As the saying goes, the eyes are the windows to the soul. And, sometimes, these windows can be overrun by heavy upper lids, baggy lower eyelids, or even low-hanging brows that impede the appearance of an awake, youthful-looking eye. With so many potential changes affecting the eye area, how is one to know if they need a [blepharoplasty](https://aedit.com/procedure/blepharoplasty-eyelid-surgery) (a.k.a. eyelid lift), a [brow lift](https://aedit.com/procedure/brow-lift-forehead-lift-browplasty), or [both](https://aedit.com/aedition/combining-blepharoplasty-and-brow-lift)? We tapped into four plastic surgeons for their insight on choosing the right surgery.

## **Age-Related Eye Changes**
The [aging eye](https://aedit.com/concern/aging-and-tired-eyes) can be difficult to navigate since there are various changes the area experiences. Like no two bodies are the same, neither are any two sets of eyes. But, for the most part, the relative transformation that most of us tend to notice to the eyes and the surrounding area include:

- **Saggy Skin**: The eyes boast perhaps the [thinnest skin](https://aedit.com/aedition/how-to-treat-the-skin-around-the-eyes-with-skincare-and-non-surgical-cosmetic-procedures) on the entire face, which is why they are often one of the first areas — if not the first — to show their age. Both the upper and lower eyelid skin can transform from firm and elastic to crepey, stretched out, and sagging.
- **Under-Eye Bags**: Not everyone experiences bulging bags, but, if they surface, it's usually from a weakening of the muscles and tendons coupled with displaced, protruding fat. 
- **Fat**: In some, there are signs of fat loss and a hollowing of the under-eye area; in others, fat becomes displaced and begins to protrude from the lower eyelids causing bags to form.
- **Fine Lines & Wrinkles**: With changes to the quality of the skin come finely etched in lines and wrinkles around the eyes (i.e. crow’s feet), underneath them, and on the upper eyelids. Sun exposure, lifestyle choices, and inherently thin skin are all to blame.

Plastic surgeons don't follow a one-size-fits-all method to correct the eyes. Since the cause of aging in the region is multifactorial, your surgeon must tailor the solutions to address the issues at hand. For instance, if someone has excess skin, the skin has to be dealt with, says [Steven Williams, MD](http://trivalleyplasticsurgery.com/), a board certified plastic and reconstructive surgeon in Dublin, CA, and the vice president of the American Society of Plastic Surgeons ([ASPS](https://www.plasticsurgery.org/)). On the other hand, if there is fat atrophy, it should not be glossed over. “Without addressing each of these issues specifically, you will get incomplete results or unsatisfactory results,” he shares. 

Although a brow lift and blepharoplasty both tackle age-related concerns on the upper third of the face, they address different facial features. As Dr. Williams explains, blepharoplasty helps correct loose lid skin on the upper and/or lower lid and periorbital fat. A [brow lift](https://aedit.com/aedition/guide-to-surgical-and-non-invasive-brow-lift-procedures), meanwhile, repositions the eyebrow and the skin of the forehead. “The brow is influenced by the frontalis muscle, a very thick, strong muscle, while much weaker muscles influence eyelids,” he notes.

## **Blepharoplasty, Brow Lift, or Both?**
Now that you know how age and gravity affect the eye area, it’s time to talk about how to [rejuvenate the region](https://aedit.com/aedition/cosmetic-procedures-for-under-eye-rejuvenation). As we’ve discussed, blepharoplasty can address skin and fat concerns on the upper and lower eyelids, while a brow lift can reposition the brow (which may have an effect on the upper lid) and target forehead skin laxity. These surgeries can be performed alone or in tandem depending on the patient and their aesthetic goals. Here’s what you need to know:

### **CHOOSE A BROW LIFT FOR EYEBROWS THAT APPEAR HEAVY**
Dr. Williams says people often see him for a blepharoplasty consultation only to find out they need a brow lift. “Patients that feel they need an eye lift may have heavy brows as well,” he explains. “Many times, Botox® can contribute to masking brow ptosis issues, which can contribute to the overall undesired look.”

A brow lift is likely redundant skin on the upper eyelids is an issue (unless the brows have fallen considerably). In comparison, a brow lift focuses on raising the brow area strictly when the drooping stems from changes to the brows themselves and the area right above them. A brow lift also corrects any feeling of heaviness, says [Robert Schwarcz, MD](https://www.naturalfacedr.com/contents/about-the-practice), a board certified reconstructive and oculoplastic surgeon in New York City.

Like so many other facial features, the brows can also ‘fall’ with time. Whenever a surgeon assesses a patient and checks their brow position, Dr. Schwarcz says the brows should ideally sit at the brow bone – not below it or encroaching on the eyelid space. When the brows sit lower on the face and closer to the upper eyelid than they should, they can block off a decent amount of real estate. 

Excess skin is just one of many reasons why it is crucial to evaluate the brow and upper eyelid together. “If the lateral brow is sagging, it contributes to sagging skin and fullness in the upper eyelid,” explains [Bob Basu, MD](https://aedit.com/provider/bob-basu-34656), a board certified plastic and reconstructive surgeon in Houston, TX. “A brow lift helps correct brow sagging and hooding to open up the entire eye.” If there is little space between the eyelid and the brow, a brow lift can open the eye area back up – likely without the need to remove as much upper eyelid skin.

A brow lift can be personalized for the patient. There are four different types of brow lifts that involve varying degrees of invasiveness (and scarring), and each one corrects a unique situation.

1. **[Coronal Brow Lift](https://aedit.com/procedure/coronal-brow-lift)**: This requires an incision from ear to ear (within the hairline) over the crown to remove excess skin and create more rejuvenated and lifted brows.
2. **[Endoscopic Brow Lift](https://aedit.com/procedure/endoscopic-brow-lift)**: Perhaps the most favored technique, this version is the least invasive surgical option and requires only a few small incisions.
3. **[Pretrichial Brow Lift](https://aedit.com/procedure/pretrichial-brow-lift)**: The hairline or trichophytic incision removes a small amount of skin as a way of elevating the eyebrows and is best suited for those with extremely high hairlines.
4. **[Direct Brow Lift](https://aedit.com/procedure/direct-brow-lift)**: Making an incision directly over the eyebrows can lower a high hairline, however, this technique does not always address the entire brow but rather only a portion of it.

To artfully perform a good brow lift, incisions are made so that the surgeon can go in either endoscopically or more traditionally to lift the muscles and raise the outer third of the brows. As a result, the eyebrows appear better positioned and regain a more natural shape, plus the forehead will appear smoother. By improving the positioning, the brows look less heavy and the eyes look more awake. 

But there is still a balancing act at play to achieve a natural result. “I always look at old photos to see if a patient always had low brows or did it change with aging,” Dr. Schwarcz says. “Not to complicate things, but in a small subset of the population – usually seen in very thin face patients – the brows can go up with age.” Furthermore, it's imperative not to overcorrect the brows when lifting them because that can make a person appear surprised. “Also, it’s important to be aware if someone has a hollow upper eyelid region, as this can be more exposed with a brow lift,” Dr. Schwarcz adds.

Preventatively, neuromodulators (i.e. the so-called [Botox brow lift](https://aedit.com/procedure/chemical-brow-lift)) can help maintain the arches. “[Recent data](https://www.ncbi.nlm.nih.gov/books/NBK574523/) proves that the frontalis muscle has a bi-directional pull on the skin of the face – both up and down,” Dr. Williams shares. “Botox may delay the need for a brow lift when used to treat that downward pull.” Even post-brow lift surgery, surgeons still may recommend neurotoxin injections to correct any wrinkles that crop up around the eyebrows, like between them (known as the glabellar lines or elevens) or right above them.

### **OPT FOR AN UPPER BLEPHAROPLASTY IF THE LIDS ARE HOODED**
A tired-looking eye can result from upper [eyelid hooding and loose skin](https://aedit.com/concern/sagging-and-hooded-eyelids) (independent of the brow). In some cases, a low-hanging lid can impede the eyes' ability to function correctly. The excess skin makes it hard to apply makeup and eye products and can even affect one's vision. In the case of the latter, insurance may [cover the cost](https://aedit.com/procedure/blepharoplasty-eyelid-surgery/cost) of treatment.

Regardless of the cause, most plastic surgeons perform an [upper eyelid blepharoplasty](https://aedit.com/procedure/upper-eyelid-blepharoplasty) in one of two ways: either as a skin pinch or as a fat and skin removal procedure. Dr. Williams says that the techniques used for blepharoplasty depend on what needs to be corrected. “For example, if excess fat is contributing to the tired or heavy look, then ignoring the fat will lead to an incomplete result,” he says. Most patients need to address a combination of thick muscle, skin excess, and fat.

A small incision is made in the crease of the upper eyelid or in the lash line (we promise it sounds scarier than it is), which provides access to excise the skin and remove or reposition fat. Depending on the patient, the fat can be adjusted instead of removed to allow it to lay better without losing volume.

### **GO WITH A LOWER BLEPHAROPLASTY WHEN STUBBORN BAGS PERSIST**
Is concealer not enough to send those under-eye bags packing? Puffy and protruding bags, which usually come with crepey skin, often require surgical intervention in the form of a [lower eyelid blepharoplasty](https://aedit.com/procedure/lower-eyelid-blepharoplasty-subciliary-approach). “In the earlier days of the blepharoplasty operation, over obsession with the excess fat was a concern,” Dr. Williams recalls. “Modern-day techniques include a combination of fat removal and also fat repositioning to avoid the hollow look that most patients want to avoid.” Although plastic surgeons can remove fat in several ways, most practice is through an [incision in the eyelid](https://aedit.com/procedure/lower-eyelid-blepharoplasty-transconjunctival-approach) to avoid an external scar.

Dr. Basu explains that a lower blepharoplasty can remove loose skin and fat bags and improve the overall appearance of the lower eyelids. “Often, fat repositioning or fat micrografting also improves a tear-trough deformity – the hollowing in the lower eye area that contributes to an aged or tired look,” he says. “I frequently complement my surgical results with [fat micrografting](https://aedit.com/procedure/facial-fat-grafting) to achieve a more refreshed, natural-looking, rejuvenated result.” This way, there's a smooth transition between the lower eyelids and where they meet the upper cheeks.
 
It should be noted that a lower eye lift cannot fully eradicate every minor imperfection. When skin texture concerns, such as lines and wrinkles, are a patient's priority to get rid of, a resurfacing laser is often used instead.

### **DO A COMBINATION FOR HOLISTIC REJUVENATION**
For some, both the upper and lower lids and the eyebrows require a refresh to create a holistic and harmonious result. “In moderate to severe cases, we can achieve the best results with both a brow lift and an upper eyelid lift,” Dr. Basu shares. It's essential to visit a board certified plastic surgeon to learn about your options and determine your needs.

Of course, when pairing procedures, your surgeon can do any combination. Still, the most popular combos are usually a brow lift and upper blepharoplasty and, perhaps, correcting the lower lids as necessary. The reason is that loose skin often accompanies upper lid hooding and a saggy brow, so addressing all the changes yields the most positive results. 

When faced with more than one eye-related issue, most patients opt to go in for [everything at once](https://aedit.com/aedition/when-is-it-safe-to-combine-plastic-surgery-procedures) because it means a reduced cost (compared to two separate appointments), one recovery period, and maximized results. When done individually, Dr. Schwarcz says it’s usually because the patient doesn't want too much surgery at once or because “they want more accurate results to see how much skin is left after the brow lift heals.” In this staggered approach, the brow surgery comes first with the blepharoplasty performed later. “This is less common because there are two healing times and extra operating room expenses,” he notes.

## **The Takeaway**

The square-inch of [skin under the eyes](https://aedit.com/aedition/how-to-treat-the-skin-around-the-eyes-with-skincare-and-non-surgical-cosmetic-procedures) may be tiny, but it packs a punch. Sagging, darkness, wrinkles — there are a host of concerns that can show up in the under eyes, many of which worsen over time. “The skin located right beneath the eye is thinner than the rest of the skin on your face,” says [Mariana Vergara, NP-C](https://www.instagram.com/marianalvergara/), an aesthetic nurse practitioner in Beverly Hills. “As we age, there is a loss of elasticity and collagen and the skin becomes even thinner.” Another issue? How much we use our eyes. “This area is constantly in motion, causing fine lines to appear,” she notes.

Due to the nature of the under eye area, it requires an approach different from the rest of your facial treatment routine. “The eyes are one of the features we are naturally drawn to when we look at someone’s face,” says [Charles Pierce, MD](https://www.normanrowemd.com/our-practice/dr-charles-pierce/), a double board certified plastic and reconstructive surgeon at Rowe Plastic Surgery. But, as he points out, under eye concerns aren’t a one-size-fits-all problem. “Each patient needs a proper evaluation of what will work for their needs,” Dr. Pierce says. “Under eye treatments are tailored to what the major problem is but almost always are approached with multiple modalities.”

So, what does the combination therapy entail? We asked the experts.

## **In-Office Under Eye Rejuvenation Treatments** 
When skincare (think: [eye cream](https://aedit.com/aedition/eye-creams-serums-to-treat-crows-feet-dark-circles-puffiness)) alone is no longer doing the trick, patients often turn to invasive and non-invasive procedures to refresh the under eye area — and providers say that a multi-pronged approach can create the most youthful, energetic-looking appearance. Below are some of the most popular non-surgical [under eye rejuvenation procedures](https://aedit.com/procedure/eye-rejuvenation-solutions):

### **1. Botox®**
The providers we consulted all suggest patients address the look of the skin and the quality of the skin separately. First and foremost, Dr. Pierce’s go-to treatment for the under eyes includes [Botox®](https://aedit.com/procedure/periorbital-botox), which is “great to soften the exaggerated muscle or animation response.” From there, he adds on a [chemical peel](https://aedit.com/procedure/superficial-chemical-peels), [microneedling](https://aedit.com/procedure/microneedling), or [laser](https://aedit.com/procedure/nonablative-laser-skin-resurfacing) session to tighten the skin and lighten pigmentation (a low key cause of dark circles).

When age has taken its toll, a full eight hours of sleep isn’t enough, and/or you find your vision slightly strained, it may be time for surgical intervention. Regardless of the type, Dr. Hilger notes that blepharoplasty techniques have evolved over time to deliver more natural results. “In years past, upper and lower blepharoplasty focused on removing — by surgical excision — redundant skin that created hooding and an aged face,” he explains, adding that this often led to a gaunt or hollowed appearance. Today's approach is more comprehensive and “includes tightening the supportive structures of the eyelids, repositioning some of the lax skin to preserve youthful contours, excising excessive skin that can block vision, or repositioning bulging fat pads,” he shares. 

These below-the-brow procedures can, he says, “provide a smooth contour that starts at the lash line and continues across the eyelid-cheek junction.” Another benefit? The surgery also has the ability to “resurface the eyelid to reduce fine wrinkles that can not be corrected by skin excision,” Dr. Hilger notes. 

### **Brow Lift**
As its name suggests, a [brow lift](https://aedit.com/aedition/theres-more-to-a-brow-lift-than-meets-the-eye) lifts the brow. The primary impetus to undergo a brow lift is to correct the loss of elasticity that makes the forehead skin drape poorly. Done via small incisions hidden under the hairline, modern versions of the surgery create a natural lift as opposed to the ‘surprised’ appearance of brow lifts past. “A brow lift repositions the brow higher, which makes the eye aesthetic zone look larger, reduces wrinkles as the skin is tightened, and reduces some [eyelid hooding](https://aedit.com/concern/sagging-and-hooded-eyelids),” Dr. Hilger explains. It should be noted that the so-called ‘[chemical brow lift](https://aedit.com/procedure/chemical-brow-lift)’ performed with neurotoxins (read: Botox®) can provide a temporary (think: three to four month) lift, but surgery is the best bet for those looking for long-term correction. 

## **Benefits of Combining Blepharoplasty + Brow Lift**
As we’ve covered, maintaining [facial harmony](https://aedit.com/aedition/facial-harmony-in-aesthetic-medicine) is the main objective of any facial aesthetic treatment, and sometimes that means combining procedures for best results. - **It Looks More Natural**: If both the brows and lids are heading south, correcting them at once may offer the most comprehensive result. “Addressing changes that span aesthetic zones provide a more natural result, as the transition from one aesthetic area to another is blended,” Dr. Hilger says.
- **It Cuts Down on Maintenance**: “The results are remarkably lasting and additional rejuvenation is rare for a decade,” Dr. Hilger shares. With more emphasis than ever on the expressiveness of our upper face — chiefly, the eyes — we figured it was time to examine a relatively common complaint that is not often discussed: eyelid ptosis (a.k.a. droopy eyelids). Here’s what you need to know about the condition and treatment options.

## **What Is Eyelid Ptosis?**
Eyelid ptosis (the ‘p’ is silent, friends), also known as blepharoptosis, is the medical term for a drooping eyelid. The condition can be caused by trauma, age, genetics, or various medical conditions, and there are two general classifications for ptosis:

1. Congenital ptosis (present at birth)
2. Acquired ptosis (developed later in life)  

Ptosis affects both men and women, and it may be present in one eyelid (unilateral) or both eyelids (bilateral). It’s estimated that somewhere between 10 and 20 percent of adults have some level of eyelid ptosis. While mild degrees of blepharoptosis may only have a cosmetic impact, more severe cases can be medically significant.

## **What Causes Eyelid Ptosis?**
We tend to take blinking for granted, but it’s actually a pretty complex marvel of physiology. Just so we’re seeing eye to eye (right?), let’s breeze through some orbital anatomy. The eyelids are composed of several muscles that are responsible for opening and closing the eye. The orbicularis oculi is the muscle that encircles the eye socket and controls the closing of the eyelids.

There are also two main muscles involved in raising and maintaining the position of the eyelids. The levator palpebrae superioris does the heavy lifting and the superior tarsal muscle provides an extra boost. Damage to either of these two muscles (or to the nerves that signal them) can lead to eyelid sagging.  

With that said, there are a host of factors that may contribute to blepharoptosis. In some cases, eyelid droop is simply the result of heavy, excess skin in the area (not impaired muscle function). Other times, it’s caused by conditions that can damage the delicate eyelid muscles:

- Genetics (researchers have found at least one gene so far)
- Age (muscles wear out as we get older)
- Eyelid tumors or cysts
- Eye injury
- Muscle disorders
- Nerve issues (including stroke and Horner’s syndrome)
- Cataract surgery (an estimated 4 to 12 percent of patients experience post-op ptosis)

And then there are the risk factors that are more lifestyle dependent. Turns out,  your mother was onto something when she told you to stop flipping up your eyelids (gross) because ‘someday they’ll stay like that.’ Eyelid ptosis is sometimes the result of:

- Contact lens use (primarily hard contacts)
- Eye rubbing
- Elective eye surgery (including LASIK)
- Neurotoxin injections (Botox®, Dysport®, Jeuveau®, Xeomin®)

### **Can Botox Cause Ptosis?**  
While the so-called ‘[Botox Brow Lift](https://aedit.com/procedure/chemical-brow-lift)’ has been making headlines recently, one of the most common side effects of neuromodulators is brow and eyelid ptosis. Visiting a highly experienced injector decreases your chances of this complication, but it's always a possibility. “Neuromodulators may cause temporary ptosis if they come in contact with the levator superioris palpebrae muscle,” says [Kian Karimi, MD](https://aedit.com/provider/kian-karimi-md-17458), a double board certified facial plastic and reconstructive surgeon in Los Angeles. “Some patients also complain of brow ptosis when the frontalis muscle (forehead) is treated with these injections.”

The good news? Neuromodulator-related ptosis generally wears off faster than the rest of your treatment. It usually resolves in three to four weeks.

## **How to Treat Eyelid Ptosis**
Sometimes ptosis is a symptom of something innocuous, like aging (*sigh*). But because blepharoptosis can be indicative of a serious condition like a stroke or a tumor, it’s smart to see your board certified ophthalmologist or plastic surgeon when you notice sagging. If left untreated, ptosis can cause:

- Vision disruption
- Amblyopia (a.k.a. lazy eye)
- Astigmatism 
- Neck problems (tilting the head to see causes tension)

Fortunately, eyelid ptosis is treatable, and your course of treatment will depend on the cause of your condition. In some cases, treating an underlying medical condition will resolve the ptosis. Otherwise, the most common eyelid ptosis treatments include:

- **Eyelid Exercises**: Dr. Karimi says that while special exercises may work in certain cases, results may not last.
- **Eye Crutch**: Also called a ‘ptosis crutch,’ this device is useful for treating temporary ptosis or for patients who aren’t candidates for surgery.
- **Surgery**: The most permanent solution, though it may not be appropriate in every case.

It should be noted that serious cases of blepharoptosis may qualify for [insurance coverage](https://aedit.com/aedition/insurance-coverage-for-plastic-surgery-cosmetic-procedures). “There are measurements that are taken to assess the degree of eyelid ptosis and tests to show if there is impairment of vision from eyelid ptosis and/or from redundant skin that can diminish peripheral vision,” Dr. Karimi explains. Once you’ve been diagnosed with eyelid ptosis, your provider will evaluate whether or not your case qualifies.

## **Eyelid Ptosis Surgery** 
Patients who have acquired ptosis unrelated to an underlying (and treatable) medical condition or those born with a droopy eyelid may be good candidates for surgical blepharoptosis repair. Eyelid ptosis surgery is typically performed as an outpatient procedure under local anesthesia. 

Depending on the severity of sagging and the provider’s preference, incisions may be made on the surface or the underside of the upper eyelid. “In general, milder cases can be treated with an [internal approach](https://aedit.com/procedure/eye-ptosis-surgery-internal-approach), whereas more significant or complex cases may require an [external approach](https://aedit.com/procedure/eye-ptosis-surgery-external-approach) with an outer incision,” Dr. Karimi says. The surgeon shortens (or reattaches, if necessary) the levator palpebrae superioris or the superior tarsal muscle based on the anatomy and needs of the patient.  

It’s common to incorporate eyelid ptosis repair into a larger set of facial rejuvenation procedures, particularly in older patients who’ve developed the condition with age. [Brow lift](https://aedit.com/aedition/guide-to-surgical-and-non-invasive-brow-lift-procedures) and [upper eye lift](https://aedit.com/procedure/upper-eyelid-blepharoplasty) (read: blepharoplasty) surgeries can often make the eyelids appear more elevated, but the simultaneous repair of true ptosis will yield the best aesthetic results.

Although every case is unique, Dr. Karimi says ptosis surgery typically has a one- to two-week recovery period, which is usually tolerated quite well. Eye drops and wound care are part of the recovery process, and [strenuous activity](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures) is usually limited for two to three weeks. Risks include poor [scarring](https://aedit.com/aedition/minimizing-scars-after-surgery), asymmetry of the lids after correction, infection, and need for additional procedures.

## **A Prescription Ptosis Solution**
For patients seeking a non-invasive alternative, the United States Food and Drug Administration (FDA) approved [Upneeq®](https://www.upneeq.com/) — a once daily eye drop — for the treatment of acquired eyelid ptosis in July 2020. The solution, which includes oxymetazoline hydrochloride (the active ingredient in nasal decongestant spray), causes contraction of the levator muscle, raising the eyelid by about one to two millimeters. While he has yet to prescribe it, Dr. Karimi sees Upneeq® as a non-surgical treatment option, particularly for patients experiencing temporary ptosis from neuromodulators.

## **The Takeaway**
Eyelid ptosis is not always just an aesthetic concern, so it’s important to see your doctor if you notice eyelid drooping to rule out underlying medical conditions. Depending on the cause(s) and type of ptosis, your provider will advise on what treatment option(s) that are best for you.

Thanks to models and influencers like Kendall Jenner, Bella Hadid, and Emily Ratajkowski, the ‘fox eye’ trend is gaining popularity on social media and beyond, with people seeking to transform the shape of their peepers with everything from makeup to surgery. 

Surgical options to achieve so-called ‘designer eyes’ include [blepharoplasty](https://aedit.com/aedition/what-its-really-like-to-get-eyelid-surgery-blepharoplasty) (a.k.a. eyelid surgery), canthopexy, and canthoplasty — the latter two tighten the lower eyelid and lift the outer corner of the eyes. All three methods have long been used to treat and correct signs of aging, but younger patients are now turning to the procedures in a quest for a more pronounced almond eye shape. 

To better understand the effects of canthopexy and canthoplasty specifically, we spoke to a board certified oculoplastic surgeon about the procedures and a trio of patients about their experiences and results. 

## **Canthopexy vs. Canthoplasty**
Both canthopexy and canthoplasty are surgical procedures that address the outer corner of the eye (a.k.a. the canthus) where the upper and lower eyelids meet. They can be performed for both therapeutic and cosmetic reasons, but they each have their own unique characteristics: 

- **[Canthopexy](https://aedit.com/procedure/lower-eyelid-malposition-lateral-canthal-suspension-canthopexy)**: Sutures are used to stabilize and reinforce the lateral canthal tendon without altering the horizontal length of the eyelid or position of the canthus. Instead, it improves tone and helps relieve the look of sunken eyes.
- **[Canthoplasty](https://aedit.com/procedure/lower-eyelid-malposition-lateral-tarsal-strip-canthoplasty)**: More invasive than a canthopexy, the surgery has the ability to alter the shape of the eye by correcting the canthal tilts that lead to sagging or drooping eyelids.

While canthoplasty is often used to refine or correct a previous eye surgery, canthopexy is often performed in combination with other procedures. “Canthopexy is a procedure where I tighten the lateral canthal tendon to give support to the lower eyelid,” says [Jessica Lattman, MD](https://aedit.com/provider/jessica-lattman-md-409), a board certified oculoplastic surgeon and clinical assistant professor of ophthalmology at NYU School of Medicine. “I often do it in conjunction with a lower eyelid blepharoplasty.” 

As she explains it, the canthopexy helps to further refine the result of the blepharoplasty. “I do the canthopexy to preserve the shape of the lower lid, not change it,” she shares. “Sometimes if the lower lid is not supported after a lower lid blepharoplasty, it can get pulled down. Doing a canthopexy keeps the eyelid in place beautifully.” 

### **Canthopexy & Canthoplasty Recovery**
Because the procedures have differing levels of invasiveness, the downtown and recovery associated with them is different, too. Canthopexy is an outpatient procedure, and the recovery for a canthopexy alone is quick, Dr. Lattman says. “There are a few sutures in the outer corner of the eyelid, and those come out in about a week,” she explains. “Pain is very minimal, and bruising is usually minimal as well.” Needless to say, coupling a canthopexy with an additional procedure will impact the recovery timeline.

A canthoplasty, meanwhile, involves a bit more downtime and recovery. Most patients will experience bruising and swelling for seven to 10 days, around which time most normal activities can be resumed. Anything that causes eye strain should be avoided for the first few days after surgery, while contact lenses are typically discouraged for a few weeks post-op. Full healing can take a month or more.

### **Canthopexy & Canthoplasty Results**
Canthopexy results are subtler than canthoplasty, though both have the ability to tighten the corner of the eye. After the bruising and swelling subsides in seven to 10 days, canthoplasty patients can generally expect to achieve a more almond-shaped eye. While the procedures can be performed as part of a more extensive rejuvenation surgery, they are now being sought by younger patients who are looking to replicate the elongated eye shape that is en vogue on social media. 

### **Canthopexy & Canthoplasty Risks**
As with all surgery, there is a level of risk involved. In the case of canthopexy and canthoplasty, the main concern is asymmetry. As such, Dr. Lattman and other surgeons often favor using these procedures for reconstructive purposes — as opposed to cosmetic rejuvenation. In younger patients, there may also be a need for revision surgery down the line as the skin loses elasticity with age. Finding a provider who is intimately familiar with the orbital area, like an oculoplastic surgeon, is of the utmost importance.

My plastic surgeon and I had a slow courtship. About 10 years ago, I started to feel like my eyelids were sagging. ### **Building A Rapport**

She understood that I wasn’t trying to look younger or “good for my husband,” as one (male) doctor suggested to me when I visited him for a consult (next!).

About four years ago, I told her I thought we’d exhausted the non-invasive strategies and it was time to talk about surgical procedures. She agreed and began outlining what that would entail. She told me that we would only operate on my upper eyelids and that it would be a mistake to do the lower ones, too, because the shape of my eye would start to change.

### **Sense and Sensibility**

I pushed back, but she held fast, and I now realize she was right. She told me she’d rather err on the side of doing less because we could always do another type of procedure later. But, if we did too much, too soon, there was no going back. I appreciated that sensibility. She also shared insight into how I could handle talking about the surgery with my children and how recovery would impact my ability to get back to work. She really got it.

I did talk to other doctors — including the one who said “your husband will be so happy with the way you look” (ugh) — but no one compared to her practical approach. And, having known her for years, I really did trust her with my face. So, she performed my eyelid lift, and it went very well.

### **Patience Pays Off**

I think my success story lies in the fact that I spent years getting to know my doctor and her staff. I trusted them before plastic surgery procedures were even a consideration for me. The clinic was able to produce an edited image to predict what the results of bleph surgery would look like on me, and to me, it made all the difference.”

**The AEDITION**: *How was your experience of the surgery itself?*

**Jackie**: My surgery went as planned, it was relatively quick, and I went home on the same day. I made sure to have my sunglasses with me, as recommended, so I was comfortable on my journey back. Once I got home I spent the rest of the day relaxing and sleeping, I wasn’t in any pain, but my eye area felt a little tight.

**The AEDITION**: *How long did it take you to recover?*

**Jackie**: In terms of looking normal enough to go out without sunglasses on, it took about 10 days, but it took just over three months until I got to see the final result. It was so exciting when I started to see the new shape of my eyelids beginning to form once the swelling had gone down. It takes quite a lot of patience, but I had faith in my surgeon and had been given realistic time frames of when to expect to reach each recovery milestone which was really helpful for me. 

**The AEDITION**: *Did your recovery match your expectations?*

**Jackie**: It did, but my face definitely looked worse than I had anticipated. I knew I’d experience swelling, but it’s difficult to picture what that will look like. I also had blue stitches that made the scars look worse until they were taken out. In terms of pain, I had no pain at all until day five, which is when I stopped taking my postoperative painkillers, so that was to be expected. By that time I was a bit bored from being in the house, but my eye movement became more restricted with the pain so I couldn’t really read or watch TV. I ended up watching Netflix on my phone because the smaller screen was tolerable, but it wasn’t ideal! Things got much better once the stitches were taken out and it felt like recovery was smoother moving forward.

**The AEDITION**: *Do you have any advice for patients considering the procedure themselves?*

**Jackie**: Prepare things to occupy yourself that don’t require you reading or using your eyes. I joined Audible and downloaded a bunch of audiobooks before my procedure, and they were excellent. I think it would be useful to have some form of voice-activated device like an Amazon Echo so that you could get things done without having to move around too much, especially in the first few days.

## **Li, Glendale, AZ**

Li, whose family is originally from China, decided to have double eyelid surgery a couple of years after she graduated college. It was not a decision she came to quickly and it was met with some apprehension from her family. “My family is very proud of their roots, and I think they saw my decision as me trying to erase that part of my identity. It took a lot of explaining on my behalf that I wanted the surgery for my own benefit, and not to fit into any social norms. I was deeply unhappy with the way my eyes looked, it was almost impossible for me to wear eyeliner as my friends did, which may seem trivial, but even compared to other members of my community I found my eyes to look a lot worse than most.”

**The AEDITION**: *How was your recovery?* 

**Li**: My recovery went well. Because the scars are in such a visible place it makes it difficult to go about daily tasks without having to wear sunglasses, but other than that it was all straightforward. There was swelling, but I managed that by icing regularly. My eyesight was back to normal within a week, and I returned to work after two weeks. It actually gave me a chance to have a good rest which I rarely give myself time for!

**The AEDITION**: *How did you manage your pain post-surgery?* 

Li**: I didn’t experience pain as such, but the swelling felt tight and a little sore if I tried to look straight ahead. I followed my surgeon’s instructions to the letter and made sure I took my medication on time, iced regularly, and slept with my head raised. I actually got more pain from my adjusted sleeping position hurting my back than from the surgery area.

**The AEDITION**: *Are you pleased with the results of the procedure? *

**Li**: I’m really delighted. I panicked for a few days after the procedure because my eyelids looked very asymmetrical, this was due to the swelling. It did take longer than I would have hoped for the final result to show, but I was glad I didn’t need any revision procedures, and the result looks naturally symmetrical.  

**The AEDITION**: If you could go back in time, would you still have had the procedure? *

**Li**: Totally, my family eventually understood why I wanted the procedure, and that was important to me. I love the results, they make me feel more confident, and I’ve been able to try all sorts of new makeup styles since the procedure that I couldn't have before. It’s a relatively small change, but it has made a massive difference to how I feel about my face.

## **Margaret, San Diego, CA**

**Margaret**: They were so supportive, even though my two sisters had also inherited the under eye-puffiness, they were not bothered by it, so I was worried they wouldn’t understand why I wanted to have the procedure. Once I’d explained how I felt and why I wanted to have surgery to fix the problem they completely understood and even picked me up from the clinic on the day of my surgery.

**Margaret**: It went well, I was pleased to have my family with me for the first few days, though I felt pretty tired. Aside from that, the swelling didn’t look good, but I wasn’t in pain, which I was worried about before the procedure. It was difficult finding things to do when I couldn’t really see that well over the first few days, but I got better really quickly, so it was fine. 

**The AEDITION**: *Do you have any advice for patients considering the procedure?*

**Margaret**: First of all, when the swelling makes your eyes look weird just remember how good that after photo will look in a few weeks time. And secondly, your eyes will probably look asymmetrical during the first week, don’t worry about it, the swelling is distributed differently for each eye, and it will settle with time.

**The AEDITION**: *How satisfied are you with the results? *

**Margaret**: I’m so pleased, it’s made a massive difference to how I feel about myself, and that was why I wanted to have the procedure, so I couldn’t have asked for anything more. The results did take some time, but I was told to expect that, and it was worth the wait. I was worried I’d look too different, but I still look like myself just with less pronounced under eye-circles.
