Despite the fact that I have heard from many friends and colleagues that they felt perfectly safe [going to a nail salon](https://aedit.com/aedition/what-its-like-to-visit-a-nail-salon-during-covid-19) for a manicure and/or pedicure during the COVID-19 pandemic, I haven’t made it in for a service. In fact, I haven’t had a professional pedicure since the summer of 2019. Yes, I am a beauty editor who loves to paint her [fingernails](https://aedit.com/aedition/how-to-treat-damaged-dry-brittle-finger-nails-cuticles) on a weekly basis and methodically layers creams and balms on her hands every night while pretty much neglecting everything that happens below the ankles.

For context, I live in New York City (which means I walk just about everywhere) and my go-to form of exercise during the pandemic has been socially distanced runs around my neighborhood. Needless to say, my [feet](https://aedit.com/aedition/cosmetic-treatments-procedures-for-feet-nails-toes) have seen better days. So, you can imagine my delight when I was invited to [Medi Pedi](https://medipedinyc.com/) — the midtown Manhattan mecca for medical-grade pedicures — to try the signature “medicure” service that is customized for every client depending on their needs.

## **What Is a Medical Pedicure?**
For starters, you might be wondering what differentiates a medical-grade pedicure from one you would get at your local nail salon. Perhaps the most obvious distinctions are the lack of water and nail polish, though there is plenty more that make medical pedicures unique. “Medical nail technicians are trained to detect and treat harder to deal with ailments like athlete’s foot, fungus, corns, and more,” explains [Marcela Correa](https://medipedinyc.com/about-1), a licensed professional medical pedicurist and founder of Medi Pedi. “Even when it comes to basic nail cutting, knowing the proper technique will prevent the nail from getting ingrown and causing the client more trouble in the end.” 

Whether you take great care in tending to your feet or treat them more as an afterthought, little issues can turn into bigger problems if they are not addressed properly. “More often than not, the reason why small ailments turn into more dangerous ones is due to lack of knowledge,” Correa says. “When getting a medical pedicure, not only do your feet get the makeover they need, you leave with the knowledge of how to maintain them healthy for a longer period of time.” 

A medical pedicure also has both immediate and long-term effects. “The treatment has many amazing benefits, with the main one being the prevention of serious infections,” Correa shares. The hygienic standards are — surprise, surprise — medical-grade, which assures the treatment is safe and effective for everyone (including those with underlying medical conditions like diabetes, she says). “Not worrying about if your tools are clean or if your nail tech knows about that weird thing on your nail gives you room to relax and enjoy the experience,” Correa adds.

## **The Treatment**
When I arrived at Medi Pedi, I was greeted by the masked office manager who immediately took my temperature and offered me hand sanitizer. I was then whisked away to my treatment room as to avoid unnecessary interaction with other clients. While Correa and her team have made some adjustments during the COVID-19 pandemic, hygiene and privacy have always been a priority.

Correa describes her signature service as “a facial for your feet.” I would liken it to a no-frills facial —  you know, the kind that is all about results — in the best way. The non-invasive, dry treatment lasts about an hour and focuses on the hygiene and aesthetics of the toenails and soles of the feet. It can target and treat athlete’s foot, corns, calluses, dry and cracked heels, ingrown and overgrown toenails, [nail fungus](https://aedit.com/aedition/what-your-nails-say-about-overall-health), nail discoloration, and more.

### **Step 1**: Cutting & Filing the Nails
Correa’s long-time associate Alejandra began my treatment by asking me if I was experiencing any pain, as she creamed my heels and placed (ingenious) silicone slings on top. I was not, and she quickly got to work manually cutting and filing my nails and cuticles. When she noticed pesky corns on each of my pinky toes, she cut them out with ease.

### **Step 2**: Buffing the Nails & Cuticles 
Next up? The electric nail file to buff my nails and remove the buildup of dry, dead skin. There were different attachments used for different toes, and she asked me to tell her if I experienced any pain or discomfort along the way (I didn’t). I had a small crack in my right big toenail that she was able to smooth, and the file removed all traces of surface stains from the nail plate. A dollop of cuticle oil on each toe started to rehydrate the freshly filed skin around my nail beds, and she finished up this part of the process by removing the silicone slings from my heels.

### **Step 3**: The Diagnosis 
At this point, Correa came into the room and immediately detected the very early stages of athlete’s foot on my soles. She recommended I pick up an over-the-counter antifungal cream on the way home to nip it in the bud (needless to say, I took her advice). She also asked if I walk around barefoot at home. I sheepishly told her I do. She thanked me for my honesty and told me she could tell. Even so, she said my feet were in good shape. I thought she might be trying to make me feel better, but Alejandra confirmed the damage was mostly cosmetic.

### **Step 4**: Buffing the Bottoms
Now it was time to focus on the bottoms of my feet. Correa applied moisturizer (a must before buffing, as not to damage healthy skin) and got to work on my heels with a single-use foot file. She recommends her clients do this once a week at home, filing in the same direction as the cracks. Her one rule? Skip the pumice stone because it harbors bacteria. She buffed my soles as well, and it’s safe to say they’ve never been smoother. All the while, she placed my shoes under a UV lamp for 15 minutes to kill any bacteria that was causing them to be, in her words, “sick.” 

### **Step 5**: Finishing Touches
As I mentioned, there is no polish with this pedicure, but that doesn’t mean your feet don’t look fabulous when you leave. Alejandra finished my treatment by giving my nails one last buff and shine that made them gleam more than any high-shine top coat ever has. 

## **The Verdict**
Nearly two months later, all traces of athlete’s foot are gone, my cuticles remain crisp, and my toenails are still shining bright like diamonds. Correa says she sees clients with painful ingrown nails and corns every four to six weeks — but every three months will do for most. I, for one, can’t wait to go back.

“To maintain an independent and pain-free lifestyle, our feet must be in tip-top shape,” Correa insists. What may seem like a mild concern (think: discoloration or peeling) can worsen into time-consuming conditions that were easily preventable. “Things like ingrown nails, thick nails, and cracked heels can cause pain that eventually affects every aspect of your life,” she explains. “Unbeknownst to many, most back problems start with the way we walk. You can think about that next time you shift your weight to avoid that painful nail that's been bothering you.” 

Her main advice? Wear comfortable shoes and don’t neglect your feet. “Caring for your feet is as vital as caring for your teeth, skin, or any other part of your body,” she says. Noted.

Summer is upon us, which means you’re likely showing a bit more skin. Pants become shorts and maxidresses and skirts become minis, as our gams finally get their moment in the sun. But, for those who are dealing with a dotted, bumpy, or spotty appearance on their legs, the short hemlines may be unwelcome. To better understand how to treat and prevent the skin conditions that are often deemed ‘strawberry legs,’ we spoke with a board certified dermatologist.

## **What Are ‘Strawberry Legs’?**
“Strawberry legs are the non-specific reference to a visual dotty appearance to the legs, usually around the hair follicles,” says [Nikhil Dhingra, MD](https://springstderm.com/physicians/nikhil-dhingra/), a board certified dermatologist at Spring Street Dermatology in New York City. “[The dots] represent trapping of a variety of common skin elements in and around the hair and oil glands.” As he explains it, the trappings can be made of up of: 

- Keratin (the skin’s main protein)
- Melanin (the source of pigment)
- Sebum (the skin’s natural oil)
- Bacteria (often, natural flora of our skin)

‘Strawberry legs’ is the colloquial term for the dark pores and dots or red bumps that appear primarily on the lower legs because they resemble the seeds of a strawberry. While strawberry legs are not medically harmful, they can be an aesthetic concern. 

## **How to Treat the Causes of Strawberry Legs**
While the appearance of strawberry legs is often the same regardless of what triggered it, there are a number of different causes. Knowing the culprit behind your dotted gams will ensure you are treating the right condition. Below are four of the most common causes of strawberry legs and the best way to treat them. 

### **1. Clogged Pores**
Just like you can get clogged pores on your face, you can also have clogged pores on your legs. Genetics and thicker body hair can cause some people to have larger pores, and, while the pores alone aren’t necessarily troublesome, they can become problematic when they get clogged with bacteria, dead skin, and sebum. [Like a blackhead on the face](https://aedit.com/aedition/how-to-treat-hormonal-acne-body-acne-cystic-acne-whiteheads-blackheads), when clogged pores on the legs are then exposed to air, the debris dries out and darkens. 

**Treatment Option**: Chemical & Physical Exfoliation 

To treat clogged pores on the legs (or anywhere on the body), borrow a step from your facial skincare routine: exfoliation.“My personal preference for my patients is chemical exfoliation, which uses ingredients like acids and retinols to increase skin cell turnover and declog pores,” Dr. Dhingra says. “What this does is gently remove keratin, oils, and other skin debris, opening up the pores and follicles, which decreases the secondary buildup of bacteria. The bacteria that cause acne and folliculitis thrive in clogged hair follicles with oils, so eliminating that build-up is key.”

He recommends finding a body wash or moisturizer with alpha- and beta-hydroxy acids (specifically, [glycolic acid](https://aedit.com/aedition/guide-to-topical-alpha-hydroxy-acids-in-skincare) and [salicylic acid](https://aedit.com/aedition/guide-to-beta-hydroxy-acids-salicylic-acid-in-skincare)) to remove the dead skin cells and other debris that cause clogged pores. “These acids exfoliate in a sufficiently gentle manner by decreasing the ‘stickiness’ of the dead or dying skin cells,” he explains. “This opens up the pores and also brings in a nice textural change and luminosity to the skin.”

### **2. Folliculitis**
Folliculitis is a common skin condition in which the hair follicles become inflamed or infected. It usually manifests as small red bumps around the hair follicles. In severe cases, people can experience hair loss in the infected area or scarring. But most cases are mild and usually clear up in a few days.

“One thing to keep in mind is that folliculitis is a broad umbrella term that refers to inflammation of the hair follicle,” Dr. Dhingra says. “This can be infectious from bacteria like staphylococcus (staph) or sterile from oil build up or trauma from shaving.” 

**Treatment Option**: Oral & Topical Medication

Folliculitis can be a “tricky condition” to treat, Dr. Dhingra says, which is why he recommends consulting with a board certified dermatologist before attempting any at-home remedies. “Folliculitis can be sterile, where red and pus-filled bumps appear secondary to things like clogged pores and shaving,” he says. “But they can also be a sign of a superficial skin infection with bacteria or yeast like staph and pityrosporum.”

While the former can be easily addressed with over-the-counter products, the latter may require prescriptions like antibiotic lotions or even pills to clear up. “A dermatologist can take a simple swab to identify if there are any organisms to target,” Dr. Dhingra says. “And, if there are, it can save you a lot of hassle and headache of trying to figure out at-home routines that work.”

**Treatment Option**: Antibacterial Skincare

When it comes to at-home treatments, they range from lifestyle changes to skincare. For starters, you’ll want to change out of your sweaty gym clothes as soon as possible and shower soon after. Once in the tub, Dr. Dhingra recommends using an antibacterial soap, like [Hibiclens](https://www.walgreens.com/store/c/hibiclens-antiseptic/antimicrobial-skin-cleanser/ID=prod6186406-product) or [PanOxyl](https://www.target.com/p/panoxyl-acne-foaming-wash-with-10-benzoyl-peroxide-5-5oz/-/A-14984165), to lather up.

**Treatment Option**: Shave Smarter

Additionally, you may want to give your shaving routine a fresh look. In addition to swapping a fancy, multi-blade razor for a disposable one, Dr. Dhingra suggests using a soothing shave cream, like the [Aveeno Therapeutic Shave Gel](https://www.target.com/p/aveeno-therapeutic-shave-gel-with-oat-for-sensitive-skin-7oz/-/A-13404207). “The tradeoff for less bumpy skin here is giving up the closest shave,” he shares. “In my mind, close shaves and folliculitis risk are inversely related.” He also says that you should shave with (rather than against) the grain every two to three shaves. “If push comes to shove and you’re prone to razor bumps, [laser hair removal](https://aedit.com/procedure/laser-hair-removal) with a trained professional will save you a lot of headache in the long run,” he adds.

### **3. Keratosis Pilaris**
Also known as ‘chicken skin,’ keratosis pilaris (KP) is a skin condition that causes tiny bumps to populate on the skin. “Keratosis pilaris is most commonly found on the upper outer arms but can affect the thighs at times,” Dr. Dhingra explains. “This represents a build-up of keratin in the hair follicles.” 

**Treatment Option**: Chemical Exfoliation 

Like clogged pores, chemical exfoliants are often an effective way of treating keratosis pilaris, but prescription-strength skincare may help as well. “Exfoliative acids are my starting remedies for keratosis pilaris,” Dr. Dhingra says. “If those don’t work, a [prescription-grade retinoid](https://aedit.com/aedition/guide-to-vitamin-a-retinol-retinoids-in-skincare) would be reasonable, but that should be used in conjunction with a certified dermatologist.”

While keratosis pilaris is often a year-round condition, it is typical to experience flare ups in the winter months when the skin is drier. Additionally, swimmers may experience a worsening of the condition due to chlorine and other pool chemicals that dehydrate the skin.

### **4. Dry Skin**
As formerly mentioned, dry skin contributes to a number of skin conditions — and strawberry legs are no exceptions. Dehydrated skin makes you more prone to irritation, especially when shaving. People with dry skin on their lower legs are more likely to experience razor burn, keratosis pilaris, folliculitis, and clogged pores, all of which can lead to a dotted appearance on the legs.

**Treatment Option**: Creams & Moisturizers 

“[Amlactin Daily Moisturizing Body Lotion](https://amlactin.com/products/amlactin-daily) and [CeraVe SA Cream](https://www.cerave.com/skincare/moisturizers/sa-cream-for-rough-and-bumpy-skin) are great starting options for extreme dry skin, especially if there’s a scaly appearance,” Dr. Dhingra shares. Ichthyosis, the medical term for dry, scaly, or thickened skin, does not always respond to traditional creams and moisturizers, which is why a dual-purpose formulation may be required. “Simple moisturizing isn’t always sufficient because there’s a thicker than normal layer of dead skin on the surface,” he explains. “Exfoliative creams gently remove that layer, allowing for thicker moisturizers, like [Aquaphor](https://www.target.com/p/aquaphor-healing-ointment-for-dry-38-cracked-skin-1-75oz/-/A-13287607), to penetrate the healthier layers below.”

## **The Takeaway**
Strawberry legs may be an umbrella term for a number of skin conditions, but there are a number of things that can be done at-home and in-office to both treat and prevent a bumpy or dotted appearance. Veins. We all have them. Some are more prominent than others. As people age, those blue and purple squiggles often become more prominent. For adults with [leg vein disease](https://aedit.com/aedition/how-to-treat-and-prevent-varicose-veins-spider-veins) (a common condition that affects up to 50 percent of all adults) regular blood flow is interrupted, causing blood to pool in the veins. The result? Varicose and spider veins. One of the most common treatments to minimize or eliminate the appearance of such veins is sclerotherapy. But, is it worth it? We asked a plastic surgeon and a patient for an honest assessment. 

## **Sclerotherapy 101**
One of the more treatment options to to improve the appearance of spider veins and small varicose veins, [sclerotherapy](https://aedit.com/procedure/sclerotherapy) is a minimally invasive procedure that uses an injectable sclerosant to dissolve the veins. 

“Sclerotherapy can be effective for both spider veins and varicose veins, but it depends on the size of each,” says [Melissa Doft, MD](https://aedit.com/provider/melissa-doft-md-63), a double board board certified plastic and reconstructive surgeon in New York City. “During the procedure, a small needle is inserted into the vein and sclerosing medicine is injected into the vein which irritates the sides of the veins and causes them to collapse, cutting off the blood flow.” The collapsed vein will dissolve into the local tissue within a few weeks for small veins or a few months for larger veins. Occasionally, some patients will need multiple sclerotherapy sessions to treat the varicose or spider veins. 

Sclerotherapy treatment has become a popular choice among patients looking to improve the appearance of the varicose and spider veins because it is relatively quick and painless. The entire treatment takes under an hour. Some solutions contain a local anesthetic, though patients may have a bit of discomfort when the fine needle is inserted into the vein. While many patients will experience side effects like bruising, redness, and discoloration around the injection site, most people can resume normal activities immediately after the treatment — though they will have to wear compression garments and avoid excess exposure.

### **Ideal Candidate for Sclerotherapy** 
Sclerotherapy is not always the best or only solution to minimize the appearance of veins. “A spider vein has to be large enough to thread with a needle and the varicose vein needs to be small enough to collapse down,” Dr. Doft says. As she explains it, some spider veins are too small for sclerotherapy, so lasers are a better treatment choice. For patients who have large varicose veins, she recommends consulting with a vascular surgeon. “[They] will not only evaluate their venous flow, but can perform a surgery to remove the feeding vessel to large varicose veins or remove the veins individually,” Dr. Doft shares.

Medical history is also an important factor in determining whether or not sclerotherapy will be effective. If you have a heart condition, are prone to blood clots, or taking certain medications or blood thinners, sclerotherapy may not be the right treatment for you.

### **Sclerotherapy Cost & Results**
Each sclerotherapy treatment session costs around $350 and has a 50 to 80 percent success rate. For some patients, health insurance may cover the procedure. “Some varicose vein treatments may be medically necessary if you have symptoms from the veins,” Dr. Doft says. “Typical symptoms include pain, swollen legs at the end of the day, and the pain interfering with your daily activities.”

One thing to keep in mind is that while the treatment is permanent, new veins may appear and require additional treatment. Dr. Doft says many patients will come into her office every few years for maintenance.

## **So, Is Sclerotherapy Worth It?** 
“I think it is worth it for the right patient,” Dr. Dofts says. “Like all cosmetic procedures, it really depends on how much the issue bothers you.” In her personal experience, lasers can serve as an effective alternative depending on the size of the vein. “I had tons [of] small spider veins on my thighs from pregnancy and surgical residency which bothered me every time I wore a skirt or a bathing suit,” she shares. “I recently used the laser in my office to treat all of them, and I could not be happier. It was so easy and not very painful.”

Because sclerotherapy procedures can amount to a large investment over time, it is important to consult with a board certified dermatologist or plastic surgeon to ensure you are receiving the best treatment for your condition. For those looking to thwart the development or worsening of varicose and spider veins, Dr. Doft suggests compressions stockings as the first line of defense. 

One of the most common treatments to minimize the appearance of varicose and spider veins is sclerotherapy. But, is it worth it? A plastic surgeon and a patient offer their honest assessments.

While summer is typically the time to enjoy shorts and higher hemlines, not everyone welcomes the opportunity to show some leg. For those dealing with inflamed varicose veins and spider veins, the struggle of slipping into your beach best can be all too real. And it’s not just about veinity — ahem, vanity. The tracks of purple-blue squiggles dotting the gams can be painful, too.

“Leg vein disease impacting blood vessels is a very common condition that affects up to 50 percent of all adults,” says Washington Square Dermatology’s board certified cosmetic dermatologist [Samer Jaber, MD](https://aedit.com/provider/samer-jaber-md-3882). Varicose and spider veins form when regular blood flow is interrupted, causing blood to pool in the veins. While it isn’t always easy to control the onset of varicose or spider veins, there are plenty of ways to improve the condition — and it all starts with a proper diagnosis.

## **Varicose Veins vs. Spider Veins**
For starters, varicose veins and spider veins are not the same thing. “They are two very distinct conditions and need to be treated accordingly,” says NYC-based board certified dermatologist [Cheryl Karcher, MD](https://aedit.com/provider/cheryl-karcher-md-4323), who specializes in treating spider veins.

Perhaps the most obvious difference between the two is size. Known for their large diameter (three millimeters or greater) and raised appearance, varicose veins are swollen blood vessels that often twist and turn in a rope-like pattern. Spider veins, on the other hand, are smaller (usually around one millimeter) red, blue, or purple vessels. While both are visible through the surface of the skin, spider veins do not bulge like their varicose counterparts. 

Another distinction? The side effects that accompany them. 

Symptoms of varicose veins include leg cramping and/or heaviness that can often be painful, but spider veins are generally considered painless and don’t directly pose any health risks beyond cosmetic concerns.

While both most commonly appear on the legs, spider veins can also present on the face — and the list of potential causes for each runs deep.  “Although women are more likely to develop venous diseases than men, factors such as aging, family history, prolonged periods of standing or sitting, smoking, not getting enough exercise, hormonal changes during pregnancy or due to birth control, a history of blood clots, or injury to the veins can lead to either condition,” says Dr. Jaber. 

Once properly diagnosed, there are plenty of remedies — ranging from minimally invasive treatments to surgical procedures — that can bring back smooth, vein-free skin. 

## **How to Treat Varicose Veins and Spider Veins** 
Patients who begin to notice the appearance of veins and experience symptoms like leg aching or cramping, mild swelling, or itchiness may wish to consult with their doctor about diagnosis and treatment options (anyone who suddenly experiences severely inflamed veins, discoloration, or ulceration of the skin should immediately contact his or her provider). 

During the visit, your doctor will likely perform a Doppler ultrasound or venogram to get a better sense of the severity of the condition before deciding how to proceed. “There’s not much you can do to prevent spider veins or varicose veins,” says Dr. Karcher. “But your treatment options are both safe and easy.”
