There are plenty of non-surgical procedures that get billed as facelift alternatives. And, while some have the ability to firm and smooth the skin sans scalpel, none have been able *remove* skin without surgery – until now. Since it received clearance from the United States Food and Drug Administration (FDA) last year, there has been a lot of buzz around [Cytrellis’s Ellacor™](https://www.cytrellis.com/technology/) and its micro-coring technology. 

Since we all know that trendy treatments have a tendency to come and go, you may be wondering if this one lives up to the hype. The short answer is that, in many ways, it does – though it’s not exactly a substitute for a [facelift](https://aedit.com/procedure/facelift-rhytidectomy). To break down everything you need to know about Ellacor™ and micro-coring, we tapped top dermatologists and plastic surgeons for their honest assessment. Here’s the scoop.

## **What Is Micro-Coring?**
Although some may be calling micro-coring the next best thing to a facelift, it's significantly different. For starters, it isn't surgery at all. Instead, micro-coring or fractional micro-coring is a new category of cosmetic treatment that involves the excisional removal of tiny columns of skin over a larger surface without cutting, thermal energy, or scarring.

Micro-injuries are made in the skin using a tiny, hollow needle while simultaneously removing small tissue areas to address mild laxity. “During a facelift, surgeons trim the skin from the sides,” says [Paul Jarrod Frank, MD](https://www.instagram.com/drpauljarrodfrank/?hl=en), a board certified celebrity cosmetic dermatologist in New York City. “With micro-coring, we inject local anesthesia and remove eight to 10 percent of the total skin surface.” As he explains, the idea of micro-coring originally came from the concept of skin tightening and wrinkle removal in studies showing [follicular unit extraction (FUE)](https://aedit.com/procedure/follicular-unit-extraction-fue), which involves removing individual hair follicles from the scalp before transplant. “When the tissue heals, the skin is much tighter,” he notes. 

To date, there’s only one micro-coring device on the market, Ellacor™, which has positioned itself as a minimally invasive facelift alternative thanks to its ability to remove skin without surgery. ## **Who Is an Ideal Candidate for Micro-Coring?**
Not everyone is an ideal candidate for micro-coring. “It's for someone who wants to take a more minimally invasive and natural-looking route to obtain tighter, younger-looking skin,” says [Dendy Engelman, MD](https://aedit.com/provider/dendy-engelman-md-312), a board certified dermatologist in New York City. This can appeal to people at [different stages of life](https://aedit.com/aedition/can-you-be-too-young-for-a-facelift) (and aging). “I sometimes recommend micro-coring to younger patients who are starting to notice changes in their skin and want to nip it in the bud,” she shares. And she also suggests it to “patients with more mature skin who do not want to undergo a full facelift and the [associated recovery](https://aedit.com/procedure/facelift-rhytidectomy/recovery).”

To see success with Ellacor™, the degree of [skin laxity](https://aedit.com/concern/facial-skin-laxity) – which should be on the mild to moderate side – is imperative. “Someone with excessive hanging skin is going to do better with a more invasive surgery,” Dr. Frank notes. In addition, the treatment works best on the [cheeks](https://aedit.com/concern/cheek-volume), around the mouth, and [under the chin](https://aedit.com/concern/neck-and-jawline-definition), since these areas are where loose skin tends to accumulate. 

As we mentioned, the procedure is currently only FDA cleared for [Fitzpatrick skin types I to IV](https://aedit.com/aedition/guide-to-fitzpatrick-skin-types). “A separate clinical study is needed to evaluate the proper protocols for using Ellacor™ on darker skin types,” Dr. Waibel notes.

## **How Micro-Coring Works**
It may seem unfathomable, but, in 2022, we are at the point where doctors can remove minimal amounts of loose skin without the need to cut with a scalpel. 

Before an Ellacor™ treatment starts, [local anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia) is injected into the area (topical anesthetic may also be used) to make the procedure more comfortable. Next, a small handpiece is placed over the desired location, creating cores in a grid formation (similar to energy-based treatments). The hollow needles within the handpiece quickly go to work and pierce out microscopic amounts of skin (less than half a millimeter). “With each excision, the tissue is removed from the needles through an integrated suction system, treating a 10 millimeter square area in less than three seconds,” Dr. Waibel says. 

Your doctor can adjust the depth of the treatment depending on your needs. There is pinpoint bleeding at each core site, which quickly resolves. Not all skin that falls within the coring square is removed. “After removing the excess skin, the body's natural healing response goes into effect as the dermal tissue quickly realigns to close the cores, resulting in improved skin architecture,” Dr. Waibel shares. As Dr. Engelman explains, removing the skin in this controlled fashion stimulates the wound-healing response that increases collagen production, ultimately promoting younger, healthier-looking skin.

## **Micro-Coring Recovery & Results**
A series of three 30-minute treatments are usually recommended to achieve optimal results, though many patients report seeing benefits after the first appointment. “Micro-coring results are more subtle than facelift surgery since each session of Ellacor™ only removes up to eight percent of the treated skin area,” says [Siamak Agha, MD](https://aedit.com/provider/siamak-agha-md-34668), a board certified plastic and reconstructive surgeon in Newport Beach, CA. “Less skin is removed than what a facelift can offer, but, with Ellacor™, that skin removal can be increased substantially without any scars.”

According to the clinical trials, each column of skin is selectively targeted and heals within an average of three days. But, of course, expect some scabbing, redness, slight [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox), and mild [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery). “The redness of the treated areas may last 30 days or more, signifying deeper tissue healing and tightening,” Dr. Agha says. Because the skin is sensitive and somewhat compromised after treatment, [a simple, bland skincare routine](https://aedit.com/aedition/post-procedure-skincare-makeup-product-guide) (similar to what is used post-laser or surgery) is often best. Once the skin is fully healed and the resulting holes from the cores are closed up, you should be safe to resume wearing makeup and following your regular skincare regimen.
 
One of the significant advantages of a micro-coring treatment over something like a facelift is the low risk of [scarring](https://aedit.com/aedition/how-to-treat-every-type-of-scar). “Due to the tiny needle used in micro-coring (22 gauge), the scars are microscopic and therefore not visible to the naked eye,” Dr. Engelman says. For reference, [needles](https://aedit.com/aedition/guide-to-needles-used-for-cosmetic-injectables-botox-filler) used to administer neurotoxins (read: Botox®) are usually 30 or 32 gauge. A [filler syringe](https://aedit.com/aedition/guide-to-injectable-techniques-needles-for-botox-filler) is 27 or 30 gauge. Higher numbers indicate smaller needles.  

## **FAQs: Micro-Coring Edition**
Now that you have a better understanding of how micro-coring technology and Ellacor™ treatments work, it’s time to get to some of the questions you no doubt have about what it’s truly capable of. We answer a few of the most common below:

### **1. Is Micro-Coring Anything Like Microneedling?**
We’d understand if, at this point, you are wondering what (if any) relation micro-coring has to microneedling. As it turns out, the only similarity between the two is that both modalities have the prefix ‘micro’ in their names, and both employ needles to some degree. As Dr. Waibel explains, [microneedling](https://aedit.com/procedure/microneedling) uses tiny needles to injure the skin, and there's no tissue removal — the tissue is only temporarily displaced — during the procedure.

The needle depth of a professional microneedling treatment is anywhere from 0.25 millimeters to 2.5 millimeters. The hollow needles used for micro-coring can penetrate up to 4 millimeters to remove the microscopic columns of skin. While microneedling safely creates micro-injuries in the skin to incite collagen production and decrease the appearance of acne scars, fine lines and wrinkles, and sun damage, it does not excise any tissue.

### **2. Is Micro-Coring a Replacement for a Facelift?**
A [traditional facelift](https://aedit.com/aedition/popularity-of-facelift-cosmetic-surgery-for-aging-sagging-neck) does more than just remove loose skin. It also addresses the underlying muscles and repositions fat and tissue for more significant support and a fully rejuvenated yet (hopefully) natural look. Every doctor we spoke to for this story agreed that micro-coring is not a substitute for a facelift, though it may be a suitable alternative for those who are not ready for surgery but desire some skin tightening and wrinkle reduction on the lower face.

Micro-coring acts on the skin alone. So, while the results of the minimally invasive procedure can be significant, it does not address the fundamental structure of the face and cannot provide a facelift-like result. But that’s not to say it doesn’t have value. “I think micro-coring is going to offer a lot more than other minimally to non-invasive treatments like [Ultherapy®](https://aedit.com/procedure/ultrasound-skin-tighteningultherapy), Thermage®, and other forms of [energy-based microneedling](https://aedit.com/procedure/microneedling-with-radiofrequency),” Dr. Frank says. “With micro-coring, we are going to fill that niche between non-invasive and invasive plastic surgery.”

### **THE SIMILARITIES**
- **Skin Removal**: Both a facelift and micro-coring can remove loose skin, however, the amount of skin laxity addressed through a non-surgical treatment is less than surgery.
- **Smoothing**: Micro-coring and facelifts improve wrinkling on the lower part of the face. Of course, what can be achieved with surgery is always greater than less invasive options.
- **Patience**: The results can take time to settle and fully reveal themselves after both a facelift and micro-coring, though there's more of a noticeable improvement right away post-facelift. 

### **THE DIFFERENCES**
- **Comprehensiveness**: Micro-coring does not correct or address anything below the skin like fat, tissue, and muscle. Only a facelift can target those underlying structures of the face.
- **Lift**: Whereas a facelift physically lifts the skin and requires tension, micro-coring induces collagen remodeling and firming through micro-injuries. The chances of having an unnatural result are lessened.
- **Scarring**: The tradeoff for a facelift is the [scarring](https://aedit.com/aedition/scarring-after-facial-plastic-surgery), though board certified plastic surgeons are trained to conceal the incision. There are no cuts, incisions, stitches, or scars with micro-coring.
- **Timing**: Two to three sessions of micro-coring are necessary to achieve full results. Dr. Engelman says patients typically start to see results of 60 to 90 days after the procedure, and there is usually significant improvement after just one treatment. A facelift is one and done, with results lasting a decade or more. 

Micro-coring is far less invasive than a sugical facial rejuvenation procedure such as a facelift. But, just because it's less invasive, doesn't mean it is fool-proof. As with all aesthetic procedures, a full consultation before the treatment is necessary and there is always the [risk of adverse effects](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications).

### 3. **Micro-Coring vs. Facelift: Which One Is Better?**
There's no right or wrong treatment here. What checks of all your [skin-tightening](https://aedit.com/aedition/best-nonsurgical-skin-tightening-procedures) and rejuvenating boxes may not do the same for someone else. For patients who don't have the time – or, should we say, downtime – for surgery or don’t want it (for whatever reasons), micro-coring is perhaps one of the most promising alternatives available. 

With that said, Dr. Engelman reminds us that no procedure (surgical or non-surgical) is without strings. There is still anesthesia involved, your skin will require a few days to recuperate, and there is no instant gratification. “Downtime is minimal, but it will take some time for the results to be visible,” she shares. And be sure to keep your expectations in check. The results “will be more subtle,” she adds.

### **4. Can Micro-Coring Delay the Need for Surgery?**
Given the array of non-surgical modalities available today, it may be possible to delay the need for facial surgery with the right combination of treatments. But will Ellacor™ alone allow you to sidestep a facelift in the future? Probably not. “Micro-coring does not offer the same results as more invasive techniques, like facelifts,” Dr. Engleman says. “So, in that regard, it cannot replace these more dramatic techniques.” That’s not to say it doesn’t have it’s place though. “It is a great alternative for people seeking more natural-looking results with less invasive methods,” she shares.

## **The Takeaway**
Micro-coring is a new skin-eliminating treatment that is the first of its kind, which means there is still a lot to learn. Will it replace the standard facelift? Probably not – at least not right now – but it gives both patients and providers a lot to be excited about. “There is absolutely no doubt that it is going to offer a new category of treatment in between totally non-invasive and totally invasive procedures,” Dr. Frank says. “It will help fulfill that growing niche of patients that want to do something significant in terms of wrinkling and skin tightening but are not yet ready or willing to go through more invasive techniques.”

During the cosmetic surgery consultation process and again before you enter the operating room, your plastic surgeon will explain the [side effects and complications](https://aedit.com/aedition/guide-to-cosmetic-surgery-complications) that can occur after a facelift. On the side effect front, bruising and swelling can be expected and usually begin to subside after the first couple of weeks. On the complication front, [hematoma](https://journals.lww.com/prsgo/Fulltext/2021/08000/Methods_for_Minimizing_Bleeding_in_Facelift.21.aspx#) – a word that means ‘collection of blood’ – occurs after an estimated 0.2 to 8 percent of rhytidectomy (read: [facelift](https://aedit.com/procedure/facelift-rhytidectomy)) surgeries, making it the most common postoperative complication of the procedure. 

Without proper attention, hematoma can negatively impact the [healing and recovery process](https://aedit.com/aedition/facelift-recovery-healing-process). So, whether you're preparing for surgery or just got home, what should you keep an eye out for? Here’s what you need to know about the causes, symptoms, and prevention of hematoma after a facelift.

## **What Causes a Hematoma?**
Hematoma occurs most often in the first 24 hours after surgery, says [Mark M. Hamilton, MD](http://www.hamiltonfps.com/), a double board certified facial plastic and reconstructive surgeon in Indianapolis and board member of the American Academy of Facial Plastic and Reconstructive Surgery ([AAFPRS](http://www.aafprs.org/)). The reason for this is that, “during a facelift, small blood vessels are disrupted in the process of lifting and tightening the skin,” explains [Samuel Lin, MD](https://www.linplasticsurgery.com/about/dr-samuel-lin/), a board certified plastic and reconstructive surgeon and associate professor of surgery at Harvard Medical School. That disruption can cause a pooling of blood beneath the skin but *outside* of a blood vessel.

As your surgeon is wrapping up your facelift, they will check to make sure that the bleeding has stopped. But, in rare cases, “bleeding may restart or unnoticed bleeding may continue after the surgery ends,” Dr. Lin notes. The collection of blood that results becomes a hematoma. Some of the most noteworthy risk factors for bleeding and bruising after a facelift include:

- High blood pressure
- Taking blood thinners (including [herbal medication](https://aedit.com/aedition/vitamins-supplements-to-avoid-before-cosmetic-surgery), ibuprofen, and the like)
- [Smoking](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health)
- [Combining it with a neck lift](https://aedit.com/aedition/pairing-facelift-neck-lift-procedures)

Additionally, male patients are at a higher risk of hematoma than females because they have more blood vessels and a stronger blood supply in their faces (learn more in our [comprehensive guide to facelifts for men](https://aedit.com/aedition/facelifts-for-men)).

## **Symptoms of a Hematoma**
While [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox) is common after a facelift, patients should be aware of the difference between a hematoma and a standard bruise. After all, hematomas often look like very dark bruises. There are, however, a few key differentiators to keep in mind. A hematoma is a firm and [swollen](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) red, black, or blue lump that can grow in size over time. It may or may not be painful. On the contrary, a bruise is usually flat and slightly tender – though not hard – to the touch. In general, bruises are much less serious than hematomas.

If you suspect you may have a hematoma, contact your surgeon as soon as possible. “Smaller hematomas may be drained in the office,” Dr. Hamilton says, though they can also require a return trip to the OR “to remove the clot and control any bleeding.” In extreme cases, a hematoma can “expand and put pressure on the airway, interfering with the patient’s ability to breathe,” Dr. Lin cautions. 

## **How to Prevent a Hematoma After a Facelift**
The fact of the matter is, hematomas are not 100 percent preventable. But there are a few steps patients can take to reduce the likelihood. The first is establishing that you are in [good overall health](https://aedit.com/aedition/why-you-should-lose-weight-before-plastic-surgery). Halting any medications that can impair the body’s natural clotting mechanism – including anticoagulants like Coumadin and Plavix – for at least two weeks before surgery is a must, says [P. Daniel Ward, MD](https://www.wardmd.com/), double board certified facial plastic and reconstructive surgeon in Salt Lake City. He notes that if a patient is on those two meds, they may not actually be healthy enough for surgery to begin with. This assessment should occur during the consultation process. 

In his practice, Dr. Hamilton tells patients to avoid alcohol for at least one week before surgery and to quit smoking for at least one month prior, as the former thins the blood and the latter can cause bleeding and impact the healing process. In case it’s not obvious, laying off both post-op will only help your [facelift recovery](https://aedit.com/procedure/facelift-rhytidectomy/recovery).

There are also steps that can be taken after surgery to mitigate the risk of both hematoma and bruising. If provided, you’ll want to wear a gentle [compressive dressing](https://aedit.com/aedition/benefits-of-compression-garments-after-cosmetic-surgery) or strap as per your surgeon’s guidelines. Additionally, you’ll be advised to avoid activities that raise your blood pressure – such as [exercise](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures). Since hematomas are most likely to occur within the first 24 hours, getting as much rest as possible and keeping any movement mindful will be key. 

## **The Takeaway**
Hematoma is the most common complication patients experience after a facelift, and, as with any procedure, risks and rewards must be weighed. While every case is unique, Dr. Ward agrees that there usually isn’t a reason for healthy individuals to skip out on the surgery for fear of hematoma. Instead, patients should feel empowered to call their surgeon if they have concerns about how they are healing. “We’re doing this to help care for our patients and make them feel better – we don’t want them to suffer,” Dr. Ward says. “Call us and make us earn our keep.”

Even so, that wide-awake look is within reach. This area requires a hybrid approach to target both the skin and the skin tone. Reach for a concealer to lift the brow and eye area rather than a foundation, as a method of highlighting the bone structure without the use of a highlighter. “Apply a thin line of concealer from the outermost corner of the eye and draw the line up towards the temple,” Muca says. “This creates the illusion of an eye lift and makes your eyeshadow look snatched.”  

For those (designer) [under-eye bags](https://aedit.com/aedition/cosmetic-procedures-for-under-eye-rejuvenation), the upward motion is also in play. “We have ditched applying our concealer by creating a triangle shape under the eye and instead opt for light dots — a little goes a long way,” she adds. Kate Pymm, a Trinny London makeup artist, suggests honoring the natural hollow between the eye and cheek to prevent under-eye concealer from looking unnatural. If you’re looking for a smokey eye or that perfect cat wing, don't feel the need to ditch the darker hues all together — just maintain a light-hand. “Avoid heavy products in general and prioritize lightweight formulas,” Pymm says. 

## **For Contoured Cheeks**
To create a plump cheek that mimics the fullness [filler](https://aedit.com/aedition/cosmetic-procedures-to-define-cheekbones) may provide, it’s all about the shadows. “On the tops of your cheekbones, use our ‘Face Ombre Technique,’” Muca suggests. The technique is as follows:

4. Last but not least, Muca says to blend the formulas together in “upward” strokes. “This will allow for the product to follow the natural shape of the face,” she says.

If using a contour stick, a trick is to blend it in evenly without that streaky giveaway. To do so, apply setting spray onto your brush and tap onto the contour stick before proceeding to your face. This will give you more control over your distribution and let you gently build rather than blending out. “Be clever with placement and how you use the product,” Pymm says. Her tip is to try using bronzer to sculpt rather than bronze. “It’s like an optical illusion, which is great for achieving that sculpted look without doing anything drastic,” she adds. 

## ** For a Snatched Jawline** 
For the jaw, focus on creating a seamless look that flows from neck to brow to guarantee an even complexion. “Before foundation, apply a cream contour to the areas you wish to sculpt, such as your cheekbones and along your [jawline](https://aedit.com/aedition/non-surgical-procedures-for-jawline-definition),” Muca explains. “Then, apply foundation on top, and your cream contour will pull through for a stunning ‘under-painted’ effect.” The grand finale: “Finally, sweep the powder contour across the areas you have cream-contoured for a seamless finish.”
 
When in doubt, try the ‘three’ technique, which she says suits most face shapes. “Using your contour, draw a three shape from forehead to cheekbone to jawline,” Muca explains. “We like to keep our contour light and fresh to lift and not hollow the structure of your gorgeous face.” 

## **What to Avoid**
While there is no specific product to avoid if you have a look that you want to achieve (we see you, smoky eye), the technique that you use is what will get you there. “If you want to achieve a lifted look, do not drag contour, highlight, blush, or bronzer down the face,” Muca cautions. Because the goal is a lift, the application must lift upwards as well. Muca’s tip is to “apply your products to the highpoints of the face and use gentle, upward strokes for a beautiful, lifted finish.”
 
When it comes to makeup, an update is never a bad thing. “We get comfortable with what we liked when we were younger and tend to stick to it but that’s what makes us look dated,” Woodall says. Her advice is to make your products work for the skin you have versus holding on to a look that may have worked in the past. 
 
A parting word from Muca? “A common mistake people make when trying to sculpt the face is applying too much product,” she laments. “A little bit goes a long way.”

Guys in Hollywood and beyond are no strangers to going under the knife, and many have been electing to get facelifts for years. Yes, the facelift is a traditionally female-dominated procedure. “Generally, many men are under the impression that it is vain to undergo surgery or that it is only for women,” shares [Jonathan Cabin, MD](https://aedit.com/provider/jonathan-cabin-md-956), a double board certified facial plastic and reconstructive surgeon in Arlington, VA. “This stigma of [male plastic surgery](https://aedit.com/aedition/what-you-need-to-know-about-male-plastic-surgery-trends) or the idea that men should somehow not care about their cosmetic appearance seems to be going away slowly — but it persists.” 

Even so, more men are joining the ranks of [facelift](https://aedit.com/procedure/facelift-rhytidectomy) patients than ever before. The thought is that as neuromodulators, fillers, and supercharged skincare become more routine, they serve as a gateway to more invasive plastic surgery procedures. “Once individuals are comfortable with [injectables](https://aedit.com/aedition/guide-to-injectable-filler-botox-for-men) as a method of enhancing their appearance, they ultimately realize the limitations of those procedures and become comfortable with pursuing more advanced facial surgeries such as facelifts,” says [Oren Friedman, MD](https://aedit.com/provider/oren-friedman-md-1609), a double board certified facial plastic and reconstructive surgeon and director of facial plastic surgery at the University of Pennsylvania.

With that in mind, it’s time to get real about what men can expect from a facelift, from who is a candidate to how to ensure the most natural-looking results. Read on for a complete guide.

## **The Ideal Candidate For a Men’s Facelift**
Let’s face it, everyone wants to look like the best and most rejuvenated version of themselves, and Dr. Friedman says that men and women alike are concerned about how they look as they age. “Our patients mostly all want to look very natural after any surgical intervention,” he notes. 

Generally speaking, women seek a facelift to correct an aging face *and* neck, while men primarily focus on the neck. They are also often concerned with the possibility of resulting scars, says [David Abramson, MD](https://aedit.com/provider/david-abramson-md-137), a board certified plastic and reconstructive surgeon in New York City. Men who ‘need’ a facelift have jowling and heaviness in the lower face and excess skin on the neck. The kicker? They need to have enough hair to hide the ensuing scars. “This is especially the case if the patient wears their hair short,” he shares. “However, if he is completely bald, it’s challenging to hide those scars.”

Those patients are “not an ideal candidate for the procedure,” Dr. Abramson notes, adding that it’s not standard practice to place scars on or inside the ears. For this reason alone, facelifts on men can be more challenging to perform than women. “Of course, there are trade-offs to a facelift, and there are other procedures and treatments we can do, like strictly neck procedures,” he adds. But a [facelift without a neck lift](https://aedit.com/aedition/pairing-facelift-neck-lift-procedures) is like eating a hamburger without a bun — it’s doable but rarely as good as the combination.

For the most part, the steps involved in facelifts for both men and women are the same, but the incision placement is different. “There are some minor technical differences between male and female facelifts,” Dr. Friedman shares. “But, in general, the goal is natural-looking results without changing the identity of a person and minimal downtime.”

As we touched on, there are also differences in what men are usually looking to address with their surgery. - **Turkey necks & neck laxity affect men**: Men are usually concerned with the fat that dangles from the front of their neck that may cause them to need a bigger shirt size. “Getting rid of the turkey waddle and naturally tightening the neck is what most men want to achieve with a facelift,” Dr. Friedman shares.

## **Achieving a Natural Result**
One thing that men stress that they don’t want is a tight, pulled look. “They don’t want to appear like they’ve had surgery, nor do they want to look feminized,” Dr. Cabin says. He says achieving a natural result starts with good surgical technique. “It's about using a combination of a deep plane lift, tension-free skin closure with carefully hidden incisions around the ear and hairline, and repositioning the skin and soft tissue at a natural angle,” Dr. Cabin explains. “Otherwise, the face can look windswept or unnatural.” 

Most men don’t realize that it’s possible to achieve such a natural  — and masculine — result. “By taking all tension off of the skin and lifting the deeper layers of the face, [deep plane facelifts](https://aedit.com/procedure/deep-plane-facelift) achieve more natural results with a reduction in scarring risk,” he adds.

There’s also the [skincare component](https://aedit.com/aedition/guide-to-mens-grooming-skincare-routine) to consider. Not all men care for their skin the same way that women do. “Sure, there are different groups of men who have different skincare habits, but, from my experience, most are not big followers of skincare unless they’ve always been like that,” Dr. Abramson shares.

Environmental and lifestyle choices, like sunbathing and [smoking](https://aedit.com/aedition/impact-of-cigarette-smoking-vaping-on-skin-hair-teeth-health), impact the results more than forgetting to apply moisturizer or bypassing [vitamin C](https://aedit.com/aedition/guide-to-topical-vitamin-c-in-skincare). For patients with poor skin quality, your surgeon may suggest a resurfacing laser during the facelift for more comprehensive rejuvenation.

## **A Clean Cut**
Most (though not all) men who get a facelift wear their hair shorter, so, of course, visibility of scars is always a worry. Dr. Friedman says facelift incisions are hidden differently in men than women because of bearded facial skin. Once healed, the scars should be a barely detectable fine, white line or even look like a tiny wrinkle.

## **The Results**
Nothing is permanent — not even a facelift. “It’s important for facelift patients to understand that the procedure is like a reset button, and you will continue to age,” Dr. Abramson advises. The changes in the nasolabial fold region happen “when the tissues of the cheek start to descend and you lose volume of the upper lip,” notes Stafford Broumand, MD, a board certified plastic and reconstrucive surgeon and founder of [740 Park Plastic Surgery](https://plasticsurgeonsnyc.com/). “It looks like a deep fold or wrinkle.”

## **How Nasolabial Fold Treatments Work**
Whether you’ve always felt like you have prominent nasolabial folds or you just started to notice your laugh lines, don’t be discouraged. There are both non-surgical and surgical treatment options to rejuvenate the nasolabial folds (and surrounding features), and what works best will depend on your age, anatomy, and aesthetic goals. Below, a few of the most effective:

### **1. Dermal Filler**

As he explains, the loss of muscle and tissue support from the platysma leads to jowls, low-strung platysmal bands, and a blunted, obtuse neck angle. “Part of the goal of a [neck lift](https://aedit.com/procedure/platysmaplasty) is to sew the platysma back together to define the cervical mental angle,” he shares. “Additionally, in a [SMAS facelift](https://aedit.com/procedure/smas-facelift), the upper portion of the platysma, which extends onto the face, is tightened to restore youthful muscle position.”

### **4. Technology**
And then there is the fact that we are all glued to our phones. So-called [tech neck](https://aedit.com/aedition/how-to-treat-prevent-tech-neck) causes pain and wrinkling from bending our heads to look at computers, tablets, and smartphones, says [Mark M. Hamilton, MD](https://www.hamiltonfps.com/), a board certified facial plastic and reconstructive surgeon in Indianapolis. “This downward tilt of the face and neck leads to early and excess wrinkling in the lower neck and chest,” he adds. 

But that’s not the only way technology is affecting the area below the jaw. The '[Zoom effect](https://aedit.com/aedition/how-to-treat-prevent-tech-neck)' is alive and well, says [Adam Kolker, MD](https://www.drkolker.com/), a board certified plastic and reconstructive surgeon in NYC. People have spent the last 18 months seeing themselves on-screen with jowls, neck folds, and creases they may not have noticed IRL. “With cameras positioned too low, this often prompts emergency calls to a plastic surgeon, but these effects are not permanent,” he explains. So perhaps limiting screen time isn’t such a bad idea after all, especially if it has an anti-aging benefit. 

## **Combination Procedures: A One-Two Punch**
When rejuvenating the face and neck area, it’s hard to treat both features with just one procedure, especially if the face and the neck show evidence of loose skin, displaced or excess fat, and poor muscle and tissue support. On their own, a facelift surgically corrects signs of aging on the face from the jawline up to the lower or midface. A neck lift, meanwhile, tightens skin from the jawline down, while removing redundant fat and tissue.

Hands down, a neck lift is a tried-and-true method for achieving a super tight, smooth neck. Most surgeons incorporate a neck lift into every facelift they do. “The vast majority of patients that need neck correction also need lower facial rejuvenation,” Dr. Hamilton shares. “Thus it is rare for a surgeon to recommend for a patient to select a neck lift alone.”

Yet, the decision to pair a facelift into a neck lift comes down to the best approach for you and your concerns. “We often perform facelifts and neck lifts contemporaneously as the face and neck are married by the aging jawline," Dr. Kolker says. Whether the face and neck are aging at the same or different paces, addressing one without the other will distort the outcome. “If one area slightly outpaces the other but both areas show signs of aging, lifting only one can be like painting one room in an old house,” he cautions. “The room right next to it will look like it needs a fresh coat even more.”

Simply put, correcting just the face and not the neck (or vice versa) highlights any imperfections, and the signs of aging will start to become more noticeable than before. “For most patients, addressing the neck without addressing aging in the lower face creates a notable difference between the appearance of the two, which is why most surgeons typically recommend rejuvenating both the face and the neck,” states Dr. Hamilton.

The criteria for considering a neck lift with a facelift:

- Your neck and face both make you feel and look older than you are
- The neck doesn’t match the face in terms of tautness or vice versa
- You don’t want to undergo multiple procedures in the future
- There is sagging in the lower face, under the chin, and on the neck

Excess fullness and skin in the area under the chin and jaw is what Dr. Vila looks for in a neck lift patient. In the case of a facelift, “the signs are jowling and a deflated look to the face,” he adds. A lack of distinction between the jawline, chin, and neck area is usually the final determining factor to perform both procedures. “Though patients may consider the two procedures to be completely separate, I view them on the same spectrum,” he shares. “It’s just that different patients require different components, and some patients require all of them.”

Rarely are there instances when only a facelift is necessary — though they do exist. “If there is facial laxity and soft tissue descent to the jawline region with minimal cervical laxity or deformity, no neck lift is required,” Dr. Kolker explains. “Most often, by resuspending the SMAS and platysma, the muscle that covers the lower two-thirds of the face and most of the anterior neck, mild neck contour issues and laxity are often corrected.” But, if moderate or severe laxity is not corrected, chances are you'll be back in the operating room to fix the now more prominently aging neck.

Sometimes, a preventative approach to aging uses [Botox® to diminish](https://aedit.com/aedition/where-to-use-botox-from-head-to-toe) prevalent neck bands or [filler along the jawline](https://aedit.com/aedition/non-surgical-procedures-for-jawline-definition) to create definition (i.e. add more distinction between it and the neck). Even so, these areas still tend to require surgical intervention, albeit at a later date. “After a facelift, [Botox® and filler](https://aedit.com/aedition/guide-to-facial-fillers-fat-grafting-after-facelift) are useful in maintaining the surgical results,”  Dr. Shafer shares.

Like other surgical procedures, the facelift-neck lift combo has evolved. “Over the years, we have learned to do two important things,” Dr. Unger says. The first involves [facial volume](https://aedit.com/aedition/best-procedures-to-restore-facial-volume). “One is almost always to [add fat to the face](https://aedit.com/aedition/guide-to-facial-fillers-fat-grafting-after-facelift) to replace areas that have lost volume due to the aging process,” he explains. “By adding volume, we can re-elevate areas of the face that have fallen for a more balanced and harmonious look.” 

The second lesson has to do with technique. Surgeons now take what Dr. Unger calls “a more anatomic approach” to facial rejuvenation. “Instead of just pulling on the skin and trying to make things tighter, we now focus on re-creating the natural contours of a more useful face by rearranging the anatomy on a deeper level than the skin,” he explains. “This creates a more natural appearance for longer-lasting results.”

## **The Takeaway**

For those who fall somewhere in between, wanting a natural-looking lift and an increase in [collagen](https://aedit.com/aedition/guide-to-collagen-in-skincare-supplements-aesthetic-medicine) (the youth-preserving protein), a [PDO thread lift](https://aedit.com/procedure/pdo-thread-lift) may be the Goldilocks option.

[Thread lifts](https://aedit.com/procedure/thread-lift) have humble beginnings. They fell out of favor in the early 2000s due to their fragility, visibility under the skin, and lackluster results. The answer is not a simple yes or no, and we’ve gone to the experts to understand the pros and cons of the PDO thread lift.  

## **What Is a PDO Thread Lift?** 
“A PDO, polydioxanone, thread lift is a procedure that uses dissolvable sutures to rejuvenate and lift sagging skin,” explains [Julius Few, MD](https://aedit.com/provider/julius-few-md-15580), a board certified plastic and reconstructive surgeon and founder of the Few Institute. It is one of three types of sutures commonly used in thread lift procedures — the other two types are made from [poly-L-lactic acid (PLLA)](https://aedit.com/procedure/silhouette-instalift) and polycaprolactone (PCA). 

When it comes to choosing PDO, PLLA, or PCA threads, Dr. Few says it really depends on the needs of the patient and their aesthetic goals. “PDO threads are better at repositioning and revitalizing tissue, while PLLA and PCA threads are better at lifting sagging parts of your skin,” he notes. These threads vary in length, diameter, size, and strength, says [Cheryl Karcher, MD](https://aedit.com/provider/cheryl-karcher-md-4323), a board certified dermatologist in New York City, which all impact the lifting process. 

As she explains, “very fine threads would be used in smaller facial areas,” such as around the eyes or the lips. “Medium strength and length threads can lift the jawline, the marionette area, or the lines around the mouth and chin,” she says. Other use cases? Lifting the nasolabial folds and decreasing the wrinkles in the neck, she continues. Last but not least, “wider and stronger threads have been used to address laxity in areas such as breasts, [sagging knees](https://aedit.com/aedition/best-cosmetic-procedures-for-knees), and other body parts that may need a mild lift,” Dr. Karcher adds. 

## **How PDO Threads Work**
If you’ve seen videos of thread lifts on social media, the process of threading might look a little graphic. In reality, it is a relatively painless and quick procedure. “A small amount of [local anesthesia](https://aedit.com/aedition/everything-you-need-to-know-about-anesthesia) is inserted into the area where the threads will be placed to lift,” Dr. Karcher says. “The threads are then inserted, gently tightened so that the tissue is gathered, and gently pulled with the excess thread cut.” The threads have little ridges that allow for the thread to act as an anchor and grasp the skin as it is pulled. 

Externally, the use of these threads gives the skin the appearance of an immediate, albeit subtle, lift. Internally, the threads stimulate the body’s natural collagen production. They break down over the course of a few months (think: three to six), but the collagen remains. Depending on what the patient is hoping to accomplish, the thread lift may be paired with other facial rejuvenation treatments. 

Dr. Few, for example, has developed his own approach for delivering more comprehensive and longer-lasting results. “The Inverso Suspension technique is a non-surgical treatment that I developed to incorporate non-surgical thread lifting with the restoration of lost volume in the jawline with [micro Botox®](https://aedit.com/aedition/aquagold-botox-microdosing-facial) or [Xeomin®](https://aedit.com/aedition/comparing-neurotoxin-botulinum-type-a-injections-botox-dysport-jeuveau-xeomin) to the frown muscles of the mouth to create a graceful appearance that is not ‘done’ or unnatural in nature,” he explains. “The results of this approach may last two years or more.”

## **The Right Candidate for PDO Threads**
While the benefits of thread lifts are promising, there are limits to what they can achieve. “The ideal thread lift candidate is usually in their [late thirties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-to-consider-in-your-30s) to [early fifties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-to-consider-in-your-50s-60s-70s),” Dr. Few says. “Most patients over the age of about 55 will benefit more significantly from facelift surgery.” With that said, if you are of facelift age but not able to go under the knife, a thread lift can be an option. “Thread lifts can provide a facelift alternative for older patients who are unable to have surgery for medical reasons,” Dr. Few shares. Because they can be performed under local anesthesia, “many people who have age-related conditions that make them ineligible for surgery — like high blood pressure, type two diabetes, and cardiovascular disease — can safely have this treatment,” he adds.

## **PDO Thread Lift Recovery & Results** 
From procedure time through recovery, thread lifts are among the most expedited facial rejuvenation options. “One of the best things about this treatment is it only takes about 30 minutes,” Dr. Few says. “After that, you’re free to go back to work or whatever else you had scheduled for the rest of your day.” Post-procedure, patients may be advised to limit [rigorous exercise](https://aedit.com/aedition/when-to-start-exercising-again-after-cosmetic-surgery-procedures) and activity for about a week, but it’s usually possible to return to work immediately after. [Swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) and [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox) may also occur depending on the patient. 

Now for the most important part: the results. As we mentioned, thread lifts have both short- and long-term effects that allow the visible results to keep on giving. “Another advantage is the results are seen immediately,” Dr. Karcher says. While the benefits of the collagen production take several months to show up, the lift from the threading is instant. The results can last up to two years, though you may seek a touch up before then...

## **Yes, You Can Get a PDO Thread Lift More Than Once** 
As we mentioned, the results of a PDO thread lift can last up to two years. Due to their nature, [maintenance](https://aedit.com/aedition/how-frequently-to-get-non-surgical-cosmetic-procedures-like-dermal-filler-botox-chemical-peel) isn’t as routine as say, neurotoxin or filler, but Dr. Few notes that, at his practice, many patients come back for additional thread lifts after the initial sutures are absorbed by the body. 

If you don't find the results satisfactory, surgical intervention may be the better solution — and it’s important to find a provider who will give it to you straight. “For patients who are clear surgical candidates, PDO is not the ideal tool, surgery is,” Dr. Few cautions. “If a patient feels, after three months, they want more lift than the PDO can deliver, I would advance them to either PLLA or surgery as an enhanced lifting, rejuvenating tool.”

## **Why PDO Threads Are Not the ‘New’ Faceliift** 
Now that we understand the powers and limitations of the procedure, it’s safe to say that, while they may offer a plumping lift, thread lifts are not a ‘forever’ option for all. “It’s a cheaper and faster alternative to facelift surgery, but the results don’t last as long and aren’t as dramatic,” Dr. Few says. Even so, the right cocktail of genetics and maintenance might make thread lifts enough for some. “Because the thread lift procedure is so low-risk, patients who like the results of their thread lift can usually opt to have a fresh set of temporary sutures placed once their old sutures are absorbed by the body,” he adds. 

For those who can afford the upkeep, “this maintenance may help forestall or eliminate the need for a facelift down the road,” Dr. Few shares. Dr. Karcher echoes this sentiment. “The PDO thread lift will give the benefit of a more uplifted, tighter area without all the downsides of surgery,” she says. “This can greatly extend the time before someone gets surgery or it can eliminate the need for surgery.”

## **The Takeaway**
Few procedures work from the inside out to enhance and lift the way PDO threads do, which is what makes this modern version of the thread lift so popular. By addressing multiple causes *and* symptoms of skin again on a plethora of areas, it’s a great option for those who aren’t yet candidates for a facelift or are looking to avoid one altogether. Many things in life have an age restriction: drinking alcohol, getting your driver’s licenses, and gambling, to name a few. One thing that has never had a definitive age requirement yet has always been associated with a more mature demographic: the [facelift](https://aedit.com/procedure/facelift-rhytidectomy). Historically, the facial rejuvenation procedure counts women and men experiencing more advanced signs of aging as its typical patient. However, an increasing number of younger folks are interested in and getting facelifts in their early forties. 

This begs the question: Can you be too young for a facelift? We asked the experts. 

## **The Shift Toward Younger Facelift Patients** 
“People of all ages always want to look their best, but women in their [forties](https://aedit.com/aedition/best-cosmetic-procedures-plastic-surgery-to-consider-in-your-40s) are now feeling in top form, and they want their appearances to reflect that,” says [Andrew Frankel, MD](https://aedit.com/provider/andrew-frankel-md-275), a double board certified facial plastic and reconstructive surgeon in Beverly Hills. And then there is the influence of [social media](https://aedit.com/aedition/march-2020-study-shows-relationship-between-social-media-cosmetic-surgery). “Additionally, social media puts a premium on beauty, and this creates an added pressure for people who would otherwise not be that concerned with some early signs of aging,” he notes. 

As conversations around plastic surgery have normalized (hi, [Marc Jacobs](https://aedit.com/aedition/marc-jacobs-facelift)), we now have more access than ever to [before and after photos](https://aedit.com/aedition/importance-of-before-and-after-photos-in-plastic-surgery) and procedure information. That mainstreaming has made patients more familiar with what’s possible from both a rejuvenation and [prejuvenation](https://aedit.com/aedition/i-was-practicing-prejuvenation-before-it-was-a-trend) perspective. “Women are more aware of what is available to them to achieve their desired look,” says [Stafford Broumand, MD](https://plasticsurgeonsnyc.com/), a board certified plastic and reconstructive surgeon at 740 Park Plastic Surgery in New York City.

For many, the early forties is when the first signs of visible facial aging start to appear around the jawline, chin, and neck. “Women start to notice a loosening and hanging of the skin, as well as jowls, and sometimes crinkly, wrinkly, sun-damaged skin,” explains [Robert Guida, MD](https://www.drguida.com/), a double board certified facial plastic and reconstructive surgeon in NYC. “These signs of aging become more noticeable in thinner women or those who have recently lost weight from dieting.” Unsurprisingly, [COVID-19](https://aedit.com/aedition/aedit-survey-reveals-how-2020-altered-aesthetic-landscape) has also influenced procedure preferences. “Since the pandemic, women are noticing these facial changes more thanks to Zoom calls and social media photos, which has led to an increase in those in their early forties getting facelifts,” she says.

Just because you hit the biological age of 40 doesn't necessarily mean you are guaranteed to see age-related changes that necessitate surgical intervention. We all age at different rates, and 40 does not look the same from one person to the next. “Some women have minimal signs of aging and others appear to look much older than they are with sun-damaged, inelastic skin and laxity,” Dr. Frankel says.  

So, your biological age should not be why you have a [facelift](https://aedit.com/procedure/facial-skin-firming-and-lifting-solutions). Instead, let the degree of looseness in the skin and neck be your guide. “A facelift is not for everyone,” advises Dr. Broumand. “But when patients have early jowling, a facelift, concentrating on the neck and jawline, is the gold standard.”

## **Filler Fatigue** 
Minimally invasive procedures all have their place in the modern anti-aging routine. But, be that as it may, some patients feel they have taken those procedures as far as possible and the next step is required. When [injectables](https://aedit.com/procedure/dermal-filler-and-injectable-solutions) (think: neurotoxin and filler) and non-surgical skin-tightening treatments don’t cut it anymore, the best remaining option is often a facelift. “Patients get facelifts because they work better than Botox® or injectables for loose sagging skin of the neck, jowls, and face,” Dr. Guida says. “Often, I suggest patients start with Botox®, fillers, neck liposuction, or skin resurfacing if they are hesitant about having an operation.”

Furthermore, the over-injected, puffed-up, unnatural face is no longer in vogue. “Too many providers overfill their patients, which leads to filler fatigue through premature aging of the tissue by overextending the skin,” shares [Julius Few, MD](https://aedit.com/provider/julius-few-md-15580), a board certified plastic and reconstructive surgeon in Chicago and founder of the Few Institute. “In addition, this undesirable appearance will often lead a woman in her forties to say, ‘Enough! I want a facelift and no more filler.’”
 
Research supports the newfound reality. A [2017 study](https://pubmed.ncbi.nlm.nih.gov/27965217/) in the *Aesthetic Surgery Journal* reveals the average age of a facelift patient is declining. More than 50 percent of women who have a mini-facelift in their forties had previously had injectables, fillers, or lasers. The patients indicate that they looked four years younger after these non-invasive treatments but almost 10 years younger after their facelift.

## **The 40-Year-Old’s Facelift vs. the 65-Year-Old’s Facelift**
The fundamentals of a facelift on a younger patient and an older patient are the same, yet the execution is different. “There’s just less manipulation needed in younger patients,” Dr. Few notes. “This may seem easier in principle, but it is the opposite.” As he explains, younger patients can quickly look “overdone” and “over-pulled” if not treated carefully. “Younger patients focus on the lower face and early neck aging,” he says. They also tend to be *very* adamant that they don’t want to look ‘done’ or fake. 

Even so, Dr. Guida says the quality of younger skin can make surgeons’ lives a bit easier in the OR. “Good facelift results are easier to achieve in a younger patient because the skin still has good elasticity and tone due to less sun damage and less aging,” he explains. “And the dermis of the skin and the underlying muscle, which I tighten in a face and neck lift, are stronger in younger women and technically easier to achieve a better result.”

Any skilled plastic surgeon knows the importance of personalizing facial surgery. “I always customize the face-neck lift procedure to the patients’ needs,” Dr. Guida notes. Based on science and genetic principles, no two faces are exactly alike, which is why no two facelifts are the same. 

## **The Modern-Day Mini-Facelift**
Today’s facelifts are not like the ones of yesteryear. Gone are the over-pulled, super tight, wind tunnel-look that appears fake, narrow, and waxy. In its place are more comprehensive yet minor surgeries that garner a refreshed, ‘still-you-but-better’ look. “The techniques that we surgeons have developed over the last decade have made the results much more predictable, durable, and even faster to recover from,” Dr. Few says.
 
A modified, limited version of a traditional facelift, the so-called [mini-facelift](https://aedit.com/procedure/minifacelift) zeros in on excess skin on the neck and jaw. Younger patients almost always have less skin laxity (unless there is extreme weight loss) than older patients. In addition, the tissue in the cheek area has not [lost as much volume](https://aedit.com/aedition/best-procedures-to-restore-facial-volume) or support, and these areas do not typically require correction.

Facelifts in 40 year olds usually have a limited incision and not as drastic a pull. The incisions, which are smaller than traditional facelifts because less tissue and muscle manipulation is necessary, are made in front of and behind the ears. Dr. Frankel explains that when there is less laxity, it is even more important to release the tissues (dissect more) so that there is not too much tension on the incisions. This allows for better healing. “Additionally, most women in their forties don’t need as much skin removed, so the dissection often stays deeper,” he adds. With time, the ensuing [scars](https://aedit.com/aedition/minimizing-scars-after-surgery) will fade, so patients can still wear their hair up. 

Some surgeons, like Dr. Few, choose to [incorporate other age-rejuvenating treatments](https://aedit.com/aedition/best-procedures-to-combine-with-a-facelift) into the overall revitalization plan. “Filler, especially in the lower face and neck, should be considered as an addition — not a primary tool — for lower face drooping,” he says. “I prefer to do suture suspensions in this age group, as our research has shown near-surgical results without the need for cutting. ## **Is Younger Better?**
If a facelift sits at the top of the priority list titled ‘Things to Do in My Forties,’ there can be a benefit to going under the knife while still a quadragenarian (as long as your plastic surgeon deems you a good candidate). Having surgery early on, while age-related concerns are still relatively minor, eliminates the need for a more complete restoration down the line. Medical and surgical advances also mean a [quicker recovery](https://aedit.com/aedition/facelift-recovery-healing-process). “Patients are not down for months at a time anymore,” Dr. Broumand says. “Instead, they are back in the office seven to 14 days later.” 

And let’s not forget about the [cost factor](https://aedit.com/procedure/facelift-rhytidectomy/cost). Younger patients who have ‘mini’ lifts may incur a smaller price tag because there is less work and time involved on the surgeon’s part. “Plus, some patients opt for a lift because other modalities are not giving an extreme enough result anymore or they have become too expensive to maintain,” Dr. Broumand adds.

The results will last for years no matter your age. It’s important to remember that the face will continue to age, just from a different starting point. “The bones continue to change size and shape, the skin thins and loses elasticity, and facial fat diminishes over time,” Dr. Frankel notes. Most patients start to notice some relaxing of the skin and tissues around the 10-year post-op mark. “The face will still look better than it did before the surgery, and it will not go back to its original appearance,” Dr. Guida says. 

## **The Takeaway**
Non-surgical solutions certainly have their place, and, for some facelift patients, they continue to be part of the anti-aging plan — especially for upkeep. After all, just because you opt for facial surgery at an earlier age does not mean that you will become immune to aging — you won’t. “A facelift will not stop the clock, but it will reverse it,” says Dr. Broumand. It’s also important to recognize that additional surgeries may be needed in the future to maintain results. “Once a patient has a facelift early, they are very likely to feel the desire to do a second one — this is the warning I give any patient considering a facelift in their early forties,” cautions Dr. Few. “LiveLoveLift.” That was the hashtag heard around the world when Marc Jacobs shared a [bandaged selfie](https://www.instagram.com/p/CRmFuG4sDBG/) of himself late last month that led many to speculate that he had undergone a facelift. In true Marc Jacobs fashion, it didn’t take long for him to peel back the curtain and share the details of his surgery with the masses.

The 58-year-old fashion designer had, in fact, gotten a facelift at the hands of [Andrew Jacono, MD](https://www.newyorkfacialplasticsurgery.com/), a double board certified facial plastic and reconstructive surgeon. The New York City-based surgeon specializes in a technique known as the [deep plane facelift](https://aedit.com/procedure/deep-plane-facelift), which is one of the most invasive but also one of the most effective types of rhytidectomy. Dr. Jacono performs the procedure in a way that leaves the skin attached to the muscle to avoid tautness and create a more natural result. “Yes, you can look like yourself but just better after #plasticsurgery,” he [wrote on Instagram](https://www.instagram.com/p/CRty0maL-tv/) in response to one of Jacobs’ post-op posts. “This is why I love the #deepplanefacelift.” 

In the days since his surgery, Jacbos has chronicled his experience every step of the way. If you were already one of his 1.6 million Instagram followers, then you are used to seeing the ins and outs of his daily routine. Even so, there was something surprising — and refreshing — about watching a public figure share their aesthetic journey in real time. 

He posted a picture of himself in a [hyperbaric oxygen chamber](https://aedit.com/aedition/benefits-of-hyperbaric-oxygen-therapy-after-plastic-surgery), which is used to promote healing. We’ve also gotten several more selfies from the designer, showing the evolution of recovery. Three days post-op, Jacobs’ [wrote](https://www.instagram.com/p/CRtwy2AMR3j/) that he was “ready for lewks,” as he showed off some minor swelling and bruising.

By day six, those side effects had intensified. “Bruised. Sore. And super swollen,” he [wrote on IG](https://www.instagram.com/p/CRzEnzWMzqj/), adding that he was on his way to see Dr. Jacono for an HBOT treatment. The next day (i.e. one-week post-op), Jacobs [said](https://www.instagram.com/p/CR18QnQFHAT/) that he was “feeling the healing.” On day 10, the designer was [updating](https://www.instagram.com/p/CR9dPWtl8mK/) us all that his stitches had been removed.

We so often write about the plastic surgery healing process and how patients plan their procedures with discretion in mind. The ability to [recover](https://aedit.com/aedition/facelift-recovery-healing-process) quietly at home with undisrupted downtime factors into when surgeries are scheduled, and there is certainly nothing wrong with stepping away from the spotlight for a few weeks to heal. 

In the past, when celebrities have chosen to disclose that they went under the knife, we hear from them after the swelling has subsided and they are back out in the public eye. By letting us in on the period in between, Jacobs is pioneering a new kind of transparency and paving the way for others to speak candidly about what to expect from procedures. 

In an [interview with Vogue](https://www.vogue.com/article/marc-jacobs-plastic-surgery-face-lift-aging), the designer confirmed his intention to normalize the conversation around cosmetic surgery. As he explained, he was a longtime patient of Fredric Brandt, MD, and began seeing NYC-based board certified dermatologist [Paul Jarrod Frank, MD](https://pfrankmd.com/), after his passing. In addition to [Botox®](https://aedit.com/procedure/botox) and [filler](https://aedit.com/procedure/dermal-filler-and-injectable-solutions), Jacobs said that he started getting [thread lifts](https://aedit.com/procedure/thread-lift) “to hold up his jowls” and the area of his face “where the skin was sagging.” When the benefits became a bit too short-lived for his taste, and Dr. Frank recommended a consultation with a plastic surgeon.

After consulting with Dr. Jacono, Jacobs knew he had met his man, and he never thought twice about sharing his experience. “I’m not covering my face. I’m not wearing sunglasses. I’m not incognito. I’ve got some scars around my ears, and my face is swollen, and I have a bruised neck, but I’m still posting on Instagram,” Jacobs [told Vogue](https://www.vogue.com/article/marc-jacobs-plastic-surgery-face-lift-aging). “I don’t care. I don’t even see it as an effort to be transparent. I’m doing what I normally do, which is I’m living my life and sharing it with anybody who is interested.”

We’ve all read the celebrity interviews in which they play coy on what’s flowing into their fountain of youth, and Jacobs isn’t here for it. “It seems so crazy when an actor or public figure denies that they've done something,” he [said](https://www.vogue.com/article/marc-jacobs-plastic-surgery-face-lift-aging). “And they say like, ‘Oh, it's olive oil, I bathe in seltzer.' It's like, come on. Like all those things might be true, but they're not why your neck is tight.” Not only does this behavior perpetuate unattainable notions of beauty, it also perpetuates the notion that having work done is something to be self-conscious about.

In response to one of Jacobs post-op pictures, Dr. Jacono [praised](https://www.instagram.com/p/CRty0maL-tv/) his patient for his “transparency and authenticity” that will “give others the courage in their journey of self care to take control of their appearance.” In speaking with Vogue, Jacobs admitted he was surprised by the adulation he’s received, but he is hopeful for what it might mean for the future. “I think you just need to start a conversation, and then maybe that will have a resounding echo that helps people feel less ashamed,” he [told the mag](https://www.vogue.com/article/marc-jacobs-plastic-surgery-face-lift-aging). “I just don’t think there’s shame in being vain.”

In recent years, social and mainstream media has made the aging face a battleground, pigeonholing people into taking sides. Are you *Team Filler* (often younger and surgery-averse) or *Team Facelift* (likely older and not scared of the scalpel)?

In reality, the science (and art) of facial rejuvenation is not so black and white. Many women are discovering the downfalls of decades of dermal filler use (those chipmunk cheeks don’t come cheap), while others are disappointed when a facelift alone can’t solve their cosmetic concerns. The truth is, facial aging is a complex conundrum for which there is no single fix — quick or otherwise. To tackle the multidimensional problem, modern plastic surgeons have been increasingly combining tightening and volumizing techniques (both surgical and non-invasive) to improve facial contour.

Today, there are three main ways surgeons restore lost volume: 

1. Repositioning underlying tissues (as in a high SMAS or deep plane [facelift](https://aedit.com/aedition/popularity-of-facelift-cosmetic-surgery-for-aging-sagging-neck)) 
2. [Fat transfer](https://aedit.com/aedition/why-everyone-is-talking-about-fat-grafting) (a.k.a. autologous fat grafting)
3. Injecting [dermal fillers](https://aedit.com/aedition/everything-you-need-to-know-about-dermal-fillers) (most commonly, hyaluronic acid-based)

## **What Is Facial Aging Exactly?**
Now’s the perfect time to talk about facial aging. For starters, it’s more complicated than you might think. We tend to characterize aging as simply a losing battle against gravity (and, unfortunately, that’s partly true) but other factors, like losing facial bone mass, are involved.

“Sagging is definitely related to volume loss, but it is not entirely due to it,” says [Andrew Frankel, MD](https://aedit.com/provider/andrew-frankel-md-275), a double board certified facial plastic and reconstructive surgeon in Beverly Hills. “The elasticity of the skin and underlying soft tissues plays a role. The underlying supporting ligaments and the bony skeleton also are important factors. As the bone remodels and shrinks the overlying skin and soft tissue begin to sag because they are draping over a smaller shape.”

Simply put: your face isn’t just falling, it’s also going flat, and changing shape, and stretching (oh my). Generally speaking, a [rhytidectomy (a.k.a. facelift)](https://aedit.com/procedure/facelift-rhytidectomy) is a surgical procedure that tightens up the tissues of the lower face. Because this sagging along the jawline (read: jowling) is associated with age, a facelift is pretty much guaranteed to turn back the clock at least a few years.

Plastic surgery techniques have come a long way since the super-taut ‘70s, with aesthetic surgeons today focused more on tightening up the underlying tissues rather than just pulling on the skin. While there are many different types of facelifts — including [deep plane](https://aedit.com/procedure/deep-plane-facelift), [high SMAS](https://aedit.com/procedure/smas-facelift), and the so-called ‘[mini](https://aedit.com/procedure/minifacelift)’ — they all share the common goal of counteracting the aging effects of gravity, genetics, and lifestyle choices that lead to sagging.

## **Does a Facelift Restore Volume?**
Those of you paying close attention already know that, historically, facelifts were primarily about skin tightening. But, too often, just pulling the skin taut resulted in a stiff, angular, unnatural result. This is because stretching skin that’s lost elasticity over an aging bone structure can’t make you look 25 again. 

“Imagine a balloon that loses air and deflates,” explains [David Isaacs, MD](https://aedit.com/provider/david-isaacs-md-912), a board certified oculofacial plastic and reconstructive surgeon in Beverly Hills. “You start to see the creases, wrinkles, hollows, and the balloon starts to change shape.” When you take all those factors into account, you realize the true complexity of the aging process. “Facial rejuvenation has traditionally been approached as a vertical vector, ‘y-plane’ problem, but it's really a three-dimensional problem,” he adds.

With that in mind, you might be wondering what the modern facelift can accomplish. “A facelift elevates tissues and repositions things into a more desirable anatomic position,” Dr. Frankel explains. “It also helps to stretch wrinkles and fine lines and, therefore, makes the skin appear smoother.” 

So, what can’t it do? “A facelift does not add volume, and, oftentimes, fillers are required to soften the patients’ appearance,” he says. “If tissues are tightly stretched across two bony high points, for example, there can be some evidence of the direction of the tension or some folds develop over time in the hollow area between those high points.”

### **Adding Volume with a Facelift** 
Whether using synthetic dermal fillers or fat transfer (more on both below!), here are some of the most common areas of the face where plastic surgeons add volume in conjunction with a facelift:

- **Jawline**: “Some people have indentations on either side of their chin where the jaw narrows a bit before it becomes wider again,” Dr. Frankel explains. “These indentations can exaggerate the degree of jowls and break up an otherwise nice jawline. Placing some filler along the bone in these areas can be helpful.”
- **Lateral Cheeks**: According to Dr. Isaacs, “the high lateral cheeks” are often addressed post-facelift. “This can give more of a sweet-heart shaped face many patients desire,” he says, adding that certain facelift techniques (like a deep plane approach) may achieve those results without filler.
- **Nasolabial Folds & Anterior Cheeks**: These are common areas of concern, but they aren't dramatically affected by a facelift, according to [Jason Roostaeian, MD](https://aedit.com/provider/jason-roostaeian-md-449), a board certified plastic and reconstructive surgeon at UCLA. “A deep plane technique does help lift up the cheek a little bit, but, if their cheek is already under volumized, I’d much rather do it with their own natural tissue than with a filler,” he explains. “If somebody has really deep nasolabial folds, I do fat grafting first.”

## **Facial Filler vs. Facial Fat Grafting**
Plastic surgeons are unique in their preferences for using [dermal fillers or fat transfer](https://aedit.com/aedition/comparing-dermal-fillers-vs-fat-grafting-for-facial-rejuvenation-contouring) in conjunction with facelifts. Sometimes, it’s not an ‘either or’ situation. It can be determined on a case-specific basis and will be dependent on the patient’s anatomy and preferences. Here’s a closer look at each:

### **Filler**
Some plastic surgeons like using synthetic dermal filler (like Juvederm® or Restylane®) after a facelift. Dr. Frankel, for instance, says he favors injectable filler over fat grafting procedures because it’s more predictable, is reversible, and doesn’t carry the risks associated with harvesting from a donor site. 

**The 411 on Filler:**

- Lab-created [hyaluronic acid](https://aedit.com/aedition/guide-to-hyaluronic-acid-in-skincare-dermal-filler) (based on substance found naturally in the body)
- Temporary and generally reversible
- Predictable
- Less risk of bruising; shorter recovery period
- May clog lymphatics and cause swelling
- Injected 8 to 12 weeks after facelift surgery (once post-op [swelling](https://aedit.com/aedition/how-to-treat-minimize-swelling-after-cosmetic-surgery) resolves)
- Regular maintenance required

Filler smooths out the junctions and hollows “and also can make the edges of the bony landmarks appear less harsh,” Dr. Frankel notes. Another benefit? “Fillers can also add some height at various key points around the face to improve overall balance,” he says. But it does require a separate appointment. “Approximately eight weeks after the surgery, we reevaluate the need for filler and where it should be placed,” Dr. Frankel explains. “When the filler is injected at this stage, it provides greater projection and does not fall under its own weight. I find this to be much more effective than trying to inject lax skin.”

### **Fat Grafting**
Other surgeons feel strongly that facial fat grafting is the best way to restore volume, especially when performed in conjunction with other facial plastic surgery (think: facelifts). “If you just do a facelift and never rely on fat grafting, you’re holding back on your results significantly," Dr. Jason says. “We all recognize that loss of volume is a huge part of aging — there’s no question about that. So, why would you rely completely on filler when you have that perfect opportunity to be in the operating room and use fat at the same time?”

**The 411 on Fat Grafting:**

- Natural harvested fat cells collected via liposuction from donor site
- Longer-lasting results than synthetic filler
- Injected at the same time as facelift surgery
- May add downtime to facelift recovery due to [bruising](https://aedit.com/aedition/how-to-treat-prevent-bruising-after-lasers-filler-botox)
- Little to no maintenance required
- Less costly over time

Dr. Jason believes that using your body's own fat (which contains stem cells) leads to more natural results. “I don’t worry about overdoing somebody with fat grafting," he explains. “Filler is an unnatural substance that takes on a lot of water. At some point, if the percentage of filler starts to become equal to or more than the amount of natural tissue that you have in the area, it’s going to look swollen because it draws in water.” That’s what can lead to an unnatural appearance. “We all know when we see those faces that just look a little swollen and weird,” Dr. Jason says. “That’s from filler, not from fat grafting.”

If you are curious about [cost effectiveness](https://aedit.com/aedition/cost-effective-cosmetic-procedures-for-face-eyes-lips), a facial fat transfer procedure will likely be a larger initial investment than filler but prove to be less costly in the long run. “The beauty of fat grafting is that I am doing a broad, very comprehensive grafting to your whole face,” Dr. Jason says. And the procedure rarely needs to be touched up. “Usually, we’re injecting something like at least 10 cc of fat in a widespread area,” he shares. “That’s like $10,000 worth of filler, but you don't have to keep injecting it.”

## **Do I Need Filler, a Facelift, or Both?**
At the end of the day, our experts agree that the only way to determine whether a patient needs filler (be it fat or synthetic), a facelift, or both to meet their aesthetic goals is to consult with a [board certified](https://aedit.com/aedition/medical-board-certification-in-plastic-surgery-cosmetic-dermatology) plastic surgeon. “The degree of facial volume needed for optimal rejuvenation is highly variable,” Dr. Frankel says. 

Even so, there are some genetic and lifestyle factors that may shed some light on the [decision-making process](https://aedit.com/aedition/facelifts-vs-injectable-dermal-fillers-which-one-is-right-for-me). “In general, people who were born with a full face tend to age better, as the volume diminishes and their features become more defined,” Dr. Frankel shares. In such cases, the patient may “not need much or any additional volume” and would instead “benefit from redistributing their existing volume to achieve a more youthful appearance,” he says. 

On the contrary, if someone has chiseled features in their twenties, “they are more likely to require additional volume as they age,” Dr. Frankel notes. In these cases, it’s important to [seek a skilled injector](https://aedit.com/aedition/finding-the-right-plastic-surgeon-dermatologist-or-cosmetic-dentist) that understands the subtleties of the face. “Caution should be used when injecting these patients so as not to add too much volume or place it into areas that would change their original look,” he says.

To put it in layman's terms: A facelift isn’t a be-all, end-all solution for signs of aging around the face. This distinction highlights the utter importance of making sure you and your doctor have an honest conversation about desired results.

As we’ve learned, the best pairing for your facelift depends on your aesthetic goals, but there are some common surgical and non-surgical combos that address a wide array of cosmetic concerns. “Facelifts reposition tissues but do not replace volume lost with age, relax facial muscles, or resurface skin,” explains [Adam Schaffner, MD](https://aedit.com/provider/adam-schaffner-md-33), a triple board certified plastic and reconstructive surgeon in NYC. “It may be of benefit in some patients to combine a facelift with non-surgical or minimally invasive procedures.”

While filler and fat grafting (more on that below) can also be used to augment the chin and jawline, a [chin implant](https://aedit.com/procedure/alloplastic-chin-implant) involves placing a synthetic implant on or around the chin to alter the shape of the face.

### **5. Facelift + Fat Grafting**
“Fat transfer or using injectable fillers, such as Restylane®, Juvéderm®, or RHA® products, will help to restore facial volume,” Dr. Schaffner shares. [Fat grafting](https://aedit.com/aedition/why-everyone-is-talking-about-fat-grafting) involves taking fat from one area of the body (i.e. a donor site) via liposuction and repurposing it elsewhere. The donor site is often the thighs or stomach (basically, anywhere you have [excess fat](https://aedit.com/concern/excess-fat)), and, once processed, the fat can be used to plump the cheeks, under eyes, lips, chin, hands, and even earlobes. Unlike synthetic fillers that are temporary, [fat grafting](https://aedit.com/aedition/comparing-dermal-fillers-vs-fat-grafting-for-facial-rejuvenation-contouring) is considered a permanent solution with no risk of rejection. With that said, the final results of fat grafting can take three to six months to reveal themselves. This is the length of time it takes for the graft to become established in its new location, and, during that period, the body re-absorbs a percentage of the fat (usually around 30 to 50 percent). 

### **6. Facelift + Neurotoxins**
Another injectable that can be paired with a facelift? Sure, a facelift addresses the *shape* of the skin, but it can’t do anything about the *look* of it. Enter lasers treatments. “Lasers can be used to help resurface the skin to reduce wrinkles,” Dr. Schaffner explains.
