Male Breast Reduction

Gynecomastia occurs when an imbalance of estrogen and testosterone cause breast tissue to enlarge in men. This condition can affect men at many different times in life, especially during times that cause hormonal shifts. Gynecomastia can be observed in male babies caused by their mother’s estrogen (which fades within several months), during puberty, and again later in life between the ages of 50-69. Certain medications and drugs that have antiandrogen effects can also cause gynecomastia, with one of the most well known being anabolic steroids. Symptoms of gynecomastia typically resolve without intervention within a few months to 2 years, however if the condition does not improve, causes pain, or greatly effects self esteem, treatment may be necessary.

For patients with true gynecomastia, glandular excision is required to reduce the size of the breast. An incision is made at the bottom border of the nipple in the shape of a crescent. The tissue will be excised and the incision closed. Depending on the severity of the gynecomastia, the nipple may be removed entirely for reposition if the patient has severely sagging skin. Incision patterns vary depending on the specific conditions and surgical preference. 

When male patients have enlarged breasts caused by fatty deposits and not breast tissue, the condition is referred to as pseudogynecomastia. A liposuction procedure can be performed to reduce the size of the breasts. A small incision will be made either at the border of the nipple or within the underarm crease. A cannula will be inserted and moved back and forth to dislodge and suction fat deposits. For some individuals liposuction and tissue removal is necessary to address enlarged male breasts or puffy nipples.

Chest

- Stop taking blood thinning medications 2 weeks before a
- Refrain from all nicotine products 4 weeks before and after surgery
- No alcoholic beverages 1 week prior
- Stop using self tanner

Before Treatment

- Crescent incision made on the bottom border of the areola
- Breast tissue excised
- Liposuction procedure may be performed for improved contour
- Sutures to close

During Treatment

- Bruising and swelling
- Mild discomfort

Immediately After Treatment

- Return to work
- No sex for 7-10 days
- Stitches removed after 1-2 weeks
- Avoid stretching and heavy lifting
- Do not drive until able to comfortably wear seatbelt

1-2 Weeks After Treatment

- Jogging permitted after 2 weeks
- Resume normal exercise routine after 4-5 weeks
- Compression garments worn daily for up to 3 months

3 Weeks After Treatment and Beyond

- Permanent
- Immediate improvement
- Final results determined after 3-6 months
