticed, save for recognizing that the person looks great. Lip filler is but another cosmetic treatment that falls into this trap. Yes, we’ve all seen a lot of so-called ‘duck lips’ or ‘trout mouths’ that may or may not be your taste (personally, I am in the ‘nay’ camp). But we’ve also all seen a lot of plush pouts that are [perfectly proportional](https://aedit.com/aedition/facial-harmony-in-aesthetic-medicine) to the rest of the face – and there is a good chance that those lips are filled, too.

I sit here today eating a helping of humble pie (what’s that they say about never saying never?) as I write this article about why I decided to fill my lips, and, better yet, why I am so happy with the results that hardly anyone has noticed. In early April, New York City-based board certified plastic and reconstructive surgeon [David Shafer, MD](https://aedit.com/provider/david-shafer-md-153), injected me with one syringe [Juvéderm® Volbella XC](https://aedit.com/procedure/juvederm) (split between my upper and lower lip), and I’ve been spending the last couple of months thinking about what I learned from the experience that might benefit others who are on the fence about going under the needle. 

Below, I’ve compiled a list of the six things I believe are useful to know if you are considering getting lip filler. Read on to get the scoop and to see my before and after photos.

### **1. Not All Lip Fillers Are the Same**
If you are a regular reader of The AEDITION, you’ve likely seen our breakdowns of the different types of [cosmetic injectables](https://aedit.com/procedure/dermal-filler-and-injectable-solutions) that exist. Within the filler category, there are [hyaluronic acid (HA)-based and biostimulatory options](https://aedit.com/aedition/hyaluronic-acid-filler-vs-biostimulatory-injectables). Within the HA family, there are an array of brands (think: Juvéderm®, [Restylane®](https://aedit.com/procedure/restylane), [Belotero®](https://aedit.com/procedure/belotero), etc.). And within each brand, there are a variety of different products that have been designed and, in on-label use cases, approved by the United States Food and Drug Administration (FDA) for specific purposes. “Juvéderm® Ultra XC is the original hyaluronic acid filler FDA approved for lip augmentation,” Dr. Shafer says. “It works well for plumping and adding volume.”

Juvéderm® Ultra XC may be the OG, but it has a younger sister that can treat the lips in a similar but different way. “Volbella is a newer generation hyaluronic acid filler in the Juvéderm® family that is good for smoothing and hydrating,” he shares. “As Volbella is a smoothing filler, it is also great for softening perioral lines such as the vertical ‘smoker lines’ on the upper lip and any creases which have formed from years of dynamic lines around the mouth, such as the smile lines.”
 
While each filler can be used individually (I only got Volbella), they can also be paired together to better treat a patient’s aesthetic concerns. “I prefer a hybrid lip augmentation technique with both Juvéderm® Ultra XC and Juvéderm® Volbella XC to both plump and smooth,” Dr. Shafer says. This allows him to “achieve the best results.”

Why am I bringing this up? You don’t need to walk into your injector’s office demanding a certain type of filler — they are the expert and, during a [consultation](https://aedit.com/aedition/questions-to-ask-your-doctor-during-plastic-surgery-consultation), will be able to share what brand and product is suited for your anatomy and aesthetic goals. Even so, it’s helpful to know that lip filler (and filler in general) is not one size fits all. What’s best for your friend or sister or some person you saw on TikTok, may not be best for you. You should feel free to ask your provider what types of filler they offer, why they think something is right for you, and what you can expect from the result. 

### **2. You Can Maintain Your Lip Shape**
This was a big one for me. Everyone has their own preferences when it comes to [lip shape](https://aedit.com/concern/lip-shape) and [lip size](https://aedit.com/concern/lip-size), and some people turn to lip filler to address both. Personally, I’ve always liked the shape of my lips. I naturally have a pretty well-defined [cupid’s bow](https://aedit.com/aedition/cupids-bow-cosmetic-surgery-fillers-for-lip-augmentation), as well as upper and lower lips that gradually taper out to the corners. Overall, I felt like my mouth fit my face, but the lips themselves were a little thinner than I wanted them to be (especially my top lip).

Because I was overly fixating on some of the more obviously augmented pouts that populate my social media feed, I was concerned that I was going to have to sacrifice the shape of my lips to enhance the size. I thought I would automatically end up with a kind of elongated and tubular upper lip shape that some people might like — it’s just not what I was going for. I am pleased to report that, in the hands of a skilled injector, the shape of your lips can be preserved while subtly enhancing the volume. 

The key: Be clear about what you want and don’t be shy when communicating it to your provider. Natural-looking lips (like any facial feature) are about [proportion](https://aedit.com/aedition/plastic-surgery-results-are-about-proportion-not-perfection), so bringing in a picture of Angelina Jolie’s plush pout isn’t particularly helpful. Instead, telling your injector exactly what you like about your lips and exactly what you don’t is the best way for them to get an understanding of what you are looking for. They will be able to make recommendations and select the proper filler for you from there. 

### **3. The Injections Don’t Hurt**
The lips are one of the most sensitive parts of the body, which means the thought of putting needles into them isn’t particularly pleasant. But here’s the thing about lip filler injections – they don’t hurt (and [you don’t just have to take my word for it](https://aedit.com/aedition/what-to-know-before-getting-lip-filler-for-the-first-time)). Dr. Shafer put [numbing cream](https://aedit.com/aedition/numbing-cream-breakouts) on my lips and left it on as he prepared the filler and injected Botox® into my forehead, glabellar lines, and crow’s feet (I got both injectable treatments done at the same appointment). After about 10 minutes, he wiped off the residue and began injecting my lips. It was a total of four pricks, and, between the numbing cream and the lidocaine in the filler, I didn’t feel anything more than a poke. 

Before sending me on my way, Dr. Shafer told me to massage or rub my lips together more than normal in the coming hours and days to “ensure a smooth result.” While the anesthetics were all still active, this was no problem. As they wore off, however, this became a bit more challenging. The injections themselves were painless, but I did experience some mild discomfort as the day went on. I saw Dr. Shafer in the morning, and, by late afternoon, my lips were tender to the touch – and rub. The act of pressing them together induced a stingy, bruise-like sensation (though I didn’t have any visible bruising). By the next morning, this tenderness had greatly subsided; by the end of the day, it had all but disappeared. At that point, I went back to rubbing them together again as per his instructions.

Aside from the massaging, Dr. Shafer suggests avoiding straws for several days after injections. Kissing is okay after 24 hours, he says, and there is no restriction on exercise. Another thing to keep in mind: Salty food may make swelling last longer, which leads me to…

### **4. The Swelling Is Real, But Short Lived**
The discomfort I experienced in the hours proceeding the filler injections was accompanied by [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery). “After any injection, patients may have swelling and possible bruising,” Dr. Shafer says. “Swelling can last 24 to 48 hours and usually is quite subtle.” As my lips got more tender, they also got firmer and a little bigger. The swelling seemed to peak around 7 p.m. (about eight hours after my appointment). When I woke up the following morning, the swelling had mostly dissipated and it continued to over the course of the next day or two.

Dr. Shafer gave me an ice pack immediately after he finished injecting me, and I used it on both the areas he treated with Botox® and my lips. I probably did about 10 minutes of icing in the office and then another 10 to 15 minutes when I got home an hour later. I drank an iced beverage – without a straw! – most of the afternoon, and the cold felt good against my lips. When I noticed the discomfort and swelling start to set in around 5 p.m., I reached for the ice pack again and alternated 10 minutes on, 10 minutes off for about an hour. I don’t know if it did anything to reduce the swelling, but it definitely helped ease the sting.

I didn’t experience any [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox), and Dr. Shafer says it’s rare. “If it does [occur], then it may take five to seven days to resolve,” he shares. “We offer complimentary laser treatments for any patient with a bruise to help it resolve faster.”

### **5. You’ll Need Less Lip Balm**
I’ve heard anecdotes about HA fillers hydrating the lips, but this one still surprised me. While my lips don’t get particularly chapped, I am the type of person who needs to have something slathered on them at all times for risk of having them feel dry (I can’t handle that sensation). I have a stable of balms, oils, and masks I rotate between throughout the day and overnight, and, while I still reach for them, I've realized that it’s now more out of habit than necessity. 

Since I got my lips filled, they rarely, if ever, look or feel dry. Because of the fact that it’s largely comprised of [hyaluronic acid](https://aedit.com/aedition/guide-to-hyaluronic-acid-in-skincare-dermal-filler) (a humectant that draws moisture into the skin and can hold 1,000-times its weight in water), Juvéderm® Volbella XC is known as a ‘hydrating’ filler. In my experience, that has meant that my Volbella-infused lips have a perpetually smooth appearance and a perpetually moisturized feel. ### **6. Nobody Has to Notice**
Depending on your aesthetic goals, the idea that no one really noticed you got lip filler might not be appealing. For me ([much like when I got nasolabial fold filler late last year](https://aedit.com/aedition/what-its-like-to-get-nasolabial-fold-filler)), it’s the best part. My lips are subtly enhanced in a way that I relish every time I look in the mirror or smile in a photo, but not in a way that has had any of the people I see most often asking me what I had done. My mom noticed the difference; most of my coworkers did not. 

I obviously have Dr. Shafer to thank for (1) really listening to my concerns and (2) choosing the right filler and injection technique for me. “As Volbella is a soft, smoothing hyaluronic acid filler with a less plumping effect, it is good for first-time users who are looking for subtle effects,” he shares. “I tend to be conservative, as we can always add more later.” Down the line, I could also consider trying a different combination of products. “Once a patient sees the potential of lip fillers, the hybrid technique is usually best for subsequent treatments,” he explains. “It's good to start out subtle and then get more aggressive with treatments depending on the patient's goals.”

Two months after treatment, I am thrilled with the results. As for how long I can expect to enjoy them, every patient is different because every body metabolizes filler differently. With that said, Dr. Shafer offers a simple way to think about scheduling touch ups. “For most patients, we set them on a schedule of Botox® every four months and fillers every other Botox® appointment, so every eight months,” he shares. 

This cadence is ideal for those who don’t want a lapse in their look. “There is a difference between how long the filler is present versus how long the clinical effect is apparent,” Dr. Shafer explains. “That is why you may hear a filler lasts 12 to 18 months, but, in reality, the next treatment would be sooner than that.” Think about it like topping off your gas tank. “Similar to when you fill your gas tank, you don't usually wait for it to go to empty before filling the tank back up,” he says.

## **The Takeaway**
If you had told me a few years ago that I would be writing an article about my experience getting lip filler, I wouldn't have believed you. But the more I’ve learned about the treatment, the more I realized that it could be minimally invasive in more ways than one. Sure, the results are immediate with little pain and no downtime. ## **What Is truSculpt® Flex+?** 
In simplest terms, truSculpt® Flex+ “is an electrical muscle stimulation machine,” Dr. Alvarez explains. It is cleared by the United States Food and Drug Administration (FDA) for the improvement of abdominal tone, strengthening of the abdominal muscles, and development of a firmer abdomen. It is also cleared for strengthening, toning, and firming the buttocks and thighs. At his practice, the technology is regularly employed on the arms, abs, glutes, thighs, and calves.

For two weeks in January, Dr. Alvarez became my personal trainer of sorts – shaping my buttocks while offering insight into the treatment and, at times, providing me with emotional support during the more intense parts of the glute-building experience. A diplomate of the [American Academy of Aesthetic Medicine](https://www.aaamed.org/) and member of the [American Academy of Cosmetic Surgery](https://www.cosmeticsurgery.org/), Dr. Alvarez informed me that truSculpt® Flex+ uses a proprietary Multi-Directional Stimulation technology “to target specific areas of the muscle imitating the motions we would do at the gym.” What separates this from a workout, however, is that “the Flex+ allows us to do it in 15 minutes,” he notes. 

So, what happens in these 15 minutes of muscle-growing? “It’s 54,000 contractions, different motions of the muscle such as pulling, compressing, twisting, and even bouncing to stimulate them,” he explains. “This replicates the motions we’d do in the gym, but it surpasses that past exhaustion.” 

## **What Is the truSculpt® Flex+ Treatment & Recovery Like?**
Unlike other muscle-building procedures on the market that have one or two paddles, truSculpt® flex+ has the ability to treat eight areas simultaneously. In my case, all eight squares were placed on my buttocks (four on each butt cheek), but they could theoretically be spread out to treat more than one area at a time. “Once a person comes in, and their medical history is assessed, we examine the areas to make sure we don’t treat over bone,” he explains. Then the gel pads are placed on the target areas, followed by the electrodes that contract the muscles. 

During the sessions, Dr. Alvarez increases the intensity of each paddle. He is a de facto personal trainer in the sense that he observes the patient's needs and pain tolerance to determine intensity. Being no stranger to [injectables](https://aedit.com/aedition/qwo-for-cellulite-before-and-after-photos) and other [high-tech devices](https://aedit.com/aedition/what-its-like-to-get-tracie-martyn-red-carpet-facial), I was game to push myself to the limit in an effort to maximize results. Anyone who has tried a muscle-stimulating body sculpting device knows that it creates a tingling, almost ticklish sensation during the contractions. The body spazzes and releases all on its own. In the first few minutes – the “tone” portion, as Dr. Alvarez explained – the experience was ticklish, but it soon progressed to the “sculpt” setting where a more uncomfortable feeling kicks in. 

At the end of my first session, I managed to progress to 70 percent intensity, which took a while to adjust to. By the end of my last session, I had reached the mid-nineties. If I’m being honest, it hurt. However, as the machine switched off, the pain instantly went away. The best thing I can compare it to is the pins and needles of a leg falling asleep – multiplied by a thousand. Alas, no pain, no gain.   

As far as recovery and post-treatment soreness, there was none. After each 15-minute session, I walked out of Dr. Alvarez’s office and forgot about any discomfort in a matter of minutes. Soreness was also nonexistent for me, unlike the aching inability to walk after a heavy lifting session. Consider me a convert. 

## **Who Is a Candidate for a truSculpt® Flex+ Butt Lift?** 
Apart from those with a recent surgical scar, certain injuries, or metal device in their body (think: pacemaker or internal defibrillator), most people are candidates for truSculpt® Flex+. “This machine is great for someone who has some [sort of injury](https://aedit.com/aedition/emsculpt-for-post-surgery-recovery) and isn’t ready to go back and put in the work, but they need to strengthen those muscles for their mobility,” Dr. Alvarez says. Those seeking improvement in their muscle tone and strength will see gradual enhancement. 

While many might perceive this as a fat-burner, truSculpt® Flex+ does not provide those benefits. I am 30, have built a decent amount of muscle on my own, and have no conditions that would prevent me from getting the treatment. My age and relatively healthy lifestyle theoretically set me up for success and speedy results. However, truSculpt® Flex+ isn’t just for the young and fit. During a session, I met another one of Dr. Alvarez’s patients who is in her fifties. Like me, she treated her bum and saw a marked improvement in volume, shape, and even quad definition. Clearly, age isn’t a factor with the multi-directional device.  

## **When Do You See Results From a truSculpt® Flex+ Butt Lift?** 
Because I opted to go higher on the intensity level, I was able to see results quickly. Individual results vary (especially when you consider diet and lifestyle factors), but, for my behind, there was a visible improvement by the two-week mark. A perkier and more volumized posterior were the first signs. And, when it came to function, I was truly impressed.

For the duration of the treatments, Dr. Alvarez told me to lay off any glute workouts. “I usually tell patients to wait 24 to 48 hours before doing any other exercise,” he says. “It is like going to the gym – the recovery period is also important.” Having opted to dial back my workout regimen per Dr. Alvarez’s instructions, I focused the few workouts I did on the upper body during that two-week period. But, after my treatments were complete, I immediately felt the difference and it helped me surpass my plateau. While my quads and hamstrings required a jumpstart to get back on track, my glutes were able to handle more weight. 

When it came to [diet](https://aedit.com/aedition/meal-plans-diet-before-after-cosmetic-surgery) (a huge element of successful body contouring treatments), Dr. Alvarez recommended sticking to high-protein and larger water intakes as my body adjusted and recovered. Technically, the ‘final’ results are enjoyed after three months after your final session. Because it’s a muscle-building treatment, that increased mass [needs to be maintained](https://aedit.com/aedition/what-to-do-after-nonsurgical-body-contouring). Needless to say, you’re not going to be able to replicate 54,000 contractions in 15 minutes at the gym, but bum-sculpting exercises can help prolong the effect. ## **How Does truSculpt® Flex+ Compare to a Traditional Butt Lift?** 
Four sessions of truSculpt® Flex+ gave me the butt lifting effect I was after, but Dr. Alvarez emphasizes that not everyone will see the same results and the results are not meant to compare to a [surgical butt lift](https://aedit.com/procedure/butt-lift) or [Brazilian butt lift (BBL)](https://aedit.com/procedure/brazilian-butt-lift). A traditional butt lift removes sagging and excess skin and places strategic sutures to provide internal support that firms and contours the backside. But, as someone who is not willing to commit to the downtime and recovery associated with surgical butt enhancement, a perkier posterior in two weeks without any impact on my daily life is about all I could ask for.

## **My Takeaway** 
It’s safe to say that, by word of mouth, the 15-minute treatment has gotten a lot of buzz in my circles. Personally experiencing the powers of this technology, I was able to put the gimmick perception I had to rest while converting a few others. With no recovery – and I mean none – I was able to go about my life, feel strong, and have the confidence of knowing I went to the right provider.

Would it be as transforming as the A-listers say? If it’s good enough for Rihanna, Madonna, Irina Shayk, and virtually every Hollywood ‘It’ girl on the scene, I needed to know if it can deliver the glow, lift, and contour to someone who doesn’t have a pampering army behind them (i.e. me). Here’s how it went. 

## **What Is the Red Carpet Facial**

There is a reason that [rhinoplasty](https://aedit.com/procedure/rhinoplasty-nose-job) (a.k.a. nose job) is perennially one of the most popular cosmetic surgeries. Whether it’s performed for [aesthetic or functional reasons](https://aedit.com/aedition/difference-between-cosmetic-rhinoplasty-vs-functional-rhinoplasty), the procedure can be transformative. Rhinoplasty is generally considered safe and effective, especially when carried out by a board certified facial plastic or plastic surgeon. Even so, there is always a [risk of complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications) and/or unsatisfactory outcomes. Furthermore, the results of rhinoplasty, while long lasting, are still at the mercy of age and gravity, which means they can change over time.

For some patients, one or more of these factors can be reason to undergo a revision rhinoplasty. Generally speaking, secondary surgeries are [more costly](https://aedit.com/procedure/rhinoplasty-nose-job/cost) and involved than primary procedures, and rhinoplasty is no exception. In fact, the revision is considered one of the most complex in plastic surgery. With that in mind, making an informed decision with the help of a highly skilled surgeon is key. Here, we hear from two patients who had a [revision rhinoplasty](https://aedit.com/procedure/revision-rhinoplasty) to learn more about their experience and any advice they have for people considering the surgery.

## **When Is Revision Rhinoplasty Appropriate?**
Although revision rates are not published, it is generally accepted that it is around 20 to 25 percent for rhinoplasty. That means one in every four or five patients pursues a corrective surgery down the line. There are two main reasons a [revision rhinoplasty](https://aedit.com/aedition/what-you-need-to-know-about-revision-rhinoplasty) may be necessary. One is chiefly cosmetic in nature and the other has a medical component. 

1. You’re unhappy with how your nose looks and would like to further refine the results
2. Your primary surgery was meant to address a deviated septum or other anatomical issue, but did not resolve the problem(s)

Rhinoplasty has one of the longest [recovery periods](https://aedit.com/procedure/rhinoplasty-nose-job/recovery) of any cosmetic surgery. While you may be cleared to resume most normal activities after a few months, it can take a year (or more) for the  body to fully heal – that includes clearing up any residual [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery), however mild, that can influence how the nose looks. But what if your patience is not rewarded with your desired result? Or what if you have a rhinoplasty [as a teenager](https://aedit.com/procedure/youthteen-rhinoplasty) and, as an adult, aren’t thrilled with how the surgery has aged? Revision is an option.

[Functional rhinoplasty](https://aedit.com/aedition/difference-between-septoplasty-rhinoplasty), meanwhile, is medically necessary surgery often prescribed for people who have a [deviated septum](https://aedit.com/procedure/septorhinoplasty), cleft palate, polyps, or even trauma from an accident or injury. In these cases, the surgery will be considered ‘[failed](https://aedit.com/aedition/guide-to-revision-surgery)’ if the functional concern the surgeon sought to address with the procedure is not resolved after healing is complete (think: your [breathing issue](https://aedit.com/concern/breathing-issues) is still, well, an issue). Again, revision is an option.

In either instance, the first step is usually to meet with your surgeon to discuss the differences between what you were looking for and what you ended up with. You are [under no obligation](https://aedit.com/aedition/what-to-do-if-you-are-unhappy-with-your-plastic-surgery-results) to pursue revision with the same surgical team, and it is best to consult with multiple surgeons. Ultimately, choose someone who specializes in revision rhinoplasty specifically, as the secondary surgery is more nuanced.

Whether it’s the first revision or the fifth (yes, seriously), there is often a great deal of scar tissue to contend with, which may also impact the skin and its ability to be stretched and re-draped. If there is not enough cartilage available to reshape the nose while maintaining its function, grafting may be required. For all of these reasons, seeking out a highly skilled revision expert is key for both the safety and efficacy of the corrective procedure.

As women, there is nothing quite like the jolt of sheer panic that shoots through your body upon [finding a lump in your breast](https://aedit.com/aedition/what-to-do-if-you-find-a-lump-in-your-breast-breast-cancer-awareness). With statistical estimations of [one in eight women](https://www.breastcancer.org/symptoms/understand_bc/statistics) in the United States (that's roughly 13 percent) being diagnosed with breast cancer in their lifetimes, it presents a moment of uncertainty so anxiety-inducing that the term ‘fear by paralysis’ comes to mind. The comforting fact is that there are *many* lumpy presences in breast tissue that indicate noncancerous issues, yet the last thing you want to do is to ignore it. 

Being prepared and informed is exactly how you can get through this type of trying moment rationally. I am sharing my own experience with fibroadenoma – and some expert insight from top surgeons – in the hopes that it will help others who find themselves in my position. 

## **My Fibroadenoma Diagnosis**
I first noticed a lump in my right breast at 31. It was relatively small, felt moveable and slippery — almost like a little blueberry beneath the surface of my skin — and was not visible from the outside. I remember reaching out panickedly to two of my girlfriends in a group chat, asking, “Have either one of you ever found a lump in your breast!?” One replied that she had not, but the other shared that she had. Her experience was what helped me overcome my fear and book an appointment with my OB-GYN.

At my appointment, my doctor reassured me, stating that the type of lump I presented with was different from the types of emergent masses that typically worry healthcare providers. For her peace of mind and mine, I was referred to a diagnostic mammogram and was diagnosed with a fibroadenoma.

…Fast forward two years. I had just moved back from Paris, France, where I was supposed to be living for two years, but my stay was cut short due to the COVID-19 pandemic. My unwelcome friend was still present and, unfortunately, felt much larger. While still invisible to the naked eye, it had become easy to feel. I worried that if it grew anymore, it would start to create an unsightly bulge.

Back to the doctor I went. I was again referred for a diagnostic mammogram and biopsy. If you ever find yourself in this position and experience fear that you deem to be irrational — in my case, it was because I had already received a benign diagnosis in the past — you are not alone. This is an objectively nerve-wracking process. During the mammogram, your breast is positioned on the machine and photographed. The most uncomfortable part is the squeezing. You are numbed for the needle biopsy, but the noisy (it sounds like a staple gun), handheld machine does leave an extremely small scar. I nervously got through the procedure thanks to an incredibly supportive staff. “Ninety-nine percent of the time, these are absolutely nothing,” the nurse told me comfortingly. “But we just want to make sure, because sometimes — rarely — they surprise us.”

Even though my fibroadenoma is benign, it's large in size. And, because it grew significantly over the course of two years, it was time to get it removed. Before scheduling my surgery, I met and consulted with both of the doctors below — a breast health expert and surgeon and a board certified plastic surgeon — in order to discover my best bet for removal. As a beauty editor, I was also curious as to whether the process would impact the integrity of my breast shape and size post-surgery or if the diagnosis could interfere with getting breast implants. Here is everything I've discovered throughout the process, from my diagnosis to my non-surgical removal.

## **What Is a Fibroadenoma?**
“Fibroadenomas are growths within the breast tissue made up of healthy tissue elements,” says [Heather Richardson, MD](https://www.bedfordbreastcenter.com/mastectomy-surgeon-heather-richardson-md), a general surgeon and breast health expert at the Bedford Breast Center in Beverly Hills. “They are not associated with breast cancer and don't turn into breast cancers.” As she explains, their presentation is typically described as “slippery” and having “an almost bubbly sensation” when a breast exam is performed. They are round with smooth edges and feel as if they are moving beneath the tissue should you try to secure it. “This is in sharp contrast to most breast cancers, which feel very hard and stuck in place,” Dr. Richardson shares. 

Present in approximately 20 to 25 percent of patients (with only little familial significance), those who have been diagnosed with a fibroadenoma are 20 to 25 percent more likely to experience another in the future. Fibroadenomas can appear at any age, though they most often occur in women between the ages of 15 and 35. They are usually diagnosed with ultrasounds. “Mammography is also used to gain more information about the nature of these growths, but ultrasound is the study of choice,” Dr. Richardson notes. Fibroadenomas are identified as smooth round or oval masses of tissue (in my opinion, it looked like a potato on the screen) and tend to be monitored over a period of time. Some growth is normal, and the protocol for monitoring and treatment changes as you age. 

In your twenties, the fibroadenoma will typically be monitored by your doctor in six month intervals. But when you get to your forties and fifties (or, in my case, thirties), the likelihood of biopsying the fibroadenoma becomes more likely. Dr. Richardson explains that this is simply to ensure that it is not something malignant presenting atypically. “We should also consider a biopsy of a suspected fibroadenoma if it is enlarging over time or has a change in shape or consistency,” she continues. Furthermore, if there is ever a questionable finding or a family history of cancer (or even a high level of patient anxiety regarding cancer), there should be a conversation about the risks and benefits of proceeding with a biopsy.

Another thing to keep in mind: “Very rarely, if they are very large or have been present for a long time, fibroadenomas can start to grow very quickly and can be reclassified as a type of tumor growth called a phyllodes tumor,” Dr. Richardson says. Again, this is *not* related to cancer. “They are typically treated with removal and do not require additional treatments,” she adds.

## **Removing a Fibroadenoma**
The removal of your fibroadenoma is largely dependent upon its position. Mine was located close to the surface of the breast. Sometimes doctors will recommend their removal, and sometimes it is patient-initiated because monitoring them with in-office visits can be a nuisance (not to mention, feeling a lump can – as a rule – be unsettling). “Patients may want to remove biopsy-proven fibroadenomas if they are noted on imaging to be changing or growing significantly,” Dr. Richardson says. Additionally, “if the patient feels that they are symptomatic, causing pain, discomfort, a cosmetic change in the breast, or even anxiety,” removal is an option, she notes. 

In cases where the patient has multiple fibroadenomas, the doctor will often only remove the more concerning ones and leave the rest in order to preserve the aesthetic appearance of the breast. There are both non-surgical and surgical removal methods, which we outline below:

### **Non-Surgical Treatments**
In the past, Dr. Richardson frequently performed cryoablation, wherein a probe is used to freeze the unwanted, internal breast tissue, destroying the area with the fibroadenoma. Over the course of a few months, the dead or dying tissue temporarily hardens, but then will slowly be flushed away by the body — somewhat akin to [CoolSculpting®](https://aedit.com/procedure/cool-sculpting) for the breast. “The downside is that we typically do still want to monitor the areas that have undergone cryoablation in the past, so those patients would not be simplifying their follow-up as they might hope,” she says.

More recently, Dr. Richardson prefers using the BARD vacuum-assisted biopsy tool, which presents patients with a minimally invasive (and more affordable) alternative to surgery. “It offers an excellent alternative for patients who don't want to have surgery or have multiple areas that would create a lot of surgical trauma in the healthy tissue if they were all removed at once,” she says. She recommends the process for small-to-medium fibroadenomas – roughly 2 centimeters or less – that are found deep in the breast. 

The treatment is performed awake with numbing agents. You will have the option for a prescribed sedative, like Xanax, but, in that case, you will need someone to drive you to and from the appointment. The entire fibroadenoma removal can be completed in an hour, with the bulk of which is taken up by the numbing. She says to think of the gradual removal process like an electronic Pac-Man that is taking bites from the fibroadenoma. The first couple of days might consist of some bruising and soreness, which can be managed by over-the-counter pain relievers.

Some doctors also use [radiofrequency (RF)](https://aedit.com/aedition/radiofrequency-skin-tighening-fat-reduction) and [ultrasound](https://aedit.com/aedition/sofwave-for-skin-tightening-collagen-production) energy to remove fibroadenomas, however, the critique in the aesthetics industry with these types of devices is that they can — in the hands of the wrong provider — result in the unintended loss of fat. Needless to say, many women would not welcome this side effect. 

### **Surgical Treatments**
The surgical excision of a fibroadenoma is, obviously, the more invasive option. It is most appropriate for larger masses. Post-op, the area will no longer need to be monitored, and, in certain cases, surrounding tissue can be sent out for analysis for greater reassurance. The procedure is typically handled by a general surgeon specializing in breast health, though, in instances where the patient is already considering a cosmetic procedure, a plastic surgeon may be involved. 

The lump may be able to be removed as part of a larger surgery. “It is rare that I would see a patient simply for a fibroadenoma, however, I will sometimes remove them in conjunction with breast surgery like breast reduction, breast lift, and breast augmentation,” says [Robert Cohen, MD](http://www.drrobertcohen.com), a board certified plastic and reconstructive surgeon in Beverly Hills. “There is obviously a scar after this surgery to remove the fibroadenoma, but often the scar can be hidden along the edge of the areola or in the breast crease.”

### **The Cost of Removing a Fibroadenoma**
Of course, cost is a significant factor to consider when choosing a treatment option. Because the surgery would have been an out-of-network procedure and my [insurance](https://aedit.com/aedition/insurance-coverage-for-plastic-surgery-cosmetic-procedures) deductible has not yet been reached, surgery would have been much more expensive for me (think: over $6,000). That being said, if you are in-network, it might be the better option for you. 

For reference, the cost of the BARD procedure was $2,000 out-of-pocket. Insurance has not yet caught onto the ease with which the tool can be used to excise fibroadenomas; thus, they only allow providers to bill for a biopsy. “The reimbursement is the same whether you take a pinch from a core needle that costs the center $20 or if you spend 20 to 60 minutes with a much more expensive device,” Dr. Richardson explains. Thus, not many centers offer the procedure for fibroadenoma removal. If, like in my case, the out-of-pocket cost is still cheaper than the price of what surgery would be, however, it provides a pretty compelling alternative.

### **Will Fibroadenoma Removal Leave An Indentation?**
Depending on the placement and size of the fibroadenoma, its removal may result in an indentation or even a change in the size of the breast. Our experts point out that this is most often the case when there are several fibroadenomas removed or a larger mass. “Quite often, the body will contract the tissue together as it heals and these depressions become less noticeable with time,” Dr. Richardson shares.

But this is not always the case, which is when an aesthetic procedure, like fat grafting, may become appealing. Following your fibroadenoma removal, Dr. Cohen advises waiting to see how the body heals itself before coming in to be assessed for a corrective treatment. “If there is a permanent or noticeable depression, the injection of [harvested fat] in tiny, thread-like injections through fat grafting is an excellent way of building back volume in an uneven area of the breast,” Dr. Richardson confirms. “This is considered safe and performed frequently for patients who have been treated for cancer and have uneven areas of the breast as a result.”

For the uninitiated, fat grafting requires fat be harvested from a donor site (typically from areas like the outer thighs or flanks) via liposuction, purified, and then injected into another area of the body (i.e. the breasts). Naturally slim or athletic patients run the risk of not having enough fat to harvest, while the best case scenario could actually yield enough fat to [augment the breasts](https://aedit.com/procedure/breast-augmentation-with-fat-transfer). “The more experienced the surgeon with aesthetic surgery of the breasts, the better the result will generally be,” Dr. Cohen says.

Though extremely rare, permanent deformity or an alteration to the size of your breast might lead you to consider [breast implants](https://aedit.com/procedure/breast-augmentation-with-implants). Even so, as one of the top five [most popular](https://www.plasticsurgery.org/documents/News/Statistics/2020/plastic-surgery-statistics-full-report-2020.pdf) cosmetic surgeries in the U.S., you might be considering breast augmentation anyway. Dr. Richardson assures us that the presence of a fibroadenoma need not deter you from undergoing any cosmetic breast surgery. “The two decisions should not have major impacts on one another,” Dr. Cohen adds.

But there are some things to consider if you have been diagnosed with a fibroadenoma. For example, if the fibroadenoma is very superficial, the addition of the implant could make it more visible. “In these cases, I would generally do both an augmentation and fibroadenoma removal with possible fat grafting, if needed,” Dr. Cohen explains. He has also had cases where the fibroadenoma was in deeper tissue, and he was able to remove it while placing implants. As an added precaution, Dr. Richardson recommends having your plastic surgeon work with a breast health specialist to ensure that the tissue is removed cleanly, expertly, and aesthetically. 

## **My Decision**
After consulting with both doctors, I decided to go through with the removal of my fibroadenoma. Dr. Cohen helped me decide to wait approximately three months to see if it leaves any sort of indentation, at which point I could consider fat grafting. As I am quite petite, there will only be enough fat to fill any hollowness that is, perhaps, present. In other words, any increase in volume would have to come via implants (and I'm not ready for those just yet).

For my removal itself, I opted for minimally invasive removal using the BARD device with Dr. Richardson. She is one of the few doctors in the entire country that removes fibroadenomas this way, and her practice graciously fit me into an appointment the very next day after my consultation — minimizing the timeframe I would otherwise experience anticipatory anxiety.

The entire appointment lasted a little bit longer than an hour. The bulk of the time was spent numbing my right breast via injections. Because the numbing contains epinephrine (which helps to constrict the blood vessels and minimize bleeding), it actually made me feel physically nervous. I felt my shoulders hunch, my heart pounded, and my voice was shaky. On the inside, though, I felt calmer than I appeared. I opted not to take a prescribed sedative so that I could drive home and keep working, as I'm a Virgo with workaholic tendencies!

As mentioned, the BARD device is used like a little mechanical Pac-Man. Dr. Richardson expertly used an ultrasound to locate and watch the fibroadenoma during the procedure, which is how she found a second — previously unknown — mass that is most likely another fibroadenoma. The larger of the two took 15 minutes to remove, a time frame she assures me is *way* longer than the norm because of its size. The other literally took five minutes! Both will be sent off to pathology to confirm the diagnosis because it is possible that the larger of the two is a low-grade phyllodes tumor.

Following the procedure, they applied a tan bandage that will remain on for five days over the tiny incision in my areola (it looks like a large pin prick!) and covered the area with gauze. Then, they wrapped me in a bandage that felt reminiscent of a corset. My breast remained numb until just before I went to bed, at which point I took two Tylenol. My outpatient paperwork indicates that I can expect soreness for a few days, but, to be frank, I cooked myself dinner the same evening as my removal and have been easily lifting my small dogs without any trouble since. I will definitely take a day off from any exercise, but am considering a ‘non-vigorous’ elliptical session tomorrow.

As for the breast itself, the appearance has not been altered a single bit. Upon touch, it feels eerily hollow for now — though it's still much better than feeling the mass — and Dr. Richardson assures me that my breast's natural tissue should fill in the space over the next three months. At that point, if I have any hesitations at all, I will be heading straight back to Dr. Cohen's office for fat grafting. And who knows? Maybe by then I'll feel [ready for implants](https://aedit.com/aedition/how-to-choose-the-right-breast-implants), as both surgeons assured me that, if and when I make that decision, my diagnosis of fibroadenomas should absolutely not be a deterrent.

I am a soon-to-be-31-year-old beauty editor, and, for as long as I can remember, I have considered my laugh lines to be no laughing matter. Many of us have a facial feature we fixate on. In my case, that feature has always been my [nasolabial folds](https://aedit.com/aedition/nasolabial-fold-treatments). Those parentheses-like creases look different on everyone. For the women in my family, they tend to run deep — and get deeper with age. 

I mention the fact that I am a beauty editor because it means that I have long been spoiled by the opportunity to try great skincare products. I have a [pretty solid routine](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-30s) that includes [exfoliating acids](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) to improve tone and texture, [antioxidants](https://aedit.com/aedition/facts-about-vitamin-c-in-skincare) to brighten and repair, and, of course, [daily sunscreen](https://aedit.com/aedition/best-sunscreens-that-act-as-makeup) to protect. But even the best lotions and potions cannot do much (if anything) to soften said folds. So, what’s a girl to do? 

As I shared a couple years ago, [I started getting Botox® injections](https://aedit.com/aedition/everything-you-need-to-know-before-getting-botox-for-the-first-time) just shy of turning 29 in an effort to get ahead of the increasingly omnipresent nature of my forehead lines. Last month, I headed to see [Lesley Rabach, MD](https://www.instagram.com/lmmedical.nyc/), a double board certified facial plastic and reconstructive surgeon, co-founder of [LM Medical](https://lmmedicalnyc.com/) in New York City, and my go-to aesthetic provider, for a neurotoxin touch-up with nary a thought of doing anything else. During our quarterly catch-up session (she fills me in on all the interesting things she’s noticing in her practice and patiently answers my questions about some of the trends we’re considering covering here at The AEDITION), however, something compelled me to broach the subject of my stubborn smile lines.

As Dr. Rabach emphasized, some amount of creasing is necessary in the region — after all, babies are born with nasolabial folds — but there are both surgical (hi, [deep plane facelift](https://aedit.com/procedure/deep-plane-facelift)) and non-surgical (think: filler) solutions that can help soften their appearance. In my case, half a syringe of [hyaluronic acid (HA)-based filler](https://aedit.com/aedition/everything-you-need-to-know-about-dermal-fillers) was all I needed to subtly yet substantially rejuvenate my face (keep reading to see before and after photos) for up to a year. Intrigued? Here’s what you need to know.

## **Returning to the Fold**
Also called laugh lines and smile lines, the nasolabial folds run from the corners of the nose to the corners of the mouth. Look back at your baby photos and you’ll see that those creases have always been there. “Even children have smile lines,” Dr. Rabach shares. As such, “it is imperative to maintain a fold because without it, we look very unnatural,” she adds. 

Depending on your anatomy, genetics, and lifestyle, however, the current state of your nasolabial folds may be quite different from their OG appearance. “As we get older, the cheeks [lose volume](https://aedit.com/aedition/best-procedures-to-restore-facial-volume) and, together with years of smiling, the nasolabial folds deepen,” Dr. Rabach explains. Yes, smiling is one of the causes of deepening folds, which means those lines are a sign of a life well lived and laughed. While I can’t help but smile at that, it doesn’t totally reconcile my relationship with said creases. 

From my teenage years through my mid-twenties, I always had a fuller midface. My nasolabial folds were fairly prominent then, too, but something about the overall proportion of my face seemed to camouflage them. By my late-twenties, my countenance bid adieu to any lingering baby fat, which added a bit more emphasis to those pesky parentheses.

It’s been in the last year or so, though, that I’ve noticed the most visible change. Doubling down on my [fitness regimen](https://aedit.com/aedition/what-its-like-to-get-emsculpt-neo), the stressors of everything going on in the world, and the anatomical realities of being a 30-something seemed to converge in a way that left my mid- and lower-face looking… blah. As someone who has always been conscious of my penchant for RBF (IYKYK), combining that with a leaner face and deepening folds made me look tired, stern, and just not like the best version of myself.

## **Fixing the Folds**
Sitting in Dr. Rabach’s Greenwich Village office on an unusually mild December afternoon, I explained to her the evolution of my smile lines and, based on how my mom, aunt, and grandma have aged, forecasted the future of my folds. While she was quick to quip that my nasolabial folds were no deeper than those of her four year old, she did admit that a little HA filler could likely go a long way.

This is, she says, “a common area to be treated with dermal fillers,” but the placement of the product can be customized based on the patient and their aesthetic goals. “Nasolabial folds can be treated either by directly filling into the fold or by [filling the cheek](https://aedit.com/procedure/cheek-augmentation-with-fillers) to lift the fold,” Dr. Rabach shares. “It depends on the individual anatomy which will be more natural and effective.” 

I was more familiar with the latter option ([augmenting the cheek](https://aedit.com/aedition/cosmetic-procedures-to-define-cheekbones) for a more lifted appearance), but, as it turns out, I am not an ideal candidate at this point in my life. “Younger patients, where the folds aren't so deep, often benefit from a very small amount of filler placed just in the deepest area to soften that,” she explains. In my case, the deepest area was by the nose. Put another way: Because my cheeks are (relatively) full, it's better to inject into the fold. “If your cheeks were flat, then injecting into the cheek is better,” Dr. Rabach notes. Either way, the goal is the same. “The goal is always maintaining a natural amount of depth,” she reiterates. 

Sure, [cheek filler](https://aedit.com/aedition/restylane-contour-cheek-filler) could help rejuvenate my folds, but it would also address a concern ([cheek volume](https://aedit.com/concern/cheek-volume)) that didn’t, well, concern me. As Dr. Rabach explained, the amount of filler I would need to see results, coupled with the fact that my goal was not cheek enhancement, made it inefficient from both a cost and benefit perspective. Instead, she said splitting a half-syringe of Versa™ by Revanesse® between my two folds would give me the subtly satisfying look I was after.

## **Fill ‘Em Up**
Until now, my experience with cosmetic injectables has been limited to [Botox®](https://aedit.com/aedition/where-to-use-botox-from-head-to-toe), which requires a bit of patience. Anyone who has ever gotten neuromodulators knows that it takes a minimum of a few days (and up to two weeks) to fully enjoy the effects of the wrinkle relaxers. Hyaluronic acid-based fillers, on the other hand, offer instant gratification.

While some areas of the face require [numbing cream](https://aedit.com/aedition/numbing-cream-breakouts) before filler injections, the nasolabial folds are not one of them. Dr. Rabach gave me a stress ball to squeeze, but, truth is, it hardly felt like anything more than a pinprick (there is lidocaine within the filler to further ease any potential pain). After two pricks into the upper part of the fold on my right side, she handed me a mirror to inspect the first half of the results. Much to my surprise — and delight — the crease was softer. She repeated the same thing on the other side and, in all of five minutes, I looked totally rejuvenated in the most understated way.

Dr. Rabach used Versa™ on me because, she says, “it is very versatile for the folds, moves with the person, and integrates well into the tissues.” Depending on the patient, however, she also likes [Restylane®](https://aedit.com/procedure/restylane), Teoxane Teosyal RHA® 3, and [Juvéderm® Ultra](https://aedit.com/procedure/juvederm) for this area. For most, HA filler results last nine to 12 months in the nasolabial fold region.

## **Recovery & Results**
Regardless of the product, the most common side effects of HA fillers are temporary swelling and mild discomfort for a few days. As with any injectable, [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox) is possible (though relatively rare in this part of the face). As you can see in my ‘after’ photos, I experienced a bit of redness at the injection sites for about 24 hours. After that, I found the area to be slightly tender to the touch (most noticeable when I washed my face) for about five days. 

A month later, I am pleased to report that the results have exceeded my expectations. Not a single person — including my ever-observant mother, with whom I FaceTimed when I got home from my appointment — has been able to pinpoint what I had done. Yet, when I show them my B&A, the results are abundantly clear.

For some, the subtlety may be a letdown. For me, it’s the draw. 13 Skincare Resolutions The AEDIT Team Is Making For 2022

Even though [cellulite](https://aedit.com/concern/cellulite) is incredibly common in female bodies (nine in 10, to be specific), I have never been able to accept it as a natural part of mine. The rippling appearance of cellulite generally begins during puberty, and I was no exception to the rule. First noticing it at the age of 13, it continued to be a bother into my early twenties. By the time I hit 30, I was seriously looking into [possible solutions](https://aedit.com/procedure/cellulite-removal-solutions). After having exhausted every at-home avenue to smooth my skin to no avail, I started to think the dimpling was something I would have to live with. That’s when [Qwo™](https://aedit.com/procedure/qwo) came into the picture.  

## **Why I Chose Qwo**
Anyone who has sought out ‘solutions’ for [cellulite](https://aedit.com/aedition/the-complete-guide-to-cellulite) will agree that no amount of dry brushing, foam rolling, coffee scrubs, firming creams, or lifestyle changes will truly change its appearance. It appeared to be a miracle cure for one of my biggest insecurities. I was excited enough just knowing that a solution was approved and within reach. But, when I was offered the chance to get Qwo™ injections administered by [Bruce E. Katz, MD](https://www.juvaskin.com/about/our-staff/dr-bruce-katz/), a board certified dermatologist and the founder of JUVA Skin and Laser Clinic in New York City who had been a part of the clinical trials for the treatment, I knew the time to tackle my cellulite had arrived. According to Dr. Katz, Qwo™ has been “tested on moderate to severe cellulite” and “can treat all forms of cellulite.”

Because the process requires three sets of injections spaced 21 days apart, I cleared my calendar and hurried to see Dr. Katz for my shot at smoother skin. 

## **Qwo Treatment & Recovery**  
During my first appointment with Dr. Katz, I was assessed and told that I was indeed a candidate for Qwo™ (cue the sigh of relief). He had been a part of the clinical trials for Qwo™ and had glowing reviews from his patients, who, two years post-treatment, were still happy with their results. After getting marked up and having my ‘before’ pictures taken (see below), I laid down for the first set of injections. 

As somebody unphased by needles, the process was seamless, and Dr. Katz went above and beyond to make me feel comfortable. Unlike [Botox®](https://aedit.com/aedition/everything-you-need-to-know-before-getting-botox-for-the-first-time), which is injected into the muscle, Qwo™ requires a different technique. When the needle is inserted, collagenases (the enzyme that breaks down peptide bonds in collagen) are released as the needle is moved around to dissolve the collagen bond. While, from the outside, it appears as though the needle is being moved around, Dr. Katz explained that this is the necessary method to ensure the bands dissolve and release, thereby reducing the dimpling appearance. The overall pain level was no more than that of other injections, and the entire process was done in under 10 minutes. I was instructed to stay on my stomach for another 10 minutes to allow for the injectable to get to work on the fibrous septae.

Now, although I did my [research on the treatment](https://aedit.com/aedition/what-its-like-to-get-qwo-for-cellulite) — reading up on aesthetics is, after all, my job — I was unaware of the dramatic impact the first round of injectables would have on me. During my initial appointment with Dr. Katz, I was reassured that [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox) and [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) were to be expected. In fact, they’re a sign that the treatment is working. “That is indicating that the fibrous bands have been dissolved by the Qwo™,” he explained. I am prone to bruising, so I thought I had a pretty good idea of what the bruises would look like on me. However, I wasn't prepared for what happened a mere 11 hours post-injection. 

As the first day progressed, I started to experience a feeling of soreness as though I’d done a glute-heavy workout, and my skin felt raw as if I had a sunburn. Not knowing what I would experience that day, I wore jeans and soon found myself feeling irritated by the denim fabric. By the end of day one (and after constant inspection of my derriere), the small injection sites had blossomed into deep purple, baseball-sized circles. Nothing could have fully prepared me for the shock factor of the bruising, but having the forewarning from Dr. Katz allowed me to sleep easy — albeit somewhat uncomfortably — the next few nights. By my second set of injections three weeks later, the bruising had disappeared and I was ready to do it all again.    

The following two treatments mirrored my first experience — sans the significant bruising. I went in, got marked up, took progress photos, and felt the same sore-raw hybrid of sensations with limited bruising (compared to what I had come to expect from round one). The entire process, from consultation to completion, lasted less than three months, and I was excited to see a vast improvement. 

## **Qwo Results & Recommendations**
Once the swelling and bruising from the first set of injections went down, I noticed a slight improvement in the appearance of my skin. The real change, however, came at the three-month mark. After the third set of injections, the bruising was barely there — though it did linger for a few weeks (Dr. Katz assured me that this was perfectly normal).  

While my case of cellulite may fall in the ‘moderate’ category, in unflattering light or under constrictive clothing, I still found myself concerned about its appearance. Now that enough time has passed, I find that my skin is much smoother, the thought process behind choosing what to wear is less complicated, and my self-confidence has been restored.

As far as the timeline for how long the results should last, there’s no specific answer. Dr. Katz has patients who participated in the trials two and a half years ago and are still happy with their appearance. Even if there is a shelf life to my results, I would gladly go back and do it all again as needed. I would also recommend it to anybody who wants a minimally invasive solution that produces visible results. 

For those considering this treatment option for cellulite, here are a few things I think you should keep in mind when scheduling your Qwo™ injections: 

### **Do**: 
- Schedule treatment for a time when you’re less likely to wear revealing bottoms
- Consider what you will wear the day of injection
- Wear underwear that doesn’t dig into the injection site
- Plan your days following the injections to be light and know that sitting or physical activity may cause discomfort
- Expect bruising, especially if you are prone to it 

### **Don’t**: 
- Schedule any physically demanding activities post-treatment 
- Push yourself physically in the days following 
- Wear anything too compressive or with a rough texture for the first few days
- Feel discouraged by the bruising, swelling, or soreness (trust the process!) 

## **The Takeaway** 
Cellulite is not a reflection of your health, diet, or fitness capabilities — it's a genetically determined structure under your skin that creates a rippled appearance. While it is currently only FDA-approved for use on the buttocks, Qwo™ is being tested for use on the back of the thighs and has shown realistic improvement to a concern that previously was hard to treat. Despite the deep bruising and discomfort for a few days, the process is quick and the results are real!

As a beauty editor, there’s not much I wouldn’t try when it comes to smoothing [fine lines and wrinkles](https://aedit.com/concern/fine-lineswrinkles). I’ve been obsessed with my anti-aging skincare regimen since my [late twenties](https://aedit.com/aedition/best-skincare-routine-products-ingredients-for-20s), and I started getting Botox® in my forehead and [around my eyes](https://aedit.com/procedure/periorbital-botox) as another preventative measure by the time I was 30. Every 12 weeks, I would sit in my dermatologist’s chair in New York City and smile as I faced one of my biggest fears – needles. As the years went on, I noticed my Botox® wasn’t lasting as long, and, as someone who is Type A, I began going in every eight to 10 weeks. That’s when the switch from Botox® to [Dysport®](https://aedit.com/procedure/dysportr) was recommended. 

## **What’s the Difference Between Botox & Dysport?**
The truth is, at a chemical level, there isn’t much of a difference between Botox® and Dysport®. The active ingredient in both [neurotoxins](https://aedit.com/aedition/comparing-neurotoxin-botulinum-type-a-injections-botox-dysport-jeuveau-xeomin) is botulinum toxin type A, which means they work in the same way. “All type A botulinum toxins have the same mechanism of action: blocking the release of acetylcholine and preventing the muscles from contracting to maintain the aesthetic effect,” explains [Dennis Gross, MD](https://www.dennisgrossmd.com/), a board certified dermatologist and founder of his [eponymous skincare line](https://go.skimresources.com?id=170670X1638612&xs=1&url=https%3A%2F%2Fdrdennisgross.com%2F). 

[Botox®](https://aedit.com/procedure/botox), or onabotulinumtoxinA, was first approved by the United States Food and Drug Administration (FDA) to temporarily improve the appearance of frown lines between the eyebrows in April 2002. It has since received more aesthetic approvals, in addition to its therapeutic uses. [Dysport®](https://www.dysportusa.com/what-is-dysport) (a.k.a. abobotulinumtoxinA), meanwhile, was first approved to treat glabellar lines in April 2009. Other cosmetic neurotoxins on the market include [Xeomin®](https://aedit.com/procedure/xeomin) and, the newest to the scene, [Jeuveau®](https://aedit.com/procedure/jeuveau).

While the active ingredient in each product is BoNT-A, they also feature unique protein blends that can affect the diffusion and wear of the injection. “The amount of core active neurotoxin in each on-label dose is different,” Dr. Gross says. “Dysport® has shown to include more core active neurotoxin compared to other neurotoxins.” 

Another point of difference? The appearance they each give is very different to my editorial eye. When treating my forehead lines, I find Botox® to give a more localized freezing, whereas Dysport® spreads evenly throughout for a more air-brushed effect. “I love the results that Dysport® gives since the results are natural-looking — fresh but not frozen,” Dr. Gross notes.  

## **How Long Does Dysport Last?**
For me, Dysport® lasts about 12 weeks just like [Botox® used to](https://aedit.com/aedition/botox-immunity-neurotoxin-resistance). For some, it can last up to five months (yes, I’m extremely jealous of those people). For others, it may be less. “The time length varies from patient to patient depending on genetics – some people break down neurotoxins at quicker rates than others,” Dr. Gross shares. 

But that’s not all. How you take care of your skin [in between appointments](https://aedit.com/aedition/maintaining-skin-with-devices-skincare-products-between-cosmetic-procedures) matters, too. He recommends using a consistent skincare regimen that includes active ingredients like [vitamin C](https://aedit.com/aedition/guide-to-topical-vitamin-c-in-skincare), [retinol](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare), hydroxy acids ([alpha](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) and [beta](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare)), and [niacinamide](https://aedit.com/aedition/guide-to-topical-niacinamide-vitamin-b3-in-skincare) to stimulate collagen production and prolong the life of your treatment.

To be honest, I never really thought much about my breasts — until I started to lose them. They were always on the smaller side of average, yet, due to my height (I’m 5’9”), I believed they looked even smaller and flatter than they actually were. I felt like my shoulders and the flatness of my torso made my upper half just look… long. But, once I started to run marathons, the volume loss and deflation became a serious insecurity for me.

## **The Decision-Making Process**
As much as I told myself that there was something impossibly chic about having tiny breasts (I mean, it’s good enough for Kate Moss!), I couldn’t stop myself from obsessing over my chest after every single run. I even bought a seamstress-style tape measure. And then a second one. I was certain the first was wrong — there's just *no way* that my A-cup bra could be baggy. It must be the tape.

It was a rude lesson to learn that losing weight is the same as gaining it. We can’t choose the body area where it comes and goes, and that deflation from volume loss is *real* (I still don’t know why I thought they would stay perky if they were flat as a pancake). But it wasn't until one Saturday, about six miles into a long run, that I realized I wasn't wearing a sports bra (or any bra for that matter) and I hadn't even noticed. At that moment, I began to seriously consider medical intervention in the form of [breast augmentation with implants](https://aedit.com/procedure/breast-augmentation-with-implants).

Then the [COVID-19 pandemic](https://aedit.com/aedition/plastic-surgery-dermatology-cosmetic-dentistry-practices-reopening-during-covid-19) happened. I certainly couldn’t stop running now — it was my only activity! But, as it did for many, the pandemic certainly put things in perspective. I always expected to get a ‘[mommy makeover](https://aedit.com/concern/mommy-makeover)’ after I had children, with a breast procedure or two on the list. But, over the last year, my perspective evolved into: What the heck am I waiting for?

## **The Consultation**
“How much volume did you lose?” my plastic surgeon, [Adam Kolker, MD](https://www.drkolker.com/), immediately asked when I began our [breast augmentation](https://aedit.com/procedure/breast-augmentation-solutions) consultation by blaming marathons for my newfound insecurity. *“How did he know?”* I thought, as we sat socially distanced with me in a chair on one side of the office, Dr. Kolker on the opposite side, and his right-hand RN, Ashley, standing against the wall between us. All of us were masked, temperature-checked upon arrival, and sanitizing our hands like crazy. Although we were all vaccinated, both rising cases and the chaotic Delta variant were increasingly worrisome. 

During the examination portion of our consult, Dr. Kolker and Ashley measured my breasts in a few different ways (think: the collarbone to nipple, nipple to base, and width, among others). We discussed my goals — “natural but juicy,” I told him — but my main concern was balancing out my tall silhouette with implants that look like they belonged on my body’s shape. Then they took the ‘before’ photos and opened a briefcase of implants to try on and take for a test drive. Dr. Kolker recommended I ignore the [measurements on the implants](https://aedit.com/aedition/how-to-get-the-most-natural-looking-breast-implants) and decide what I liked based on the look and feel, not the number of CCs. 

Left alone, I tried on every single one, starting with the largest. I placed each set of implants in my bra and modeled from every angle. I felt like Goldilocks, and giving the different sizes a test run was immensely helpful for me to determine just how large I wanted to go. When Dr. Kolker returned to wrap up our conversation about size goals, I told him that I thought 250-ish CCs looked the most proportional to my body. He noted my preference and explained that the final decision on CC amount is decided in the OR, based on several factors that are revealed only once the incision is made. We also went over his instructions for pre-surgery care. As I was heading out, Ashley asked me to send a photo of my COVID-19 vaccination card before the big day. 

## **The Surgery**
On the day of the surgery, I arrived alone — no companions allowed — and went over what to expect before and after the surgery with Kat, the recovery room nurse. Dr. Kolker then came in to review our surgery goals once again, take a few more photos, and finally — the dramatic moment I was waiting for — [draw on my body with a Sharpie](https://aedit.com/aedition/purpose-of-a-plastic-surgeons-markup-pen-during-cosmetic-surgery) just like they do in movies. At that moment, it all felt very real. I thought to myself, *“I have Sharpie on my body, it’s REALLY happening!”*

In the last, and less dramatic, step before walking to the OR, I had a tête-à-tête with the [anesthesiologist](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia) to align on our sedation plan. I asked him what the likelihood of accidentally waking up mid-surgery (a new-found fear), and he said he was zero for 60,000-plus, meaning that he’s managed more than 60,000 surgeries and zero patients have woken up before they were meant to. I felt very safe and that weird new fear became a memory as we walked to the [operating room](https://aedit.com/aedition/unexpected-things-to-know-about-the-plastic-surgery-operating-room) to meet Ashley and Dr. Kolker.

I was very impressed with the OR — all of my information was splashed up on a wall-mounted TV across from Dr. Kolker, music was playing, the temperature was chilly (my favorite), and Ashley put a pillow under my knees for extra circulation. It felt kind of like a spa, until the anesthesiologist attached the needle. 

Next thing I know, I woke up in the recovery room with Kat, who told me that my friend was on her way to the office to pick me up. Three juice boxes later, I was cleared to go home and hopped in an Uber.

It was all very efficient and speedy: neither Dr. Kolker nor his nurses overstayed or lingered. They came in, got down to business, answered my questions with care and compassion, and left because, after all, we were still indoors, in small treatment rooms, during a pandemic.

## **Post-Op**
Happily, the [breast implant recovery process](https://aedit.com/aedition/breast-augmentation-recovery-healing-process) wasn’t as intense as I predicted. Pain was manageable, and I only took prescription pain killers for the first two days and then just Tylenol after that. The first few days post-op were mostly just uncomfortable. My chest felt pressure but rarely pain unless I forgot I just had surgery and tried to open our heavy fridge door. [Sleeping](https://aedit.com/aedition/sleeping-better-after-cosmetic-surgery-aesthetic-procedures), however, was nearly impossible for the first week. Imagine trying to sleep sitting up straight — and I mean stiff-as-a-board-straight — with a [pillow fort](https://aedit.com/aedition/best-pillows-for-plastic-sugery-recovery-healing) all around so you are immobile. 

Dr. Kolker called me the day after surgery to check in, and I went back into the office on day seven for my first post-op appointment. On my post-care sheet, Dr. Kolker recommended a few [specific options for sports bras](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) to wear at one-week post-op, and I brought them along. I was temperature checked, masked, and ushered into a treatment room, where Dr. Kolker inspected [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery), the incisions, and the implants, followed by taking a set of photos. I was instructed to wear a heavy-duty sports bra 24/7, along with a white strap that sat right above the implant to help with settling.

Two days later, I had a mini freakout that my sports bra was compressing my implant too much and called the office. Rather than have me come by (which I suspect they would have had me done in ‘Normal Times’), Dr. Kolker hopped on FaceTime to visually inspect my compression concerns and reassure me.

On day 14, I began a set of three implant displacement ‘massages,’ which are very much not massages but rather awkward-feeling push and pull movements to encourage the implant to move, reducing the risk of capsular contracture. I was cleared by Dr. Kolker to take long walks and even a quick dip in the pool, but I still had to wear the sports bra and strap for most of the day. 

At my week four in-office post-op appointment, Dr. Kolker evaluated my healing progress and took another round of ‘after’ photos. I couldn’t stop gushing about how happy I was with the size and the totally manageable recovery. Somewhere around week three, I stopped feeling discomfort when lying down and sleeping became fun again. At the one-month mark, I was allowed to start wearing regular bras during the day, and I raced home to online shop — adding only the sexiest and laciest push-up bras to my cart.

## **My Takeaway** 
This past year, I’ve lost any sense of being able to evaluate what is weird. I may be in the minority, but I don’t think that my experience of getting breast implants during a pandemic deviated *that* much from what the surgery would have looked like in pre-COVID times. Aside from the masking, sanitizing, and temperature checking, it seemed business as usual. All of my post-op visits, just like my [consult](https://aedit.com/aedition/questions-to-ask-your-doctor-during-plastic-surgery-consultation), were very efficiently managed but never rushed, just the good sense to discourage anyone to linger in tight spaces unnecessarily. 

I couldn’t be more thrilled with my results. My body finally looks proportional and natural, which I always kind of thought was just a thing you say to be nice — but they *actually* look natural! Hands down, I’m happy that I didn’t wait. Pandemic or not, I’d do the surgery again.

It was weird, but definitely not as weird as I was expecting.

_Finding the right plastic surgeon is a lot like dating. You’ll likely have to consult with a few doctors — and maybe meet a frog or two along the way — before you find the best one for your cosmetic surgery._

_So, how do you know when you've found The One? There are a few important factors. For starters, he or she must be board certified by the [American Board of Plastic Surgery](https://www.abplasticsurgery.org/), must have attended an accredited medical school, and must perform all procedures in an accredited or state-licensed surgical facility and operating room. What makes them the right doctor for you beyond professional accreditation is a bit more subjective. In this series, we ask real patients to share their surgeon-selection stories to help guide — and safeguard — your own cosmetic surgeon search._

_The below interview has been edited for length and clarity._

## **Elizabeth, 52**
My two aesthetic plastic surgery experiences were at two extremes: the worst and the best.

The first time I had surgery was more than 12 years ago. I wanted my [nose reshaped](https://aedit.com/procedure/rhinoplasty-nose-job) and, during the consultation, was convinced by the doctor that I should also have a [chin implant](https://aedit.com/procedure/alloplastic-chin-implant) and some [liposuction](https://aedit.com/procedure/liposuction). The surgeon I chose was older — probably in his sixties at the time — and I figured he’d had lots of experience. He’d also appeared on _Oprah!_, which, in 2005, was a big deal. The first time I went to the surgeon's office, I took a friend from the beauty industry with me, and we were both impressed. Maybe less by him and more by his art-filled Upper East Side office and name-dropping, but I felt that if he’d had a successful practice for years, I’d be safe in his hands. Now, more than a decade later, I can see the red flags I missed.

### **The Immediate Warning Signs**

For starters, he smoked a cigar throughout our consultation. Seriously. He also talked at me the entire visit, bragging about his accomplishments and not really taking any time to get to know me or understand my concerns. He (successfully) upsold me on more surgery procedures — that chin implant and lipo — when I’d only come in about my nose. He never showed me his operating room, which was in the same building as his office. I also dismissed negative reviews I’d read online. At the time, chat rooms and community boards were relatively new and, while I did look at them, I didn't take them seriously. They were littered with negative comments about him, mostly referring to the fact that he had a terrible bedside manner. I should have paid closer attention. 

I could write a book about my negative plastic surgery story, but one detail sums up the experience: I woke up in the middle of my surgery to the doctor banging on my nose. That was emblematic of everything that followed. He botched my nose job, taking too much off the end and making my nostrils look over-exposed. He also left stitches inside my nose that caused an infection. He put in a chin implant that was too large and he overdid the liposuction, causing the skin in my stomach to permanently tighten and twist. In the end, he accepted no responsibility for anything and, while I did try to pursue legal action, my lawyer told me to save my money and put it toward reconstructive surgery. 

### **Second Time Around**

It was during the pursuit of corrective surgery for my nose that I found the [best surgeon](https://aedit.com/aedition/finding-the-right-plastic-surgeon-dermatologist-or-cosmetic-dentist). I first met him when I developed the infection in my nose. I’d been treated so poorly by the first doctor that, when the pain started, I didn’t trust him to take care of it. So, I searched online for the type of procedure I wanted and board certified plastic surgeons who had rave reviews when it came to nose jobs. I called the one whose name came up most often. I was desperate and scared. I explained my story to the nurse on the phone, which she communicated to him, and told me that even though he was done seeing patients for the day, he would wait at the office for me until I could get there. I was so thankful. He immediately put me on antibiotics and followed up with me until the infection was gone. 

He also helped outline the surgical procedures he thought would help me and explained that I would have to allow my nose to fully heal and wait a year before I could go under the knife again. In our follow-up meetings, he took the time to understand what I wanted and even asked me to draw a picture of what I wanted my nose to look like. He was an excellent communicator and clearly explained the kind of result I could realistically expect from these new plastic surgery procedures. He seemed to both understand what I wanted and be capable of creating it for me. I trusted his artistic skill as much as his medical acumen. I did wait a year until the second surgeon could do my corrective surgery. He restored my nose and removed the oversized chin implant. 

### **Lessons Learned**

I learned a lot during those two years of surgery and recovery. First, don’t dismiss what you read online. The negative reviews are good cautionary tales, but the positive ones are also important. People are more apt to pound out angry comments, but when someone takes the time to write about exemplary work, that’s just as telling. I also believe you should trust your gut. There were warning signs with the first doctor — the smoking, the over-talking, the disinterest in me — that made me wary, but I repressed that anxiety because of this doctor’s fancy office and television appearance. Always listen to your inner voice. Beware of any doctor who goes overboard telling you everything they think is "wrong" about you and all the additional cosmetic procedures they think you "need." If you go in for a nose job, that’s what you should be discussing. Finally, consider a surgeon with a specialty in the specific procedure you want. The second surgeon I chose is known for nose jobs — it’s what he does the most often. That turned out to be one of the most important things for me.

How I Found My Plastic Surgeon: Trust Your Gut

When I started noticing some early signs of aging around my eyes, I didn’t dash to the doctor right away. I was a little apprehensive about cosmetic treatments, even non-surgical options. I had mixed feelings about modifying myself in the name of beauty. I wanted to still look like myself and not like I'd had "work done." I wondered if I was being vain or succumbing to some perceived pressure from society's standards. After a bit of soul searching, I realized that I’d been letting fear of possible judgment interfere with my own desires. As long as a treatment is safe, both physically and psychologically, I believe people are entitled to self-improvement in all forms, cosmetic procedures included.

Armed with self-confidence, I took the plunge. I did extensive research and met with a board certified plastic surgeon who treated my wrinkles with [Botox®️](https://aedit.com/procedure/botox-facial) and my volume loss with a hyaluronic acid-based filler. I was pleased with the results, but not with the idea of periodic upkeep. I eventually opted to undergo surgical fat grafting, a procedure designed to provide more permanent facial volume. I was excited to talk about the surgical procedure with my family and friends. After all, my trusted inner circle would be accepting of anything that made me happy, right?

Not so much. While a few of my friends were genuinely interested and supportive, some others, particularly people related to me, were not as enthusiastic. Most of them didn't say it outright, but their body language, skeptical tone, and relative silence said it all. They didn’t really approve, but they didn’t want to hurt my feelings by admitting it. 

While their judgment didn’t lead me to regret my choice, it did make me question being so vocal about it. Should I have kept my cosmetic procedures a secret? I’d made subtle changes, so presumably keeping it under wraps would have been a possibility, but it didn’t feel right to me. I wanted to share my experience, rather than keep it from my loved ones for fear of a negative reaction. Was there a way to educate people about my cosmetic surgery in a way that wouldn’t prompt pessimism? Where was that unfavorable reaction coming from? In search of answers, I sought out plastic surgery professionals and other patients to learn more. 

“Good plastic surgery is invisible,” says [Jason Roostaeian, MD](https://aedit.com/provider/jason-roostaeian-md-449), Associate Professor of Plastic Surgery at UCLA and board certified plastic surgeon. “My goal is always to produce a result that looks as though my patient might've been born with it. Of course, depending upon the area and the extent of what we change, patients may or may not be able to keep their surgery to themselves.” 

As cosmetic surgery procedures become less detectable, it surely impacts people's decisions to disclose them. “As technical advances allow for increasingly natural-looking results, patients are actually having to face that dilemma more and more frequently," says Dr. Roostaeian. "Fortunately, the popularity of plastic surgery procedures, combined with increased exposure via social media, continues to significantly reduce the stigma associated with aesthetic surgery.”

Understanding that our society is becoming more accepting of cosmetic surgery made me feel less self-conscious, but I was curious how other patients had navigated talking to family and friends about their procedures.

Dr. Sean Saadat, MD is a plastic surgery resident at UCLA who has had two [rhinoplasties](https://aedit.com/procedure/closed-rhinoplasty). The first was at age 19, followed by a revision which he chronicled on social media. Dr. Saadat told me that he didn’t initially disclose his desire to change his nose because he felt admitting his insecurities about his appearance would make his parents nervous. He also worried that he’d be judged as a male seeking cosmetic surgery, which is stereotypically for females. “When I did bring it up, I said I was having trouble breathing, and that when the surgeon suggested fixing the prominent hump on my nose at the same time, I just kinda went along with it," he says.

Dr. Saadat's family turned out to be supportive. “Once I’d made the decision, my family stood behind me," he says. "Having your nose done is quite common in Iranian culture. Many women in my family had undergone rhinoplasties, so I felt like I was surrounded by a rhinoplasty support group.” He wasn’t so confident about his friends, however, so he lied and said he was having his wisdom teeth removed. “In my first couple of days post-operation, with my splint intact and eyes still bruised, I invited them over one by one for a big 'ta-da' moment," he says. "Surprising my friends felt satisfying and prevented them from saying anything negative beforehand. Ultimately, they were happy for me because they knew how insecure I’d been about my nose.”

Aubrey Nolan, co-founder of CrushFit, also felt apprehensive about revealing her cosmetic treatments. “I feel like there’s a huge stigma against getting any kind of work done, especially on your face," she says. "I got my [lips](https://aedit.com/procedure/permanent-lip-fillers) done and I was scared to even talk about it on social media. I feared what people would think of me and I considered not telling anyone.” But after giving it some thought, Nolan reconsidered. “I’ve been as transparent as possible with everything that I do.  I guess I just don’t see what the big deal is.” When a few followers responded negatively to her choices, she initially felt defensive, but now realizes that “getting work done doesn’t change the heart I have and I no longer feel the need to defend my actions.”

YouTube beauty blogger Elwa Saleh had a different concern surrounding her decision to have plastic surgery to correct her partial nasal collapse. Her family and friends were very supportive from the beginning, so Saleh says she “was not concerned about any judgment or scrutiny. But I was worried about the swelling lasting a long time since I make my living blogging about beauty and fashion. Most of my work is on camera and my face is a big part of my job.” After the procedure, “I did get a lot of questions about why I did it from my social media community," she says. "Some jumped to conclusions and thought I had major cosmetic surgery but once I spoke up about what I had done, the judgments dissipated.” 

One thing is clear: everyone has an opinion about cosmetic surgery and the more obvious the change, the stronger people tend to feel. But the degree to which people notice a person’s cosmetic surgery depends on more than just the procedure and the surgeon’s skill. It’s directly linked to the level of familiarity involved. The better we know someone, the more likely they are to notice a change. 

This connection was the key to understanding my family’s lackluster response to my own cosmetic surgery. They felt comfortable enough with me to express their uncensored emotions. Strangers risk this behavior, but social media has reframed what we consider familiar. All people are prone to pass judgment. “When you decide to go under the knife, that decision should be made with the understanding that people will judge because of their own insecurities and they may try to hurt your feelings," says Dr. Saadat. "If you do your homework and find a surgeon who will do a very good job, people will be pleasantly surprised to realize that you actually look like an improved version of yourself, and they’ll come to terms with it.”

Here are some tips for talking to your family and friends about cosmetic surgery: 

### **Choose Wisely**
“You need to be careful about who you tell beforehand," says Dr. Saadat. "They need to be open-minded people who aren’t going to be thinking about their own insecurities.” Although it’s impossible to really predict how a person will react to your decision to seek cosmetic surgery, confide only in those friends or family members who have your best interest and self-esteem at heart. 

### **Keep Calm and Confident**
If you’ve thoroughly considered every aspect of choosing cosmetic surgery, from examining your own psychological motivation to researching your procedure and cosmetic surgeon, someone else's snarky comments become insignificant. Relay your reasoning without defensiveness or excessive emotion. If you communicate your seriousness about improving your quality of life, your friends and family may be less likely to find fault in your choice.

### **Look Deeper**
Fear or insecurity could be behind someone's negative reaction. The topic of cosmetic surgery is so charged because it can tap into our deep insecurities and can sometimes bring up feelings of inadequacy in someone. Keep in mind, your loved one’s negative response may have very little to do with you.

### **Don’t Try To Win**
Stay focused on your purpose. You're having this conversation to inform your loved one about a decision that you’ve made for yourself, not to try to change their mind about cosmetic surgery. If things start getting heated, agree to disagree before feelings get hurt. 

### **Be Patient**
Sometimes time is all it takes for people to accept their loved one’s decision to undergo a cosmetic procedure. “At the end of the day, it’s the patient who truly has to live with their transformation," says Dr. Roostaeian. "If an aesthetic surgeon does their job well, the results will blend so seamlessly with their existing features that eventually, even their closest friends and family may forget there was ever a change.” 

### **Be Yourself**
While there’s no magic formula for talking to your friends and family about cosmetic surgery, keep these pointers in mind and be prepared for the onslaught of opinions. People who truly care about you will appreciate your honesty, so be as forthcoming as you feel comfortable being. Remember, how we present ourselves is up to us, but how we are perceived by others is beyond our control. Staying true to yourself will keep you feeling beautiful, inside and out.
