3-Piece Inflatable Penile Implant

Penile implants are typically the last course of action available for patients who have exhausted all other options to treat erectile dysfunction (ED). ED not only affects the physical and sexual health of a patient, but the mental well-being too. Addressing complications associated with ED can greatly improve the quality of life for patients. Individuals who are undergoing gender confirmation surgeries can also benefit from the realistic results achieved by a penile implant. 

There are 2 different kinds of implant devices: semi-rigid and inflatable. Semi-rigid implants have the advantage of being simple to use and is a fairly simple procedure. Inflatable implants, while more surgically involved, offer patients an easier to conceal implant and more natural results than their non-inflatable counterpart. Inflatable implant devices are comprised or either 2 or 3 pieces. 2-piece implants insert rods within the penis shaft and a pump inside of the scrotum. Saline fluid is contained within the rods and becomes stiff when the pump is squeezed. 3-piece implants contain saline fluid within a separate reservoir that is implanted within the abdominal cavity. When the pump is squeezed, fluid infiltrates the rods and allows for an erection. While a 2-piece device is a less complicated surgery, a 3-piece device allows for the best concealment of the implant when flaccid. Each implant has its advantages and disadvantages. During surgery, the patient will be placed under general anesthesia. An incision will typically be made either at the base of the penis of the lower abdomen. Implant size will be determined by stretching the spongy tissue within the penis. The implant rods are customized for each patient to ensure best results. The inflatable rod implants are inserted within the corpora cavernosa inside of the penis shaft. A pump will be placed within the scrotum as well as fluid reservoir beneath the abdominal wall. Once securely in place, incisions will be closed. Penile implant surgery usually takes 45 minutes to an hour.

- Stop taking diet pills for 4 weeks before
- Stop smoking 4 weeks prior
- Do not take diabetic medication the morning of surgery
- No eating or drinking 8 hours before surgery

Before Treatment

- Incision made at the base of the penis or in the lower abdomen 
- Implant size determined and customized for each patient
- Implant cylinders placed within penis shaft
- Implant pump inserted within scrotum
- Fluid reservoir implanted beneath abdominal wall
- Incisions closed
- Surgery completed in 45-60 minutes

During Treatment

- Bed rest recommended for 2 days
- Walking and moderate activity permitted after 3 days
- Keep penis in upward position to prevent downward curvature

1-3 Days After Treatment

- Most patients able to return to work
- Non-dissolvable sutures removed
- Implant should be inflated twice daily to expand tissue around device

1-2 Weeks After Treatment

- Resume strenuous exercise as tolerated
- Resume sexual activity as tolerated
- Some patients may experience discomfort for up to 2-3 months

4-6 Weeks After Treatment and Beyond
